    Notes: The coaxial plasma accelerator is a simple, compact, and mechanically robust device that utilizes the Lorentz J×B force to accelerate plasma to high velocity. Originally developed in the 1950s for the purpose of providing energetic plasmas for fusion energy experiments, coaxial plasma accelerators are presently being investigated as an environmentally sound and economical means of materials processing and advanced manufacturing. While commercial applications of this technology are already on line, future commercial applications will require improving accelerator reproducibility and efficiency, better controlling the accelerated plasma flow velocity or energy, and better controlling the distribution of directed energy or power on target. In this paper, the magnetohydrodynamic flow physics of magnetically nozzled plasma accelerators is presented with a view to achieving the accelerator control necessary for future industrial applications. Included is a fundamental description of plasma production, acceleration, and flow in a magnetic nozzle. © 1998 American Institute of Physics.

    Notes: Transseptal Atrioventricular Nodal Ablation. To our knowledge, prograde access to the left ventricle to perform a left-sided AV node (AVN) ablation has not been reported. We present the case of a patient with Noonan's syndrome and supravalve aortic stenosis who required aortic valve replacement with a mechanical valve. Due to chronic atrial fibrillation, a transvenons pacing system was implanted and transcatheter radiofrequency ablation of the AVN was performed. Attempts at right-sided AVN ablation were unsuccessful. Due to the presence of the mechanical aortic valve, a left-sided AVN ablation was performed successfully using the transseptal approach. The patient died suddenly 6 months after the procedure, and the histologic study of the heart is reported.

    Notes: Initial reports of percutaneous transluminal coronary angioplasty (PTCA) in the elderly (≥: 75 years) showed a significantly lower primary success rate, higher in-hospital mortality, and a higher risk of emergency or elective coronary artery bypass graft (CABG) compared to younger patients. There are few data concerning acute outcomes and clinical follow-up after the use of coronary stenting in the elderly compared to 〈 the 75-year-old age group. We evaluated 82 elderly patients and 280 younger patients who received Palmaz-Schatz stents during 1995, at a time when high pressure deployment and antiplatelet therapy was routinely used. The success rate and acute major complications were not significantly different between the elderly and younger patients. Clinical events (death, myocardial infarction [MI], repeat PTCA, or CABG) during 6-month follow-up were also not significantly different. Coronary stenting in the elderly can be carried out with a high success rate and low incidence of acute major complications. Thus, short-term clinical outcomes in elderly patients appear similar to results obtained in younger patients.

    Notes: Abstract: S100β has been implicated in the formation of dystrophic neurites, overexpressing β-amyloid precursor protein (βAPP), in the β-amyloid plaques of Alzheimer's disease. We assessed the effects of S100β on cell viability of, neurite outgrowth from, and βAPP expression by neurons in primary cultures from fetal rat cortex. S100β (1–10 ng/ml) enhanced neuronal viability (as assessed by increased mitochondrial activity and decreased lactic acid dehydrogenase release) and promoted neurite outgrowth. Higher levels of S100β (100 ng/ml, but not 1 µg/ml) produced qualitatively similar, but less marked, effects. S100β also induced increased neuronal expression of the microtubule-associated protein MAP2, an effect that is consistent with trophic effects of S100β on neurite outgrowth. S100β (10 and 100 ng/ml) induced graded increases in neuronal expression of βAPP and of βAPP mRNA. These results support our previous suggestion that excessive expression of S100β by activated, plaque-associated astrocytes in Alzheimer's disease contributes to the appearance of dystrophic neurites overexpressing βAPP in diffuse amyloid deposits, and thus to the conversion of these deposits into the diagnostic neuritic β-amyloid plaques.

    Notes: Abstract: Previous experiments suggested that the human cell adhesion molecule L1 interacts with different integrins via its sixth immunoglobulin-like domain in an RGD-dependent manner. Here we have described the expression of this domain from early postnatal mouse brain, analyzed the structure of the recombinant protein by circular dichroism and fluorescence spectroscopy, and performed solid-phase binding studies to αvβ3, αIIbβ3, and α5β1 integrins. The domain was found to have the expected β-sheet organization, which was lost in the presence of guanidine hydrochloride. The midpoint of the single-step transition occurred at 1.5 M guanidine hydrochloride. The sixth immunoglobulin-like domain of mouse brain L1 contains two RGD motifs and was found to bind in a concentration-dependent and saturable way to αvβ3, αIIbβ3, and α5β1 integrins, suggesting specific interactions with these ligands. However, only the interaction to αvβ3 could be inhibited in a concentration-dependent manner by an RGD-containing peptide, and the IC50 was determined to be ∼20 nM. Mutants of the domain, which lack either one or both of the RGD sites, demonstrated that the RGD site comprising residues 562–564 is involved in the interaction to αvβ3. Our findings indicate an RGD-independent mechanism for the interactions to αIIbβ3 and α5β1, as no involvement of any RGD motif could be demonstrated.

