    Notizen: Renal biopsy in the early puerperium was carried out on 20 patients with toxaemia and 13 abruptio placentae without preceding toxaemia. The tissue obtained was studied by light and electron microscopy. All patients had a very similar glomerular lesion with (1) swelling and slight proliferation of the endotheliat cells, (2) increase in the number of mesangial cells and the amount of the mesangial matrix, and (3) granular deposits derived from fibrinogen within the endothelial cells and the mesangial matrix.It is suggested that in both toxaemia and abruptio placentae there is release of thromboplastin from the placental site into the circulation which causes disseminated intravascular coagulation; this process is complicated by the fact than in pregnancy the fibrinolytic mechanism is in any case impaired. The glomerular lesion in both groups is the result of the response of the endothelial cells and mesangium to deposited fibrin or its derivatives.

    Notizen: Differential protein clearances were carried out in 31 patients with toxaemia of pregnancy and in 21 non-toxaemic patients with abruptio placentae. In 6 patients with toxaemia and in 7 with abruptio placentae, dextran clearance determinations were also performed. Results were expressed in terms of selectivity indices relating the clearances of these macromolecules to their molecular weights. In toxaemia, protein and dextran selectivity results were in the intermediate range, dextran values being slightly higher than the values obtained for protein. In abruptio placentae, however, the values obtained for protein selectivity were very low, while those obtained for dextran selectivity were very high.The results suggest than in toxaemia the proteinuria is of glomerular origin, but that in abruptio placentae the urinary protein is post-glomecular and is derived from plasma protein passing through damaged renal tubular epithelium.

    Notizen: Triploidy is a fairly common cause of abortion in the first trimester. It is uncommon for such a pregnancy to continue beyond this time. Two cases of triploidy with hydatidiform degeneration of the placenta and fetal abnormality where the pregnancies continued to 29 and 25 weeks are reported. The concentration of HCG in these abnormal placentae was high and the patients excreted larger than normal amounts of it. Severe pre-eclampsia and anaemia occurred. Other cases in the literature are reviewed.

    Notizen: Abstract This study was designed to evaluate the role of111In-labeled leukocyte imaging and fecal excretion in the assessment of inflammatory bowel disease. We compared these tests to various indices of disease activity in Crohn's disease, to Truelove's grading in ulcerative colitis, and to endoscopy, x-ray, and pathology in both diseases. Eleven controls, 16 patients with Crohn's disease, 13 with ulcerative colitis, and 3 with other types of acute bowel inflammation were studied (positive controls). Indium scanning was performed at 1,4, and 24 hr. Fourteen of 16 patients with active Crohn's disease had positive scans but in only five was localization accurate. One patient had inactive ulcerative colitis, and the scan was negative. Of 12 patients with active ulcerative colitis, 10 had positive scans but disease localization was accurate in only four. Disease extent was correctly defined in 1 of the 3 Positive Controls. There was no significant difference in the accuracy of scanning at 1,4 or 24 hr.111In fecal excretion was significantly higher in patients with inflammatory bowel disease than in controls, and there was correlation between111In fecal excretion and most of the indices of disease activity in Crohn's disease. In ulcerative colitis,111In fecal excretion did not correlate with Truelove's grading but reflected colonoscopic assessment of severity. In conclusion,111In-labeled leukocyte scanning lacks sensitivity with respect to disease extent, but fecal excretion of111In correlates well with disease severity as determined by other methods.
