Abstract The results of a retrospective clinico-pathological study of 11 cases of fistula-in-ano (FIA) and 10 cases of peri-anal abscess in children are presented. The overwhelming male predominance is confirmed. The early onset of FIA and the high percentage of bilateral and multiple fistulae support a congenital aetiology. Persistent inflammatory changes around anal glandular epithelium lining the fistulous tract may be important in the pathogenesis and may also account for metaplasia, ultimate destruction, and disappearance of the epithelium in some cases. Perianal abscesses may antedate and progress into FIA.
