    Notes: Objective To determine the safety and efficacy of intramuscular oxytocin plus ergometrine compared to intravenous oxytocin for prevention of postpartum haemorrhage, and the significance of administration at the end of the second stage of labour compared with that after the third stage.Design A prospective cohort study.Setting A university affiliated tertiary medical centre.Participants Two thousand one hundred and eighty–nine women delivering singletons during 40 consecutive weeks.Main outcome measures Postpartum haemorrhage (〉 500 ml), prolonged third stage (〉 30 min), retained placenta (〉60min), elevated blood pressure (systolic 〉 150 mmHg, diastolic 〉 100 mmHg).Results The rate of postpartum haemorrhage was not significantly different for oxytocinergometrine compared with oxytocin, when administered at the end of the second stage of labour (odds ratio 1.10, 95% confidence interval (CI) 0.75–1.61) or after the third stage (odds ratio 0.95, 95% CI 0.68–1.34). The patients receiving oxytocics at the end of the second stage of labour had significantly lower rates of postparturn haemorrhage, for both oxytocinergometrine (odds ratio 0.69, 95% CI 0.49–0.98) and oxytocin (odds ratio 0.60, 95% CI 0.41–0.87), compared with those treated after the third stage.Conclusion Administration of oxytocin alone is as effective as the use of oxytocin plus ergometrine in the prevention of postpartum haemorrhage, but associated with a significantly lower rate of unpleasant maternal side effects. Oxytocics administered after delivery of the fetal head compared with after the placental expulsion are associated with a significantly lower rate of postpartum haemorrhage.

    Notes: We present a technique for using modulated electroabsorption measurements to determine the built-in potential in semiconductor heterojunction devices. The technique exploits a simple relationship between the second-harmonic electroabsorption signal and the capacitance of such devices. We apply this technique to hydrogenated amorphous silicon (a-Si:H)-based solar cells incorporating microcrystalline Si p+ layers. For one set of cells with a conventional plasma-deposited intrinsic (i) layer we obtain a built-in potential of 0.98±0.04 V; for cells with an i layer deposited using strong hydrogen dilution we obtain 1.25±0.04 V. We speculate that interface dipoles between the p+ and i layers significantly influence the built-in potential. © 1996 American Institute of Physics.

    Notes: Aqueous geochemical data from unconfined sand aquifers beneath two operating domestic septic systems are used to illustrate and evaluate a conceptual model of septic-system geochemistry. This model emphasizes the changing redox and alkalinity conditions in the septic system and the subsurface. The septic-tank effluents flow as distinct plumes downward through the unsaturated zones and then primarily laterally in the ground-water zones. The composition of the effluent was measured at several points in each system. At each site, the septic-tank effluent underwent aerobic oxidation in the unsaturated zone, which caused conversion of NH4+ to NO3−, organic C to CO2 and organic S to SCh42-. At the first site, calcium carbonate dissolution in the unsaturated zone buffered the acidity released by the redox reactions. In contrast, the second system was poorly buffered and the pH dropped from 6.7 to 4.9 as aerobic oxidation occurred. Below the water table a small amount of aerobic oxidation occurred at each site. Nitrate-N concentrations in the cores of both plumes were above 25 mg/1 as the plumes traveled from the septic systems. At the second site, the ground-water plume discharges to a river at the edge of the property. As the effluent flowed through the organic C-rich sediments of the river bed, NO3− disappeared and alkalinity increased, presumably due to denitrification. Differences in sediment composition at the two sites also led to different behaviors of Fe, Al, and possibly PO43-. The conceptual model offers an organized approach to interpreting the major geochemical trends observed in the two systems, which are determined mostly by the well-aerated unsaturated zones below the drain fields and the amount of buffering material present in the sediments.

    Notes: In 1974, the Securities and Exchange Commission (SEC) noted that an increasing number of companies were capitalizing interest costs, and that this practice was not being adequately disclosed (FASB, 1979, par. 26). In light of the alternative practices concerning the accounting for interest and lack of adequate disclosure by companies that were already capitalizing interest, the SEC recommended that the Financial Accounting Standards Board (FASB) consider the issue of accounting for interest cost. As a result of the SEC's initiative, in 1979 the FASB issued Statement of Financial Accounting Standards [SFAS] No. 34, Capitalization of Interest Cost, which mandated uniform interest capitalization rules in accounting for interest costs associated with the acquisition of qualifying non-current assets.The purpose of this article is to examine SFAS 34 in terms of its financial statement impact, the congruence of its assumptions with economic behaviour, its effect on subsequent standards related to interest capitalization, and its implications on financial accounting standard setting.To explore these issues we first illustrate the extent to which interest capitalization affects financial statements. We then empirically analyse the measure employed in SFAS 34 for the capitalization of interest cost in cases where debt is not directly linked with the acquisition of qualifying non-current assets. In addition, we critically examine the treatment accorded interest cost in subsequent FASB standards. Our research suggests that SFAS 34′s rationale for interest capitalization is incompatible with firm behaviour, and that the rules for interest capitalization as reflected in various accounting standards are inconsistent. These findings suggest that in the case of interest capitalization the benefits of comparability in financial reporting are not realized. A policy recommendation is then offered to alleviate some of these difficulties. The recommendation is to disallow the capitalization of interest cost in the absence of a direct link between the debt and the acquisition of qualifying assets.

    Notes: Objective To evaluate the outcome of late selective fetal termination based on combined data from eight tertiary perinatal centres.Design Israeli law requires institutional committee approval for pregnancy termination. Moreover, after 24 weeks only a district superior committee can approve the termination. A total of 36 bichorionic twin pregnancies who underwent selective fetal termination after 24 complete gestational weeks were identified in eight Israeli centres.Results In 23 women (63.9%) the indication for selective termination was structural anomaly, and in 13 (36.1%) the indication was chromosomal abnormality. The mean gestational age at the time of diagnosis was 24.1 ± 1.9 weeks. The termination procedure was performed at a mean gestational age of 25.5 ± 2.0 weeks (median 25, range 24–33 weeks). There was one case of immediate procedure-related complication (i.e. amnionitis) and the unaffected infant died, which was the only perinatal death in this series. No evidence of coagulopathy was reported. Only five women (13.8%) delivered before 34 completed weeks. The mean gestational age at delivery was 36.9±2.9 weeks (28–41 weeks), and the mean procedure-to-delivery interval was 11.8±3.2 weeks (median 13, range 3–17 weeks).Conclusions Late (〉 24 weeks) selective termination in twin pregnancies is associated with favourable perinatal outcome of the healthy twin. In countries where the law permits late pregnancy termination, the parents should be informed of that possibility.
