    Notes: Summary A midline cerebellar tumor in an 18-year-old male, on light microscopy composed of closely packed, small featureless cells with little cytoplasm, under electron microscopic examination showed distinct epithelial features. The cells were closely apposed and joined by zonula adherentes, puncta adherentia and classical desmosomes. The latter structure has not been described in unequivocal glial or neuronal tumors or in normal cells dervied from the neural tube in vertebrates, not even in ontogenesis. In many areas the tumor cells abut on extensive basement membranes. Although the majority of the cells were undifferentiated, we occasionally observed elements containing dense core vesicles, microtubules and cilia. In addition, typical astrocytes were identified. The presence of these features strongly favors the neuroectodermal origin of this neoplasm.

    Description / Table of Contents: Zusammenfassung Aufgrund anatomischer Gegebenheiten, sekretorischer und klinischer Untersuchungen, sowie des signifikanten Einflusses des intraoperativen Elektrotests auf postoperative Sekretion und Rezidivrate, ist folgendes grundsätzliches Vorgehen bei der PSV gerechtfertigt: 1. ausgedehnte Befreiung von Kardia und Fundus, 2. Denudation des Oesophagus auf 6 cm, 3. Skeletieren der Kleinkurvatur bis auf 6 cm an den Pylorus heran, 4. Vollständigkeitskontrolle mit dem intraoperativen Elektrotest, und 5. in Einzelfällen die vordere oder zirkuläre Myotomie des Oesophagus. Die Indikation zur Myotomie sollte nur aufgrund eines persistierend positiven Elektrotests gestellt werden.
    Notes: Summary Based on anatomical conditions, results of clinical trials, and the significant influence of the intraoperative clectrotest on postoperative secretion and recurrence rate after PGV, a reliable surgical technique should include: (1) extended dissection of the cardia and the fundus, (2) denudation of the oesophagus up to 6 cm, (3) skeletonization of the lesser curvature down to 6 cm from the pylorus, (4) an intra-operative electrotest and (5) in selected cases an anterior or circular myotomy of the oesophagus. Myotomy can only be justified by a persistingly positive electrotest.
