Summary The 24-hours course of blood cortisol and ACTH levels was investigated in three metabolically normal volunteers and in four patients with bilateral adrenocortical hyperplasia confirmed by operation. The ACTH levels were determined generally by measuring the increase of corticosterone in the adrenal venous blood of hypophysectomized rats. In the healthy individuals both hormones were found in a parallel diurnal rhythm, characterized by a relatively high peak in the morning and a maximal depression at midnight. ACTH varied in the range from below the measuring threshold up to 1,0 mµ/100 ml plasma, as previously described in the literature. In the patients with hypercorticism before subtotal adrenalectomy this diurnal rhythm was abolished for both cortisol and ACTH. The constancy of ACTH secretion alone causes an increase in daily production which is shown to be able to produce the full-blown biochemical picture of Cushing's syndrome. In one of the patients, maximally elevated ACTH levels (500 mµ/100 ml plasma) were found immediately after operation. Normal circadic rhythm of ACTH secretion was lacking. Several months later, ACTH levels were still markedly elevated, but demonstrated again diurnal rhythm comparable to the normal.

Zusammenfassung Bei Stoffwechselgesunden sowie bei Cushing-Kranken mit operativ bestätigter bilateraler Nebennierenrinden-Hyperplasie wurden vergleichende Untersuchungen des Cortisol- und ACTH-Blutspiegels im 24-Std-Verlauf durchgeführt. Die ACTH-Blutspiegel wurden meist anhand des Corticosteronanstieges im NNV-Blut hypophysektomierter Ratten gemessen. Bei Gesunden fand sich eine für beide Hormone völlig gleichsinnige Tagesrhythmik, die durch einen relativ hohen morgendlichen Gipfel sowie niedrigste Werte um Mitternacht ausgezeichnet war. Die Rhythmik von ACTH schwankte in dem von der Literatur her bekannten Bereich bis maximal 1,0 mE/100 ml Plasma. Bei Cushing-Patienten vor der subtotalen Adrenalektomie war diese Rhythmik für beide Hormone aufgehoben. Allein die Konstanz der Sekretion, d.h. der Verlust des nächtlichen Abfalls, bewirkt eine Tagesmehrproduktion von ACTH, die auch im Experiment in der Lage ist, die Cortisolspiegel Cushing-Kranker zu erzeugen. Unmittelbar nach der Operation lagen in einem Fall maximal erhöhte ACTH-Werte (500 mE/100 ml Plasma) vor. Sie waren konstant ohne Schwankung im 24-Std-Verlauf zu messen. 12 Monate später war jedoch auch bei diesem Fall bei immer noch stark erhöhten ACTH-Blutwerten die beim Gesunden nachweisbare Tagesrhythmik wieder erkennbar.
