Zusammenfassung Bei Patienten mit histologisch gesicherter, kompensierter Lebercirrhose wurden nach i.v. Verabreichung von (3H)-Cortisol und (14C)-Corticosteron nach dem Isotopenverdünnungsprinzip die Sekretionsrate dieser beiden Steroide bestimmt und außerdem nach Injektion von (14C)-Testosteron und (3H)-Δ4-Androstendion oder von (14C)-Dehydroepiandrosteron und (3H)-Dehydroepiandrosteronsulfat der Abbau dieser Steroide in Androsteron-, Ätiocholanolon-und Testosteronglucuronid sowie die Sekretions- und Produktionsrate von Testosteron berechnet. Die Cortisolsekretionsrate lag bei 10 Lebercirrhotikern mit im Mittel 14,5 mg/Tag (s=2,92 mg) nur gering unter dem Mittelwert Gesunder ( $$\bar x$$ =17,3 mg/Tag), obwohl die Ausscheidung von 17-Hydroxycorticosteroiden sowie von Tetrahydrocortisol und Tetrahydrocortison signifikant geringer war als bei den Normalpersonen. Die Corticosteronsekretion betrug bei dem gleichen Kollektiv von Leberkranken im Mittel 4,22 mg/Tag (s=1,0 mg) und war somit eindeutig höher als bei Gesunden ( $$\bar x$$ =3,3 mg/Tag). Auch die Ausscheidung von Testosteron-, Androsteron-, Ätiocholanolon- und Dehydroepiandrosteronglucuronid sowie der 17-Ketosteroide war bei 10 Männern mit Lebercirrhose im Mittel signifikant niedriger als bei gesunden Probanden einer analogen Altersgruppe; die geringere Exkretion dieser Steroidmetaboliten ist jedoch Folge des gestörten Stoffwechsels der Androgene, denn die Lebercirrhotiker schieden im Vergleich zu Stoffwechselgesunden am 1. Tag p.i. im Mittel einen signifikant geringeren Teil der injizierten radioaktiven Androgene als Androsteron und Ätiocholanolon aus. Die prozentuale Umwandlung von Dehydroepiandrosteron und Dehydroepiandrosteronsulfat in Ätiocholanolon (=5β-Androsteron) war jedoch geringer als der Abbau in die 5α-Verbindung Androsteron, ein Befund, der offenbar mit dem extrahepatischen Stoffwechsel der Androgene in Zusammenhang steht. Außerdem sprechen unsere Ergebnisse für eine geringere Interkonversion von Dehydroepiandrosteronsulfat in Dehydroepiandrosteron. Die Testosteronproduktionsrate war nur bei 4 von 7 Patienten eindeutig vermindert, während der Anteil der Sekretion an der Gesamtproduktion von Testosteron eindeutig höher lag als bei Gesunden. Außerdem zeigen die Befunde, daß bei Patienten mit Lebercirrhose aus den Ergebnissen von Steroidanalysen im Urin nur bedingt Rückschlüsse auf die inkretorische Funktion der Nebennierenrinde und der Gonaden möglich sind.

Summary In 10 patients with cirrhosis of the liver secretion rates of cortisol and corticosterone were determined using an isotope dilution technique. The mean cortisol secretion rate was only slightly reduced ( $$\bar x$$ =14.5 mg/day;s=2.92) when compared with findings in normal persons (x=17.3 mg;s=3.84) through excretion of 17-hydroxycorticosteroids, tetrahydrocortisol and tetrahydrocortisone was significantly diminished. The secretion rate of corticosterone however was elevated to a mean value of 4.22 mg/day (normal 3.3 mg/day). The glucuronides of androsterone, etiocholanolone and testosterone were separated from urine after the i.v. administration of14C-testosterone,3H-androstendione,14C-dehydroepiandrosterone and3H-dehydroepiandrosteronesulphate in 10 cirrhotic males and in a comparable group of controls. Urinary excretion of these androgen metabolites were below the normal level in cirrhotic patients. Conversion of the tracers to androsterone and etiocholanolone was also significantly decreased. However, the metabolic break-down of dehydroepiandrosterone and dehydroepiandrosteronesulphate to etiocholanolone was lower than to the 5α-metabolite androsterone possibility indicating the greater importance of extrahepatic metabolism in the cirrhotics. In this group of patients interconversion of dehydroepiandrosteronesulphate to dehydroepiandrosterone was also reduced. Testosterone production rate determined from the specific activity of testosteroneglucuronide was definitely decreased in only 4 of 7 males with cirrhosis while the portion of urinary secretion rate on the total testosterone production was higher in the cirrhotics than in the controls. Our findings also demonstrate that in patients with cirrhosis of the liver, conclusions on the function of the adrenals and the gonads, based on results of urinary analysis of steroid hormones, are limited.
