    Early management in poor grade aneurysm patients (1994)

    Notes: Summary Aneurysm surgery began in Lübeck only in 1986 when the department was completely reorganized. Early operation in the good grade patients (I–III, according to Hunt and Hess) was performed. In every case we also discussed the feasibility of operating on the poor grade patients (Hunt and Hess IV and V). During a five-year period (1986–1991) a total of 277 SAH patients were admitted to the department. 109 (39%) patients arrived in a poor grade (Hunt and Hess IV or V), 12 of these patients died within hours of admission. 25 patients, who presented with a large intracerebral and/or subdural haematoma, were urgently operated upon by haematoma evacuation and aneurysm clipping. An external ventricular drainage was performed on 72 patients. Of the ventriculostomy group 33 patients improved and 27 were operated upon. In 17 of the 39 patients without improvement after CSF-drainage we decided to operate. Overall 69 patients were surgically treated (craniotomy, aneurysm clipping) and 40 were not. The mortality rate in the surgical cases was 16 (23%) compared with 30 (75%) without operation. It is concluded that poor grade aneurysm patients can achieve a better outcome with active treatment based on immediate ventriculostomy and optimal haemodynamic parameters after haematoma evacuation and early occlusion of the aneurysm.

    Notes: Summary Acute subdural haematoma (SDH) secondary to a raptured intracranial aneurysm is a rare event. Out of a total of 292 patients with a verified aneurysm (period 1986–1992) in five cases SDH was the diagnosis on CT-evaluation. One patient was in such a bad condition that no treatment was indicated. The remaining four patients were operated on: craniotomy and haematoma evacuation in two cases, craniotomy for haematoma evacuation and aneurysm-clipping in the other two cases. Two patients died and two achieved a good outcome.

    Notes: Thiol-terminated polyethers were prepared by two different routes, the starting materials being commercially available polyether polyols and epoxy resins. The products were pale colored liquids and were found to be very stable on storage at room temperature. Thiol-terminated polyethers of functionality from 2 to 4 were prepared. They are interesting building blocks for polymers through oxidation of the terminal SH groups or through coreaction with epoxy resins.

    Notes: A detailed study of the reaction of catharanthine N-oxide and vindoline has been carried out employing various conditions. Under optimum conditions, which involve low temperatures and trifluoroacetic anhydride as reagent, 3′, 4′-dehydrovinblastine (XIII, R = COOCH3), in reasonable yields is essentially the exclusive product. However two additional products, 18′ (epi)- 3′, 4′-dehydrovinblastine (XIV, R = COOCH3) and 1′-hydroxy- 3′, 4′-dehydrovinblastine (XVI, R = COOCH3) are also often isolated.The reaction, which follows the course of a Polonovski-type fragmentation process, has been extended to the N-oxide derivatives of dihydrocatharanthine and decarbomethoxycatharanthine to provide again a series of bisindole alkaloid derivatives, also vinblastines. A mechanistic rationale is provided to explain the various results obtained.

    Description / Table of Contents: From the reaction of trimethyl silanole with elementary rubidium and caesium, quantitative yields of the new compounds rubidium and caesium trimethylsilanolate have been obtained. Physical and chemical properties as well as IR and NMR spectra of the alkali trimethylsilanolates being now completed are reported and discussed.
    Notes: Durch Umsetzung von elementarem Rubidium und Caesium mit Trimethylsilanol wurden erstmals ein Rubidium- bzw. Caesiumtrimethylsilanolat in reiner Form dargestellt. Neben einer vergleichenden Betrachtung ihrer physikalischen und chemischen Eigenschaften wurden an den Verbindungen der auf diese Weise vervollständigten Reihe der Alkali-trimethylsilanolate IR- und NMR-spektroskopische Untersuchungen durchgeführt. Die Ergebnisse werden in Hinsicht auf die vorliegenden Struktur- und Bindungsverhältnisse diskutiert.

Bei der Zuordnung der Spektren werden jeweils Vergleichssubstanzen herangezogen, die die Gruppierung R3X -  mit dem entsprechenden Peroxyd gemeinsam haben, um durch Aussonderung der Frequenzen des R3X-Gerüstes die Schwingungen der XOOX-Brücke isolieren zu können. Dabei kann als wahrscheinlichste Struktur für die Verbindungen des Typs R3SiOOSiR3 und R3COOCR3 ein wasserstoffperoxydähnlicher Bau der Moleküle abgeleitet werden. Diese Deutung wird durch die Tatsache gestützt, daß andere Bearbeiter mit Hilfe von Messungen elektrischer Momente zu ähnlichen Aussagen über den Molekülbau des Di-tert.-Butylperoxyds gelangen.
