Zusammenfassung Partielle oder totale Organdefekte an Magen, Dünn- und Dickdarm bedürfen im Rahmen der operativen Reparation gelegentlich auch des autoplastischen Ersatzes. Maßgeblich für die Indikationsstellung sind in erster Linie die Wiederherstellung einer befriedigenden Funktion und erst in zweiter Linie eine solche der Form. So stagnierte die auf Radikalität ausgerichtete Carcinomchirurgie des Magens wegen der unbefriedigenden Lösung des Problems der radikalen Tumorausrottung einerseits und einer befriedigenden Funktion des Rest- bzw. des Ersatzmagens. Ähnliches gilt auch für ausgedehnte Resektionen der Geschwürskrankheit im Magen. Hier geht es um die plastische Erweiterung des proximalen oder distalen Magens. Gilt die autoplastische Überbrückung von Defekten am Dickdarm als Not lösung, so sind die Ersatzoperationen am Dünndarm in jüngster Zeit entschieden verbessert worden. Hier wurden Fortsc0hritte erzielt, die die Behandlung der aus gedehnten Dünndarmresektion betreffen und ferner den Dünndarm in seiner Ersatz funktion nach Verlust des Dickdarmes unterstützen. Der Vortrag behandelt im einzelnen Indikationsstellung, operative Taktik und Technik der angedeuteten Problemstellung.

Summary Partial or total organ defects of the stomach, the small and large intestine, within the framework of surgical repair, from time to time also require replacement by autoplasty. The primary indication is the restoration of satisfactory function, the form this takes is of only secondary importance. Radically-orientated surgery in stomach cancer made no progress while there was on one hand no satisfactory solution of the problem of radical eradication of the tumour and on the other hand the problem of achieving a satisfactory function of the residual or replacement stomach. Similar problems also arise in extensive resection of a gastric ulcer. Here we are concerned with the plastic dilation of the proximal or distal stomach. While autoplastic bridging of defects of the colon is still considered only as an emergency solution, replacement operations of the small intestine have lately produced much improved results. Advances which were made concern the treatment of extensive resections of the small intestine and the function of the small intestine as a substitute after the loss of the large intestine. The lecture covers the various indications and surgical techniques in the light of the problems indicated above
