    Notes: EOG correction is a class of techniques that account for ocular artifact in the electroencephalogram (EEG) by subtracting electrooculographic data from the EEG. The purpose of this study was to evaluate four of these correction techniques (Verleger, Gasser, & Möcks, 1982 [VGM]; Gratton, Coles, & Donchin, 1983 [GCD]; Semlitsch, Presslich, Schuster, & Anderer, 1986 [SPSA]; Croft & Barry, 2000 [CB]). Blinks, vertical eye movements (VEM), and horizontal eye movements (HEM) from 26 subjects were corrected using these techniques, and eye movement event-related potentials computed to aid validation. HEMs were corrected better by CB, VGM/GCD then SPSA, VEMs by CB, VGM/GCD then SPSA, and blinks by CB, SPSA, GCD and then VGM, with the advantage of CB substantial for blinks (η2〉.72), VEMs (η2〉.60), and HEMs (η2〉.27). It is argued that the CB procedure adequately accounts for ocular artifact in the EEG. Reasons for the limitations of the other procedures are discussed.

    Notes: We report on the laser-induced permanent changes of the critical current (Ic) and normal resistance (Rn) of YBa2Cu3O7−x (YBCO) step-edge Josephson junctions. The 2- to 20-μm-wide junctions were prepared from a 200-nm-thick YBCO film deposited by a pulsed KrF excimer laser onto 300-nm-high steps etched in the LaAlO3 substrate. The laser modification experiments were performed by illuminating the junctions at 50 K with a focused Ar-ion laser beam of various intensities. Depending on the illumination power density, either increase or decrease of the junction Ic has been observed. In particular, after illumination at the 0.6×105 W/cm2 power level, a 75% enhancement of Ic and increase of the IcRn product up to 25% were obtained without a measurable change in the junction critical temperature. The laser-induced modifications were very reproducible and remained unchanged even after a subsequent room-temperature/helium thermal cycling of the sample. Photoassisted, thermally activated oxygen redistribution in the YBCO grain boundary region is proposed to explain the observed behavior. © 1995 American Institute of Physics.

    Notes: Background:  The established pathologic criteria for minor salivary gland (MSG) involvement in chronic graft-vs.-host disease (cGVHD) could play a role in monitoring response to therapy.Methods:  We evaluated MSG sequential biopsies during cGVHD therapy in 14 allogeneic bone marrow transplantation (BMT) patients. Nine patients that did not develop GVHD after BMT entered the control group. Biopsies were examined using hematoxylin-eosin, Periodic acid-Schiff (PAS) and leukocyte common antigen staining.Results:  A significant loss of PAS+ acinar volume was observed at the diagnosis of cGVHD as much as at the end of treatment when compared with the control group. In the second evaluation, the inflammatory infiltrate was still greater than control group.Conclusions:  The results suggest that persistent xerostomia after cGVHD treatment is because of maintenance of lymphocytic infiltrate and consequent absence of MSG secretory unit recovery. This data may be useful to provide improved insight into the histopathology of this organ involvement.

    Notes: Abstract PURPOSE: Patients with cardiopulmonary compromise who require transabdominal colon and rectal procedures are at increased risk for postoperative cardiac and pulmonary complications and prolonged hospital stays. Because epidural anesthesia has been shown to minimize reductions in functional residual capacity and consequently improve pulmonary function, we sought to determine its impact on patients undergoing colon and rectal procedures who were at high risk for postoperative cardiopulmonary morbidity. METHODS: We prospectively studied 31 patients undergoing colon and rectal procedures in whom epidural anesthesia with spontaneous ventilation were used. The criteria for entry included histories of respiratory insufficiency, atherosclerotic heart disease, and extremes of age. Patients who received general endotracheal anesthesia were excluded from consideration. Cardiac and pulmonary morbidity, mortality, and length of stay were analyzed. The data obtained were compared with those of a similar group of 50 patients who during the same time interval had undergone transabdominal colon and rectal operations with general anesthesia in the absence of epidural anesthesia. RESULTS: There were 19 males and 12 females in the epidural study group. Mean age was 71 (range, 35–92) years. There were no cases of pulmonary morbidity. Cardiac morbidity was 6 percent (2/31), with a mortality rate of 3 percent (1/31). Average length of postoperative hospital stay was 10.5 (range, 7–19) days. There were 29 males and 21 females with a mean age of 67 (range, 51–92) years in the general anesthesia group. Pulmonary morbidity was 18 percent (9/50). Incidence of cardiac complications was 4 percent (2/50). There were no mortalities. Average length of stay in the general anesthesia group was 13.6 (range, 6–24) days. CONCLUSION: Use of epidural anesthesia with spontaneous ventilation in elective transabdominal colon and rectal procedures may decrease the incidence of pulmonary complications and length of postoperative hospital stay in a select group of high-risk patients.

