Summary 1. Of 108 patients treated surgically for benign gastric ulcer, during the years 1951–1961, 15 died during the postoperative period—13 after emergent, and 2 after elective, procedures. 2. The mortality associated with emergent operations was 23 per cent, and that associated with elective procedures was 4 per cent. 3. The causes of these deaths are reviewed. Over 25 per cent were caused by suture-line leaks. This should be a preventable complication. 4. The fact that accurate differentiation between benign and malignant gastric ulcers has reduced the hazard of conservative therapy has led to an increase in the frequency of emergent surgery for benign gastric ulcers.
