    Notes: Abstract Objectives: A prospective study was undertaken to determine the transcorneal penetration of three topically applied fluoroquinolones into aqueous humour. Methods: Two hundred and twenty-four patients undergoing cataract extraction received 0.3% ciprofloxacin, norfloxacin or ofloxacin eye drops by two different administration modes with different frequencies and intervals of application. At the beginning of cataract extraction (0.5–3 h after the last drop), 50–100 μl aqueous fluid was aspirated from the anterior chamber and immediately stored at −80 °C. Antibiotic concentrations were measured using high-performance liquid chromatography. Results: Generally, topical ofloxacin and ciprofloxacin yielded aqueous humour levels higher than topical norfloxacin. The highest concentrations of all tested fluoroquinolones were measured after using an application mode, in which one drop was given every 15 min between 0600 hours and 0800 hours, prior to operation. When applied by this mode, ciprofloxacin achieved a mean aqueous level of 0.380 (±0.328) μg · ml−1 (range 0.033–1.388 μg · ml−1), norfloxacin 0.182 (0.118) μg · ml−1 (range 0.038–0.480 μg · ml−1) and ofloxacin 0.564 (0.372) μg · ml−1 (range 0.064–1.455 μg · ml−1). These mean concentrations were above the minimum inhibitory concentration (MIC90), concentrations required for inhibition of 90% of pathogen strains in vitro of gram-negative bacteria, such as Proteus mirabilis and Escherichia coli. Therapeutic values above the MIC90 of Staphylococcus epidermidis, the pathogen causing eye infections most frequently, were reached by 67.5% of patients after ofloxacin and by 41% after ciprofloxacin, but never after norfloxacin treatment. Conclusion: Of the currently available topical fluoroquinolones, ofloxacin achieved the highest aqueous humour concentration. This fluoroquinolone may be an useful ophthalmic agent for topical antibacterial management, but it does not seem to be prophylactically effective against Streptococcus pneumoniae or Pseudomonas aeruginosa.

    Notes: Abstract We evaluated 49 paired cerebrospinal fluid (CSF) and serum samples of 35 patients infected with the human immunodeficiency virus type 1 (HIV-1) for laboratory evidence of cytomegalovirus (CMV) infection. The patients were grouped according to clinical criteria as probable CMV encephalitis/polyradiculomyelitis, CMV retinitis, cerebral toxoplasmosis, progressive multifocal leukoencephalopathy, HIV-1-related cognitive/motor complex, HIV-1-associated myelopathy, and other neurological diseases. Paired CSF and serum samples were analysed for CMV deoxyribonucleic acid (DNA) by polymerase chain reaction (PCR), quantitative intrathecal synthesis of immunoglobulin G (IgG) antibodies specific for recombinant phosphoprotein 150 (pp150) of CMV and CMV-specific serum IgM. Intrathecal synthesis of pp150-specific IgG was detected in 26% of patients (9/35), serum IgM was found in 23% of patients (8/35), and PCR of CSF was positive in 11% of patients (4/35). Detection of CMV-specific DNA in CSF preceded the intrathecal antibody synthesis in three patients for whom serial samples were available. PCR results of the CSF became negative in one patient with CMV polyradiculomyelitis after successful therapy with 9-[2-hydroxy-l(hydroxymethyl) ethoxymethyl] guanine (DHPG). PCR has a higher diagnostic specificity in the acute phase of CMV infection than intrathecal antibody synthesis. The serum IgM response to CMV cannot be used to monitor a compartmentalized immune response in the central nervous system while an intrathecal immune response seems to be associated with recovery either spontaneously or as a result of treatment.

An experiment on the radiative π{+}-meson photoproduction from the proton ( γp → γπ{+}n) was carried out at the Mainz Microtron MAMI in the kinematic region 537MeV 〈 Eγ 〈 817MeV, 140°≤ $ \theta_{{\gamma \gamma ^{\prime }}}^{{{{\rm cm}}}}$ ≤180°. The π{+}-meson polarizabilities have been determined from a comparison of the data with the predictions of two different theoretical models, the first one being based on an effective pole model with pseudoscalar coupling while the second one is based on diagrams describing both resonant and nonresonant contributions. The validity of the models has been verified by comparing the predictions with the present experimental data in the kinematic region where the pion polarizability contribution is negligible ( s1 〈 5mπ2) and where the difference between the predictions of the two models does not exceed 3%. In the region, where the pion polarizability contribution is substantial ( 5 〈 s1/mπ2 〈 15, -12 〈 t/mπ2 〈 - 2), the difference $\ensuremath{(\alpha -\beta )_{\pi^{+}}}$ of the electric (α) and the magnetic (β) polarizabilities has been determined. As a result we find $\ensuremath{(\alpha -\beta )_{\pi^{+}}=(11.6\pm 1.5_{{\rm stat}}\pm 3.0_{{\rm syst}}\pm 0.5_{{\rm mod}})\times 10^{-4}{}{\rm fm^{3}}}$ . This result is at variance with recent calculations in the framework of chiral perturbation theory.
