    Notes: The current carried by a steady-state, finite-width beam produced by space-charge-limited emission in two-dimensional parallel plate geometry is known to diverge strongly from estimates based on the classic one-dimensional Child–Langmuir problem. The two-dimensional problem presents formidable analytic difficulties, but a numerical approach to this problem has been developed. The approach simultaneously determines the electrostatic potential and the unknown current density profile. Calculations show that the total current is a function of the dimensionless ratio of beam width (w) to anode–cathode gap (d), but that the current density profile varies with both w and d separately. © 2001 American Institute of Physics.

    Notes: The objective of this research was to evaluate the outcomes and costs of alternative approaches to managing patients previously treated for peptic ulcer disease and Helicobacter pylori infection.〈section xml:id="abs1-3"〉〈title type="main"〉Materials and Methods.A decision-analytic model was used to compare (1a) urease breath testing (UBT) for assessment of H. pylori status versus (1b) observation without further testing or treatment, among patients who were symptom-free following initial antimicrobial and antisecretory therapy for endoscopically demonstrated ulcer and H. pylori infection; and (2a) UBT versus (2b) repeat endoscopy with H. pylori testing, and versus (2c) repeat antimicrobial and antisecretory therapy without further testing, among patients who remained symptomatic following initial therapy.〈section xml:id="abs1-4"〉〈title type="main"〉Results.Among patients who were symptom free after initial therapy, 6.1% receiving UBT had symptomatic ulcer at one year, compared to 18.2% of those simply observed. The expected first-year cost per symptom-free patient following initial therapy was $591 for UBT compared to $480 for observation. Among patients with persistent symptoms after initial therapy, 21% receiving repeat therapy had symptomatic ulcer at one year, compared to 23.8% receiving repeat endoscopy, and 23.3% receiving UBT. Corresponding medical costs per patient were, respectively, $766, $1787 and $1122.〈section xml:id="abs1-5"〉〈title type="main"〉Conclusions.The optimal approach to managing patients following initial treatment for ulcer and H. pylori infection depends on symptom status following initial therapy. For symptomatic patients, the preferred approach is to prescribe a repeat course of antimicrobial and antisecretory therapy. For patients without symptoms following initial therapy, UBT is the preferred approach because it is associated with a threefold lower risk of symptomatic ulcer at one year, although it costs an additional $110 per patient, compared with observation.

    Notes: This article describes the development of a generic loss assessment methodology, which is applicable to earthquake and windstorm perils worldwide. The latest information regarding hazard estimation is first integrated with the parameters that best describe the intensity of the action of both windstorms and earthquakes on building structures, for events with defined average return periods or recurrence intervals. The subsequent evaluation of building vulnerability (damageability) under the action of both earthquake and windstorm loadings utilizes information on damage and loss from past events, along with an assessment of the key building properties (including age and quality of design and construction), to assess information about the ability of buildings to withstand such loadings and hence to assign a building type to the particular risk or portfolio of risks. This predicted damage information is then translated into risk-specific mathematical vulnerability functions, which enable numerical evaluation of the probability of building damage arising at various defined levels. By assigning cost factors to the defined damage levels, the associated computation of total loss at a given level of hazard may be achieved. This developed methodology is universal in the sense that it may be applied successfully to buildings situated in a variety of earthquake and windstorm environments, ranging from very low to extreme levels of hazard. As a loss prediction tool, it enables accurate estimation of losses from potential scenario events linked to defined return periods and, hence, can greatly assist risk assessment and planning.

    Notes: Our group and others have previously reported enhancement of cutaneous wound healing following the transfection of tissue with plasmid vectors expressing the DNA for growth factors. In these experiments, growth factor treated animals were usually compared to animals treated with control plasmid vector. To achieve consistent transfection, high DNA plasmid load and repeated penetrations of the wound by needle or gene gun were required. In the current experiments, we assessed the effect of the plasmid load and repeated tissue penetrations on wound healing of excisional wounds in diabetic C57 mice. Animals received 5 mm excisional wounds, and were assigned to the following groups, no treatment, phosphate buffered saline solution injections, and plasmid vector injection with and without the keratinocyte growth factor-1 gene. Intradermal injections of 100μg plasmid were given adjacent to the wounds at days 1–5, 7 and 11. At day 9, wound closure was more advanced in keratinocyte growth factor-1 treated animals compared to those treated with control plasmid. But a detrimental effect of the DNA plasmid injection was evident from a comparison of the DNA control group versus the non-injected group. Therefore, the challenge for developing an effective system for the enhancement of wound healing lies in improving transfection efficiency.

