    Notes: During standard graded exercise, blood lactate and blood ammonia levels increase in parallel and the two thresholds appear at the same relative intensity. The first part of this study aimed at comparing ammonia threshold to lactate threshold with five different graded exercise protocols. A significant difference between the two thresholds was found only in the continuous protocol with 4-min steps in which ammonia threshold appeared at a higher intensity than lactate threshold. The second part of the study investigated the correlation of ammonia threshold to some common physiological measurements. Correlation was significant with lactate threshold, ventilation threshold and endurance time. No correlation was found the anaerobic power, anaerobic capacity measurement and peak VO2. Ammonia threshold was also studied before and after endurance training and sprint training. The 8-week endurance training program was able to delay the appearance of ammonia threshold, but not the 8-week sprint training program. Findings in this study demonstrate that ammonia threshold is a physiological parameter that should be recognized and has potential to be applied for the evaluation of exercise capacity and the effectiveness of endurance training programs.

    Notes: The lipid content and fatty acid profile in soft tissue of the green-lipped mussel Perna viridis, a widely distributed tropical and subtropical bivalve species, were investigated. The total and neutral lipid contents were 6.17 ± 0.71 and 2.71 ± 0.42 mg per 100 mg dw, respectively. The content of docosahexaenoic acid (DHA, C22:6n-3) was 24.38 ± 2.50 mg/g dw whereas eicosapentaenoic acid (EPA, C20:5n-3) was present at 12.29 ± 2.12 mg/g dw. These two fatty acids were also at the top high percentage among all the fatty acids, with 38.06 ± 0.86% and 17.89 ± 1.24% (w/w) in total lipids and 34.64 ± 2.74% and 20.33 ± 1.05% (w/w) in neutral lipids, respectively. Therefore, green-lipped mussel is a rich source of DHA and EPA, the two n-3 polyunsaturated fatty acids that are beneficial to human health and nutrition.

    Notes: Summary We compared the performance of mainstream capnometry as a measure of arterial carbon dioxide tension (Paco2) with sidestream recordings in adult neurosurgical patients undergoing supine craniotomy. Two hundred and forty patients were randomly assigned so that the end-tidal carbon dioxide tension (PÉco2) was measured using either a mainstream or sidestream infrared capnometer. All patients received propofol anaesthesia and ventilation was adjusted according to clinical requirement. Arterial blood gas analyses were performed after induction, prior to dural incision, during surgery and before wound closure. Simultaneous haemodynamic and ventilatory parameters were also recorded. For 1007 paired measurements of PÉco2 and Paco2 (mainstream, n = 503; sidestream, n = 504), the mean (SD) mainstream arterial to end-tidal carbon dioxide tension difference, 0.64 (0.16) kPa, was smaller than the corresponding sidestream values, 0.99 (0.40) kPa (p 〈 0.001). The limits of agreement for the mainstream analyser, 0.32–0.96 kPa, were also narrower than the sidestream recordings, 0.19–1.79 kPa (p 〈 0.001). In both capnometers, the arterial to end-tidal difference in carbon dioxide tension did not change with time. However, there was greater within-patient variation in the sidestream group. Our study showed that mainstream PÉco2 provided a more accurate estimation of Paco2 than sidestream measurement.

    Notes: Background:  Distinguishing primary cutaneous adnexal neoplasms (PCANs) from metastatic carcinomas (MCs) can be difficult. We study the utility of p63, CK 5/6, CK 7, and 20 expression in PCAN vs. MC.Methods:  Twenty-one PCAN with sweat gland differentiation (six benign, 15 malignant), one sebaceous carcinoma, and 15 MC (14 adenocarcinomas, one urothelial carcinoma) to skin were retrieved from the pathology files. Immunostains for p63, CK 5/6, CK 7, and CK 20 were performed and graded as follows: 1, 〈10; 2, 11–50; and 3 〉50% of tumor cells stained.Results:  Twenty of 22 PCAN expressed p63 and CK 5/6. Four of 15 and two of 15 MC were positive for CK 5/6 and p63, respectively. Thirteen of 22 PCAN and 13 of 15 MC were positive for CK 7, respectively. All PCAN were negative for CK 20, two of 15 MC were positive. The sensitivity and specificity for the diagnosis of PCAN were 91 and 73% for CK 5/6, 91 and 100% for p63, and 60 and 13% for CK 7, respectively.Conclusions:  For distinguishing PCAN from MC: (1) positivity for p63 and CK 5/6 are relatively specific and sensitive for PCAN, (2) CK 7 and 20 are neither sensitive nor specific, and (3) CK 7 positivity in PCAN was focal with a specific pattern in contrast to the diffuse positivity for MC.

    Notes: Background:  Solitary sclerotic fibroma (SF) presents as a well circumscribed dermal nodule, composed of sparse spindle cells with alternating wavy collagen fibers arranged in a storiform pattern. The histogenesis and nature of this histologically distinct lesion are uncertain. Whether this peculiar tumor represents a true hamartoma or a degenerating end of various fibrous lesions such as pleomorphic fibroma (PF), dermatofibroma, or angiofibroma is still controversial. High proliferating index of spindle cells in SF argues against the possibility of being a degenerating end product of another lesion.Methods:  We studied morphological features and immunoprofile of eight SFs, in comparison with four PFs, one collagenized dermatofibroma, two angiofibromas, and two periungual fibromas. Immunostains for CD34, CD31, O13 (CD99), Factor XIIIa, S-100, CD68 (KP-1), and MIB-1 were carried out using a labeled streptavidin–biotin method with DAKO-automated immunostainer. Paraffin blocks of two SFs were reprocessed for electron microscopic studies. Clinical data of all patients with SF were also reviewed.Results:  Spindle cells and pleomorphic cells in SF and PF showed diffuse immunoreactivity for CD34 and O13 but were negative for CD31, S-100, and CD68. Spindle cells in one dermatofibroma and one angiofibroma were positive for Factor XIIIa. Proliferating index (MIB-1) was very low in all cases of SF, contradicting some previous reports.Conclusions:  SF is a fibrotic lesion with cells positive for CD34 and O13. It shares a common immunoprofile with PF but is distinct from dermatofibroma and other common spindle cell lesions of skin. O13 expression in SF has not been previously described.