    Notes: Simulations of a room-temperature p-i-p coherently strained Si0.5Ge0.5/Si superlattice quantum-parallel laser diode have been made. Calculations have been made of the local-in-k-space population inversion between the nonparabolic heavy-hole valence minibands, HH2 and HH1. Lasing is at 5.4 μm and the optical dipole matrix element is 3.7 A. Analysis of radiative-and-phonon scattering between the "mixed" bands indicates a lifetime difference between the upper and lower states of 2.4 ps. At an injected current density of 5000 A/cm2, a laser gain of 134 cm−1 is calculated. © 1998 American Institute of Physics.

    Notes: Prior to the advent of health maintenance organizations (HMOs), indemnity insurance plans provided economic incentives that fueled a significant portion of the cost crisis in health care. Although managed care successfully curbed many of the ethical and service-inflation problems that resulted from fee-for-service systems, the powerful economic incentives that fueled these systems still remained but in opposite form: Profits to HMOs tend to increase as service quantity declines. As the industry has matured, a number of new ethical questions have arisen specific to managed care. The authors examine these questions and take the view that mental health HMOs require legislative safeguards to ensure adequate consumer protection and service delivery. Psychotherapy outcome research and current clinical practices in managed care are examined to support this view. A “unionization” of providers around a managed care-specific body of ethical guidelines is proposed.

    Notes: Enhancement of the photocurrent density generated by internal moving space charge electric fields in semi-insulating GaAs multiple quantum wells is observed when the sample is biased with a high speed, zero-mean square wave electric field which increases the internal space charge electric field strength. Tunability of the space charge field formation time is also demonstrated by adjusting the amplitude of the bias signal. This technique can be used to enhance the sensitivity of optical doppler frequency measurements based on moving space charge field effects in photoconductive semiconductors. Electronic tunability of the space charge field formation time also adds versatility in optical power spectrum analysis applications. © 1998 American Institute of Physics.

    Notes: Abstract. The pathogenesis of tobacco-related periodontal diseases is not well understood. The purpose of this study was therefore to investigate smokeless tobacco extract (ST) and nicotine effects on prostaglandin E2 (PGE2) and interleukin-1β (IL-1β) secretion by peripheral blood mononuclear cells (PBMC, consisting of monocytes and lymphocytes) and gingival mononuclear cells (GMC). Both peripheral blood and gingival tissue adjacent to the alveolar crest were taken from non-smoking adult periodontitis patients. Gingival tissue was treated with collagenase and deoxyribonuclease and GMC and PBMC were isolated by Ficoll-Hypaque centrifugation. GMC and PBMC (100,000 cells/200 μl) were cultured for 24 hours in supplemented RPMI 1640 alone (control), or in supplemented RPMI 1640 containing 1% ST, 100μg/ml nicotine, 1 μg/ml Porphyromonas gingivalis LPS, or 1 μg/ml P. gingivalis LPS and either 100 μg/ml nicotine or 1% ST. Enzyme immunoassays were used to quantity PGE2 and IL-1β. Treatments were compared by repeated measures ANOVA. 100 μg/ml nicotine (7-fold, p〈0.02) and 1% ST (3.5-fold, p〈0.004) significantly increased secretion of PGE2 by PBMC relative to control cultures. 100 μg/ml nicotine and 1% ST, however, had no effect on IL-1β secretion by PBMC. Enhanced PGET secretion also was seen when PBMC were treated with P. gingivalis LPS+100 μg/ml nicotine relative to P. gingivalis LPS alone (p〈0.007). In contrast, 100 μg/ml nicotine significantly downregulated IL-1β secretion by GMC relative to medium alone (p〈0.008) and had no effect on PGE2 secretion by GMC, These data indicate that while nicotine and ST can stimulate PBMC to secrete PGE2, they cannot activate further mononuclear cells extracted from gingiva, possibly due to maximal previous stimulation in the periodontitis lesion.