    Notes: Depolarizing stimuli increase catecholamine (CA) biosynthesis, tyrosine hydroxylase (TH) activity, and TH phosphorylation at Ser19, Ser31, and Ser40 in a Ca2+-dependent manner. However, the identities of the protein kinases that phosphorylate TH under depolarizing conditions are not known. Furthermore, although increases in Ser31 or Ser40 phosphorylation increase TH activity in vitro, the relative influence of phosphorylation at these sites on CA biosynthesis under depolarizing conditions is not known. We investigated the participation of extracellular signal-regulated protein kinase (ERK) and cAMP-dependent protein kinase (PKA) in elevated K+-stimulated TH phosphorylation in PC12 cells using an ERK pathway inhibitor, PD98059, and PKA-deficient PC12 cells (A126-B1). In the same paradigm, we measured CA biosynthesis. TH phosphorylation stoichiometry (PS) was determined by quantitative blot-immunolabeling using site- and phosphorylation state-specific antibodies. Treatment with elevated K+ (+ 58 mm) for 5 min increased TH PS at each site in a Ca2+-dependent manner. Pretreatment with PD98059 prevented elevated K+-stimulated increases in ERK phosphorylation and Ser31 PS. In A126-B1 cells, Ser40 PS was not significantly increased by forskolin, and elevated K+-stimulated Ser40 PS was three- to five-fold less than that in PC12 cells. In both cell lines, CA biosynthesis was increased 1.5-fold after treatment with elevated K+ and was prevented by pretreatment with PD98059. These results suggest that ERK phosphorylates TH at Ser31 and that PKA phosphorylates TH at Ser40 under depolarizing conditions. They also suggest that the increases in CA biosynthesis under depolarizing conditions are associated with the ERK-mediated increases in Ser31 PS.

    Notes: The successful employment of embolization coils for transcatheter occlusion of the patent ductus arteriosus (PDA) has resulted in acceptance of transcatheter occlusion as the treatment of choice for small to medium-sized PDAs. The Duct-Occlud device was developed to further improve the technique by utilizing a controlled release mechanism and coil shape specifically designed for the geometly of the ductus arteriosus. Clinical studies have demonstrated excellent efficacy with low embolization rates and low incidences of complications. Newer modifications have been designed for occlusion of larger PDAs and subaortic ventricular septal defects, and preliminary clinical results have been promising.     Notes: This paper examines the introduction of competition into the English and Welsh water and sewerage industry following privatisation of the 10 regional water authorities in 1989. It outlines the development of comparative, capital and product market competition, arguing that the greatest opportunities now lie with the last through the introduction of common carriage agreements, the extension of Inset appointments and the introduction of transferable abstraction licences. Despite competitive innovations, the industry remains highly regulated, complex and difficult to enter. One of Ofwat's outstanding challenges for the next decade is to examine the means by which the regulatory burden might be lightened and barriers to entry lowered, to encourage potential entrants to compete with incumbents.

    Notes: The huge subsidization of sprawling development and the unbridled use of the private automobile needs to be brought to the forefront of public policy debate, not left to the excesses of the market-place. To do this we need citizens-in-the-community, not consumers-in-the-market. The present costly and unsustainable situation of our human settlements is a result of a profound failure in public policy; a failure that resulted when we left the fate of our human settlements to the vagaries of the market-place. We can no longer rely on the present subsidized, distorted, market-place system approach to designing, building and operating our human settlements. The consequences have been disastrous for human welfare, health and safety, and the natural environment upon which all life depends.

    Notes: The transport properties of modulation, shifted modulation, and uniformly doped Al0.20Ga0.80N/GaN superlattices are presented. The modulation-doped sample is doped only in the AlGaN barriers. The shifted-modulation-doped sample has its dopants shifted by one-quarter period. Measurements reveal a strong improvement in mobility and resistivity for the modulation-doped and shifted-modulation-doped structures versus the uniformly doped structure. The modulation-doped sample has a mobility of 9.2 and 36 cm2/V s at 300 and 90 K respectively and a very low resistivity of 0.20 and 0.068 Ω cm at 300 and 90 K, respectively. Capacitance–voltage profiling shows multiple two-dimensional hole gases. The results are consistent with a reduction of neutral impurity scattering for modulation-doped structures as compared to uniformly doped structures. © 2001 American Institute of Physics.

    Notes: Multi-year Instrumental records for input, throughflow and output waters of the Lilburn Cave system provide control on denudation rates as they respond to seasonal and spatial variability. Data suggest that maximum denudation is in the late fall and early winter. This is when non-snowmelt discharge is at Its maximum. At lower discharge rates the volume of water moving through the cave system Is the limiting control on the volume of denudation. During periods of snowmelt the limiting control is the rate at which the calcite dissolves. This is probably the result of water flowing through wider channels during these times. Based on instrumental measurements, there is considerable variation in terms of where denudation occurs inside the cave. The loci of dissolution change from year to year. This is to be expected in the dynamic environment of the cave where materials shift routinely. This variability should be studied over longer periods of time in order to more fully understand its extent. The relatively small area of carbonate exposure relative to the area of the drainage basin gives rise to relatively high denudation rates. The carbonate Is being removed at a rate of about 5000 metric tons per year, or at about 830 mm/y. This is about five times the rate reported in the humid karst regions of Malaysia. This information indicates that the relative proportion of carbonate in the drainage basin needs to be considered when trying to estimate denudation in other areas.
