    Notizen: Background  Darier's disease is a rare, dominantly inherited genodermatosis. Although it has been well studied in caucasians, very little is known about the clinical spectrum of this disorder among Asians.Objectives  To determine the demographic and clinical profile of Asian patients with Darier's disease.Methods  This is a retrospective study of all new cases of Darier's disease seen in our centre over a 20-year period (1982–2002).Results  Twenty-four nonrelated cases of Darier's disease were studied. The incidence rate was 3·1 per million per decade. The gender distribution was 19 males and five females, and the ethnic origin was 21 Chinese, two Malays and one Nepalese. The peak age of onset was between 11 and 20 years. Sun exposure exacerbated the disease in 13 of the patients, and three had neuropsychiatric disorders. The disease affected predominantly seborrhoeic areas in 19 patients, flexural in three, acral in one and was segmental in one patient. Hand involvement was common and included palmar pits in nine patients, acrokeratosis verruciformis in four and nail changes in 12 patients. Haemorrhagic macules were not seen. Rare features included oral mucosal lesions (two patients) and guttate leucoderma (three patients). Pathogens involved in cutaneous infections included herpes simplex virus, Staphylococcus aureus, Streptococcus species and Morganella morgani. All patients treated with oral retinoids had improvement of clinical signs. In contrast, the response to topical retinoids was poor.Conclusions  Compared with western studies, our results show a similar incidence rate, age of onset, distribution of disease patterns and association with neuropsychiatric disorders. Features that differ include co-occurrence of guttate leucoderma, rarity of acrokeratosis, absence of haemorrhagic macules and poor response to topical retinoids.

    Notizen: Abstract Granisetron, a potent and selective 5-hydroxytryptamine receptor (5-HT3) antagonist was reported to be an effective antiemetic agent both in animal studies and in patients given highly emetogenic chemotherapy. A sample of 43 patients with breast cancer was accrued from September to November 1992 in a phase II study to assess the efficacy of granisetron in patients receiving FEC (5-FU, epirubicin, cyclophosphamide). Each patient received 3 mg intravenous granisetron as a single dose just prior to chemotherapy. Oral metoclopromide was prescribed to each patient as a rescue anti-emetic. The emetic episodes and degree of nausea were evaluated on a daily basis. Good control of emesis (0–2 episodes of vomiting) and nausea (mild or no nausea) was in the range 77%–98% and 77%–93% respectively. There was a complete response (no emetic episodes throughout the 6-day period) in 16 patients (37.2%). Onset of emesis tends to occur on day 1 and tend to subside after day 3; 85% of patients had onset of emesis in the first 2 days after chemotherapy. Control of emesis and nausea tends to improve after day 3, which could be the result of the reduced emetogenicity of the combination FEC with time. Altogether, 77% had good control of acute emesis; control of delayed emesis was better with 84% achieving a major response on day 2 after chemotherapy, which improved to more than 90% after day 4. Granisetron was generally tolerated with headache being the most common side-effect folloed by constipation and flushing. This study suggests that granisetron is an effective and well-tolerated anti-emetic agent, which deserves randomised trials to elucidate its efficacy further.

    Notizen: Abstract Background: Despite its moderate anti-tumour activity in head and neck cancers there have been no reports on the activity of paclitaxel in patients with nasopharyngeal cancer, a highly chemosensitive tumour. A phase II study was thus initiated to determine the objective response rate and toxicity of paclitaxel in patients with previously untreated metastatic nasopharyngeal cancer. Patients and methods: Twenty-four patients with previously untreated measurable metastatic nasopharyngeal carcinoma were accrued, one of them ineligible because of concomitant beta-blocker usage. Male : female ratio was 19 : 5, with a median age of 46 years. All had previously received radiotherapy but were chemotherapy-naïve. The great majority (20 of 24) had undifferentiated carcinoma. Paclitaxel (Anzatax, Faulding Pharmaceuticals) 175 mg/m2 was given intravenously over three hours every 21 days after premedication with oral dexamethasone and intravenous diphenhydramine and cimetidine. Results: There were five (21.7%) partial responses while eight patients remained stable. Median response duration was 7.5 months and median survival was 12 months. The main toxicity was haematological, with grade 1–2 neutropenia in 19% and grade 3–4 neutropenia in 4.5% of cycles. Three cycles were complicated by grade 3–4 anaemia and one patient required a blood transfusion. No thrombocytopenia was seen. Peripheral neuropathy was frequent (20 of 23 patients) but mild. Alopecia was complete in 14 patients. There were no cardiac toxicity or hypersensitivity reactions. Conclusions: Paclitaxel is well tolerated even in previously irradiated patients with metastatic nasopharyngeal cancer. Single-agent activity was 22% and its inclusion into combination chemotherapy regimens should be studied.

    Notizen: Tolerance stack analysis involving geometric characteristics is usually difficult as it involves numerous rules and conditions. Users need to acquire an adequate knowledge of geometric dimensioning and tolerancing before making any attempt at the computation of tolerance stack analysis problem. Conventional stack form methods are very widely used in tolerance stack analysis, but the computation requires tedious work with numerous rules to remember that often complicate the problem and confuse the user. A need therefore arises to simplify the tolerance stack methodology. This paper presents a straight forward, easy-to-use graphical approach known as the "Catena" method for tolerance stack analysis, involving geometric characteristics in form control – flatness, straightness, circu-larity and cylindricity. Three examples are given to illustrate the effectiveness of the proposed method. No complicated mathematical formulae are required in deriving the solution.

    Notizen: Abstract Populations of Iridaea splendens at Brockton Point, Stanley Park, Vancouver, Canada were observed to alternate in dominance between the gametophytic phase in summer and tetrasporophytic phase in winter. The mechanism regulating this alternation is not clear. Using a matrix projection model to simulate population growth, we show that this alternation is possible if there are differential survival and recruitment rates of the two phases in summer and winter. Sensitivity and elasticity analyses indicate the relative importance of perennation vs. recruitment. Recruitment from tetrasporophytes and from gametophytes both contribute about 25% to the population growth. Perennation among gametophytes is more important than among tetrasporophytes. The implication of this is that if this population is to be harvested, more tetrasporophytes can be harvested than gametophytes without resulting in the depletion of the resource. This is simulated in the matrix model by comparing the relative effects on population growth of increasing the mortality rate of the perennation phase of tetrasporophyte and gametophyte by 50 to 75%, and increasing recruitment rate in either phase, from summer to winter or from winter to summer.
