Zusammenfassung Liegt keine komplette Unterbrechung des hinteren Ringsegmentes vor, so kann der Beckenring allein durch eine ventrale Verklammerung (Verplattung, Fixateur externe u. a.) stabilisiert werden. Eine komplette dorsale Ringunterbrechung erfordert 1. eine Rekonstruktion des hinteren, 2. eine Verklammerung des vorderen Ringsegmentes. Bei ossärer Verletzung ist die Plattenosteosynthese angezeigt. Bei ligamentärer Sprengung der Kreuzdarmbeinfuge kann die transarticuläre Fixation mit zwei 6,5-mm-Schrauben empfohlen werden, die 3 – 4 cm ventral der Spina iliaca superior posterior in einem Winkel von 15 – 20 Grad zur Frontalebene nach ventral eingebracht werden.

Summary If there is no complete rupture of the posterior ring segment, one of the ways to stabilize the posterior pelvic ring segment is by ventral insertion of an internal plate and external osteosynthesis. Complete rupture of the posterior pelvic ring requires: (1) reconstruction of the posterior ring; (2) plating of the ventral segment. Osteosynthesis by plating is indicated for bone injuries. In cases of ligamentary rupture of the iliosacral joint, we recommend transarticular fixation by two 6.5-mm screws of the ASIF system, in a ventral position 3–4cm from the iliac spine, posterior superior, at an angle of 15–20° to the frontal plane.
