    Notes: Abstract The present study was conducted to evaluate the effectiveness of percutaneous papillary balloon dilatation (PPBD) as a therapeutic option for cholecystocholedocholithiasis, in combination with laparoscopic cholecystectomy (LC). A total of 15 patients with both bile duct and gallbladder stones were clinically investigated. In 14 patients, PPBD was performed 2 to 7 days prior to LC, while in the remaining patients, PPBD was performed immediately after LC under general anesthesia in one continuous session. The bile duct stones were successfully pushed out into the duodenum in all the patients, seven of whom required two sessions for complete stone clearance, while the other eight needed only one session. Two patients had bile duct stones larger than 12mm in diameter, necessitating electrohydraulic lithotripsy under cholangioscopy. The insertion of a percutaneous transhepatic biliary drainage tube did not cause intra-abdominal adhesions severe enough to contraindicate the use of LC. The operation times for LC varied from 80 to 184 min, with a mean operation time of 132 min, and the average postoperative and overall hospital stays were 9 and 26 days, respectively. there were no deaths or major complications, apart from transient hyperamylasemia. The findings of this study indicate that PPBD combined with LC is a safe and effective therapeutic option for cholecystocholedocholithiasis.

    Notes: Abstract  We examined bimanual coordination in a patient before and after each stage of callosotomy surgery. We tested how well the patient coordinated movement direction between the hands. The patient drew symmetrical or asymmetrical figures simultaneously with both hands. Before surgery, symmetrical figures were drawn well and asymmetrical figures were drawn poorly. Following anterior callosotomy, the drawings improved slightly. Symmetrical figures were still drawn well, and asymmetrical ones were still drawn poorly. Thus, spatial integration remained intact despite the loss of interhemispheric communication between frontal cortical sites. After posterior callosotomy, spatial coordination deteriorated significantly. Mirror-image drawings became less symmetrical, while asymmetrical drawings improved. These data indicate that the posterior callosum mediates the coordination of direction information between the hands during bimanual movements. Given the topographical organization of the corpus callosum, this integration is likely carried out by parietal cortex.

    Notes: Abstract We studied sexual disease transmission behaviors among patients of a county sexually transmitted disease (STD) clinic in rural North Carolina. Six hundred-sixty patients responded to a 147-item questionnaire. Among those with symptoms (N = 358), 50% of the men and 66% of the women came to the clinic a week or more after experiencing symptoms of a curable infection. Sixty-eight percent of the men and 39% of the women reported having multiple sex partners in the last 3 months. Fifty-nine percent of men and women reported never using a condom during vaginal sex with their main partner. Condom non-use was twice as likely with main partners than with casual ones. Factors independently associated with condom non-use (older age, cohabitation, non–African-American race, and social norms for risky sex) did not vary by partner type after the correlational nature of these data was taken into account. STD-related behaviors among STD clinic clients in this rural county were at least as frequent as reported for urban populations. The relative absence of anonymity in a small community may play a role in several of the behaviors.
