    Notes: Abstract Policy implementation has been an infrequently studied process although it has become increasingly evident that the implementation processes of policy development, policy approval, and policy application can shape and delay prospective implementation of legislation. The current study analyzes the implementation of P.L. 99–457 (Part H) the Education for the Handicapped Amendments of 1986. This law provides the states with planning money from the federal government to design a comprehensive interagency, multidisciplinary program of services for children with handicapping conditions and their families. Ratings were taken from key administrators and citizens knowledgeable about the law on state progress in implementing the fourteen requirements of the law and also on proposed influential conditions that might affect that implementation. Additional demographic information was obtained for the fifty states. The factor that seemed to be most influential in a subset of 35 states was the prior presence in the state of systems for interagency planning and development. Neither prior history of early childhood services, available resources, wealth of the state, or geographic region appeared to have a significant influence on policy development. There was some indication that different factors might become influential in the next phase of implementation, policy approval. It appears that progress in state policy development may be linked to the special requirements of the legislation itself and to prior availability of institutional structures that make mandated collaborative efforts more possible.

    Notes: Abstract The purpose of this study was to determine the safety and efficacy of transcatheter embolization in the management of bleeding visceral artery pseudoaneurysms. Eight patients (four women and four men) whose mean age was 61.0 years (range 44 to 77 years) underwent emergency transcatheter embolization for acute hemorrhage from a visceral artery pseudoaneurysm. Arteriographic technique was used to localize and selectively embolize these seven visceral artery pseudoaneurysms (three inferior pancreaticoduodenal, one gastroduodenal, two hepatic, and one splenic) by means of intravascular steel coils. Arteriography demonstrated visceral artery pseudoaneurysms in all eight patients. Acute hemorrhage was documented by endoscopy, falling hematocrit levels, CT scans, and arteriography. Successful embolization of these visceral artery pseudoaneurysms was achieved in seven (88%) of eight patients. In one patient embolization was not attempted because of a worsening clinical status, and this patient subsequently underwent emergency surgical resection. There was no significant morbidity associated with the procedures and seven patients remain asymptomatic with no further bleeding at a mean follow-up of 21.1 months (range 11 to 46 months). Arteriographic embolization of visceral artery pseudoaneurysms is a safe and highly successful technique for the effective identification and treatment of hemorrhage even in critically ill patients.

    Notes: Abstract Case reports of three patients presenting with acute limb-threatening lower extremity ischemia as a result of thrombosed popliteal artery aneurysms are described. Intra-arterial urokinase was administered to each patient prior to definitive surgery. This improved the infrapopliteal runoff in each case, allowing for successful arterial reconstruction without limb loss.

The various alternative treatment possibilities will be presented together with an evaluation of the effectiveness of such programs to date.

    Notes: Mechanisms for decomposition of 1- and 2-phenyltetralins were investigated using low resolution mass spectrometry and metastable ion techniques. Four primary decompositions were observed for 1-phenyltetralin radical cations: (1) the loss of C6H6 via a 1,4-elimination; (2) the elimination of ethene via competing losses from carbons 3 + 4 and carbons 2 + 3; (3) the loss of C8H8, probably through a stepwise Diels-Alder cycloreversion to expel styrene; and (4) the loss of methyl radical involving carbon 2 and possibly carbon 4. Three major decompositions were observed for 2-phenyltetralin radical cations: (1) the loss of C8H8, possibly through a Diels-Alder cycloreversion to expel styrene; (2) the loss of C6H6 via a 1,3 elimination; and (3) the loss of methyl radical from carbon 1. Various exchange reactions occur prior to these losses, but they proved to be incomplete even for metastable ions.
