    Notes: A simple rapid method for determining total coliforms and Escherichia coli in drinking water was evaluated in a preliminary pilot-scale study at three independent laboratories by comparison with the standard membrane filtration technique. No significant difference was found between the two methods.The method is based on defined substrate technology. It gives confirmed results within 24 h and requires only limited equipment. It was considered particularly suitable for weekend and emergency use and for on-site testing at remote sampling points.

    Notes: Abstract Biological observations were made on the antihaemostatic activity in the saliva of the giant Amazon leech Haementeria ghilianii. Particular emphasis was placed on determining where the fibrino(geno)lytic enzyme hementin is produced and secreted in the salivary gland complex. Hungry third-fed Amazon leeches produce about 650 units of hementin, with by far the majority of the activity (75%) in the two posterior glands. The two anterior glands contain much less hementin (12%), and a small amount (12%) resides in the proboscis itself, presumably in the salivary gland ductules. In the anterior gland, hementin appears to be produced by only certain cells. The secretion of hementin is confined to the lumen of the proboscis. Secretions in the proboscis lumen are rich in antihaemostatic activity, as evidenced by fibrinogenolysis (hementin), prolongation of prothrombin time (PT), activated partial thromboplastin time (APTT), thrombin clotting time (TCT) and Atroxin clotting time, and inhibition of collagen-induced platelet aggregation. In contrast, no anti-haemostatic, including hementin, activity was detectable at the tip. Suprisingly, the wound from the bite by the Amazon leech is not associated with prolonged bleeding. This is in marked contrast to the wound by the European medicinal leech Hirudo medicinalis which bleeds for an average of ten hours. The absence of bleeding is compatible with the interpretation, based on the above findings, that during feeding the Amazon leech does not inject antihaemostatic factors into the host, or at least not in the vicinity of the wound.

    Notes: Abstract The case of a 10.5-year-old boy who had complete right main pulmonary artery thromboembolism complicating steroid-resistant nephrotic syndrome is presented. Despite the massive nature of the lesion clinical symptoms were relatively minor. The thromboembolism was successfully treated with intrapulmonary artery streptokinase. The case highlights the discrepancy that may be found between the clinical and pathological findings in pulmonary thromboembolism in the nephrotic syndrome of childhood.

    Description / Table of Contents: Résumé Cinquante malades souffrant d'une instabilité antérieure chronique du genou ont bénéficié d'une reconstruction ligamentaire à l'aide d'une «prothèse composite de Westminster» comportant un axe en fibres de carbone entouré d'une gaîne de polyester. Chez 42 patients la prothèse a été utilisée comme complément d'une greffe de fascia lata, associée à une reconstruction extra-articulaire selon la technique de MacIntosh. La prothèse n'a été employée isolément que dans huit cas, qui avaient déjà subi une intervention extra-articulaire avec un résultat seulement passable. L'âge moyen des malades au moment de l'opération était de 28 ans et demi (de 18 à 50); l'instabilité durait depuis 5.3 ans en moyenne et le recul moyen était de 3.8 ans (de 2 à 6 ans). Le bilan a comporté une estimation subjective de la fonction (selon la cotation du genou de Lysholm) et une estimation clinique de la stabilité. Le résultat était excellent ou bon chez 37 opérés (74%), moyen chez 11 (22%) et mauvais chez 2 (4%). Le signe de Lachman était coté à 1 ou moins chez 35 malades et on ne retrouvait pas d'instabilité rotatoire chez 37 malades. Les signes cliniques d'instabilité étaient bien correlés avec la cotation du genou de Lysholm. Vingt-deux patients non sélectionnés ont subi un examen arthroscopique avec biopsie du «néo-ligament» 10.4 mois en moyenne après l'opération. La prothèse a été trouvée stable et bien recouverte d'une épaisse couche de tissu fibreux («néo-ligament») chez 19 d'entre eux. La prothèse était rompue partiellement chez deux malades et complétement chez un troisième. Trente-deux patients (64%) avaient repris leur activité sportive antérieure et 13 d'entre eux (26%) avaient retrouvé le niveau qu'ile attoignaiont avant l'accident. Dans l'ensemble, 45 sujets (90%) étaient satisfaits du résultat de l'intervention.
    Notes: Summary Fifty patients with chronic symptomatic anterior cruciate insufficiency underwent ligamentous reconstruction using the “Westminster Composite Prosthesis” which consists of a central core of carbon fibre with an external weave of polyester. In 42 patients, the prosthesis was used as an augmentation within a tube of ilio-tibial tract, combined with a MacIntosh's extraarticular reconstruction. The prosthetic replacement alone was used in eight patients where there had been a previous extraarticular reconstruction and was associated with only fair results. The average age of the patients at operation was 28.5 years (range 18 to 50 years); instability of the knee had been present for a mean of 5.3 years and the average follow-up was 3.8 years (range 2 to 6 years). Assessment included subjective functional rating (Lysholm knee score) and clinical examination for instability. Thirty-seven patients (74%) had a good or excellent result, 11 (22%) had a fair result and two (4%) had a poor result. The Lachman test was grade I or less in 35 patients and the pivot shift sign was eliminated in 37 patients. Clinical signs of instability correlated well with the Lysholm knee score (p〈0.001). Twenty-two unselected patients (44%) underwent an arthroscopic assessment and biopsy of the “neoligament” at an average of 10.4 months post-operatively. The prosthesis was found to be stable and well covered by a thick fibrous sheath (“neoligament”) in 19 patients. The prosthesis was partially ruptured in two patients and completely disrupted in one. Thirty-two patients (64%) returned to their previous sports and 13 of them (26%) achieved their pre-injury level of performance. Overall, 45 patients (90%) were pleased or satisfied with the results of their knee reconstruction.
