Zusammenfassung Bei 44 613 Endoskopien am oberen Gastrointestinaltrakt seit 1969–1984 ereigneten sich 55 (0,12%) Perforationen der Speiseröhre (diagnostische ÖGD 1/42 378, Fremdkörperextrak-tion 1/198, Oesophaguswandsklerosierung 4/862, pneumatische Dilatation 5/82, Bougierung 13/629, PEP 31/311, Polypektomie 0/12, Mediastinoskopie 0/134, Thoracoskopie 0/7). 52/55 Perforationen wurden innerhalb von 24 h erkannt. Es wurden 38 Perforationen konservativ, 17 operativ behandelt. Die Letalität (bis 30 Tage) betrug 3/38 (8%) bzw. 5/17 (29%). Voraussetzung zur konservativen Therapie sind kein Leck zur Pleura- oder Peritonealhöhle, breite Drainage zum Oesophaguslumen und minimale Symptomatik.

Summary Among 44,613 endoscopic investigations of the upper gastrointestinal tract since 1969–1984, 55 perforations of the esophagus have occurred: diagnostic EGD (1/42,378), foreign body extraction (1/198), sclerotherapy (4/862), pneumatic dilation (5/82), bouginage (13/629), PEP (31/ 311), polypectomy (0/ 12), mediastinoscopy (0/ 134), and thoracoscopy (0/7). Thirty-eight perforations were treated conservatively, 17 surgically. Perforations were diagnosed within 24 h in 52/55. Lethality (within 30 days) was 3/38 (8%) and 5/17 (29%), respectively. The criteria supporting conservative therapy are absence of leakage to the pleural or peritoneal cavity, wide drainage back into the esophagus, and minimal signs of sepsis.