    Notes: This paper describes a project which uses the quality-function-deployment technique to improve a distance-learning program, operated by a government training organization in Hong Kong, for junior staff in the clothing-manufacturing industry. The scope of this study is twofold: it identifies 12 appropriate study modules for modifying the course content; and it identifies the kinds of supporting services which help students to study the course. An evaluation is made of the existing study modules and supporting services that are identified as successful. Suggestions are made regarding bridging the gap between importance and satisfaction for some of the important quality dimensions. The methodology attempts to translate customers' voices into various design features of the products and services in order to prepare the course for continuous improvement.

    Notes: The presence of hard alpha (HA) anomalies in titanium alloys represents a significant potential degradation to gas turbine component performance. Although HA defects in titanium alloys are rare, when they are present, they can crack and ultimately result in failure. In static fracture and fatigue test specimens, embedded HA defects had significantly higher fracture strengths than anticipated. The objective of this work was to determine if residual stresses caused by thermal expansion mismatch during material fabrication were the cause of the observed behaviour. The residual stress fields in and around surface and embedded HA particles in Ti–6Al–4V (Ti–6–4) were determined using an elasticity solution and measured coefficient of thermal expansion (CTE) data. The calculated stress distributions serve as the foundation for comparisons of the local stress and the fracture strength, the stress intensity factor K and the crack growth threshold ΔKth, with the experimentally determined fatigue lives. The analytical results indicated that CTE-induced residual stress around HA particles can contribute to the fatigue strength of Ti–6–4 by delaying microcracking of HA anomalies and reducing the driving force (effective ΔK) of the fatigue crack. Based on the analysis results, the differences between the surface and subsurface results as well as the difference between predicted and measured fatigue lives could be largely attributed to the residual stress effects caused by the mismatch of the particle and matrix properties.

    Notes: Aims:  To assess the proliferative activity of gestational trophoblastic disease (GTD) using one of the novel proliferation markers (MCM7) and to determine its prognostic value in hydatidiform mole (HM).Methods and results:  Immunohistochemical staining for MCM7 was performed on 122 samples of paraffin-embedded trophoblastic tissues including 22 normal first-trimester placentas, 12 term placentas, 12 spontaneous miscarriages (SM), 21 partial moles (PM), 44 complete hydatidiform moles (CM), and 11 choriocarcinomas (CCA). The correlations between the proliferative indices assessed by MCM7, proliferating cell nuclear antigen (PCNA) and Ki67 (MIB1) immunoreactivity as well as clinical progress were assessed. MCM7 immunoreactivity was found predominantly in the nuclei of cytotrophoblast and intermediate trophoblast and decreased with placental maturation. MCM7 expression was highest in CCA, followed by CM, PM, normal first-trimester placenta, SM and term placenta. MCM7 index was significantly higher in PM and CM than in SM (P = 0.007, P 〈 0.001) but not between PM and CM themselves (P = 0.560). Eighteen of the 65 patients with HM developed persistent trophoblastic disease (PTD) requiring chemotherapy. There was no significant difference in MCM7 indices between the patients who developed PTD and those who did not (P = 0.312). MCM7 indices correlated well with Ki67 (P = 0.002) but not with PCNA (P = 0.054) indices. MCM7 indices demonstrated less variability than PCNA and Ki67 and may be a better proliferation marker than the latter two.Conclusions:  We conclude that MCM7 is useful in differentiating molar and non-molar gestations but is not helpful in discriminating PM from CM or in predicting PTD.

    Notes: Development of follicular dendritic cell sarcoma in hyaline-vascular Castleman’s disease of the nasopharynx: tracing its evolution by sequential biopsies Aims: Hyaline-vascular Castleman’s disease (HVCD) and follicular dendritic cell (FDC) sarcoma occurring in the nasopharynx are both extremely rare. We report the first case of transformation of the former into the latter as documented by sequential biopsies. The steps involved in the transformation were described in detail and the possible role of p53 studied. Methods and results: The patient presented at the age of 23 years with nasopharyngeal HVCD. Hyaline- vascular Castleman’s disease with FDC overgrowth was diagnosed in a recurrence 8 years later, and a frank FDC sarcoma developed at the same site 11 years after initial presentation. The patient remained disease-free 3 years after excision and adjuvant chemotherapy. The FDC sarcoma comprised swirling fascicles of spindly cells with indistinct cell borders. The tumour cells expressed the FDC markers CD21, CD35 and CNA.42 and in-situ hybridization for Epstein–Barr virus-encoded RNAs was negative. Over-expression of p53 protein was observed in the FDC sarcoma and an increased number of weakly p53-positive spindly cells could also be demonstrated in the HVCD specimen. This finding suggested a possible role of p53 in the evolution from HVCD to FDC sarcoma. Critical analysis of the literature shows that, among the 13 reported cases of FDC sarcoma associated with Castleman’s disease, possible progression from the latter to the former is documented in only two cases. Conclusions: The sequential changes observed in the current case provide further evidence to strengthen the role of HVCD as a possible precursor of FDC sarcoma. There is a possible role of p53 in the transformation process but confirmation by future studies is needed.
