    Notes: Fibreoptic bronchoscopy (FB) is frequently associated with a decline in Pao2, whose degree and duration can be substantial especially in infants. The effect of a face mask, which allows the administration of 100% oxygen and continuous positive airway pressure during FB, on the incidence and severity of hypoxaemia was studied in thirty-one consecutive infants. Sedation was provided by intravenous propofol titrated to allow patient comfort. A transient fall in Spo2 〈95% was recorded in 6/31 patients during endoscopy of the upper airway (lasting 1.6±1.1 min) and in 11/31 patients during endoscopy of the lower airways (lasting 1.4±1.1 min). Capillary blood gas analysis before and after endoscopy of the lower airways demonstrated an increase in the Pco2 6.4±1.3 to 7.3±1.4 kPa (49±10 to 56±11 mmHg). The risk of hypoxaemia in sedated infants breathing spontaneously is low when 100% oxygen and continuous positive airway pressure are administered during FB.

    Notes: Zum Thema Bakterielle Arthritiden sind in der Klinik und der ambulanten Praxis keineswegs Raritäten. Wegen ihres meist akuten Auftretens und der schmerzhaften klinischen Symptomatik erfordern sie rasches Handeln und stellen hohe diagnostische und differentialdiagnostische Anforderungen, natürlich auch bei der Abgrenzung zu funktionellen Gelenkbeschwerden, die in das Gebiet der Orthopädie fallen. Auch wenn sich diese Übersicht mehr mit den Grundlagen der pathogenetischen Mechanismen und weniger mit den klinischen Folgen beschäftigt, sei sie den praktizierenden Ärztinnen und Ärzten empfohlen. Die 3 prinzipiellen pathogenetischen Formen von bakteriellen Gelenkentzündungen werden hier dargestellt, die infektiöse septische Arthritis, die reaktive Arthritis und die Arthritis bei Lyme-Borreliose, die hierzulande noch nicht einmal so lange bekannt ist. Die Kenntnis der pathogenetischen Mechanismen wird die Diagnostik und Therapie bakterieller Arthritiden sicher wesentlich erleichtern.

    Notes: Abstract This collection of articles is the first of two parts of a special issue on “Neural Networks and Structured Knowledge.” The contributions to the first part shed some light on the issues of knowledge representation and reasoning with neural networks. Their scope ranges from formal models for mapping discrete structures like graphs or logical formulae onto different types of neural networks, to the construction of practical systems for various types of reasoning. In the second part to follow, the emphasis will be on the extraction of knowledge from neural networks, and on applications of neural networks and structured knowledge to practical tasks.