    Description / Table of Contents: Summary A prospektive multidimensional study with 71 remitted bipolar outpatients (37 bipolars without, 34 with psychotic symptoms during acute mania) was made to assess relevant influential factors on relapse and readmission to hospital: characteristic data of previous course, psycho(patho)logical state and personality after remission, personality disorders, and ways of coping. In the course of the 5 years after the first examination, 48% of the patients were readmitted, 60% of them because of a (schizo)manic state. The study confirms the prognostic relevance of past course, e.g. number of previous episodes and mania-quotient. A further risk-factor are residual psychopathological alterations which persist after remission („negative symptoms” of strictly bipolars, increased vulnerability for dynamic derailment for the bipolars with psychotic symptoms). Patients with „syntonic” personality in the free interval suffered less relapses. The relevance of other personality traits differed in the diagnostic subgroups of nonpsychotic and psychotic bipolars. Active coping was positively correlated with staying healthy, but the correlations were weak.
    Notes: Zusammenfassung Im Rahmen einer prospektiven, mehrdimensionalen Studie werden wesentliche Faktoren für die Rückfälligkeit und erneute Hospitalisierung von 71 bipolar manisch-depressiven Patienten (37 Bipolare „ohne”, 34 „mit psychotischen Merkmalen”) untersucht. In den 5 Jahren seit Erstuntersuchung waren 48% der Patienten erneut hospitalisiert worden, davon 66% wegen eines (schizo)manischen Syndroms. Geprüft wurden in ihrem Gewicht für die Rückfälligkeit im Beobachtungszeitraum: Verlaufsdaten bis zur Erstuntersuchung, Intervallpersönlichkeit und kategoriale Persönlichkeitsstörungen, Befinden und residuale psychopathologische Veränderungen im Intervall sowie Strategien der Krankheitsverarbeitung. Dabei erwies sich das prognostische Gewicht des bisherigen Verlaufs insbesondere hinsichtlich Phasenzahl und Maniequotient. Ein weiterer Risikofaktor sind residuale psychopathologische Veränderungen, die trotz Abklingen der Akutsymptomatik im Intervall fortbestehen. Patienten mit syntoner Intervallpersönlichkeit erlitten wenig Rückfälle. Die Bedeutung anderer Persönlichkeitszüge differierte in den beiden diagnostischen Untergruppen. Die Beziehungen zwischen Copingvariablen und Rückfälligkeit lagen zwar durchgängig in der erwarteten Richtung, wurden aber nicht signifikant.

    Notes: Abstract Context: Long-term survival of patients with metastatic colorectal cancer has been achieved only in patients who underwent complete resection of metastases. Such surgery could be performed in a greater proportion of patients if effective chemotherapy could downstage previously unresectable metastases. This approach has been limited by the low tumor response rate achieved with conventional chemotherapy. Objective: We studied the outcome of patients with initially unresectable liver metastases from colorectal cancer treated with a three-drug chemotherapy regimen followed by liver metastases surgery whenever possible. Patients and methods: From March 1988 to June 1994, 151 patients with colorectal liver metastases were considered initially unresectable because of large tumor size (〉5 cm), multinodular (〉4) or ill-located metastases. All patients received fully ambulatory chemotherapy with 5-fluorouracil, leucovorin and oxaliplatin (chronotherapy in 83% of them). They were periodically reassessed for surgery by a joint medico-surgical team. Results: In 151 patients, the size of liver metastases decreased by 〉50% in 89 patients (59%) and median overall survival was 24 months (95% confidence interval (95% CI): 19–28 months), with 28% surviving at five years (20%–35%). Surgery with curative intent was attempted in 77 patients (51%), complete resection of liver metastases was achieved in 58 patients (38%). The median survival of the 77 operated patients was 48 months (25–71), with a five-year survival rate of 50% (38–61). Conclusion: This new strategy of combining effective chemotherapy with surgery apparently altered the natural history of unresectable colorectal cancer metastases.

    Notes: Abstract A short warm ischemia before reperfusion has been shown to improve the tolerance of the heart and the liver to a prolonged warm ischaemia. The present experimental study was conducted to evaluate the effect of such preconditioning on hepatic tolerance to an extended cold ischemia. In a model of isolated perfused liver, livers from Wistar rats (250–350 g) were stored for 24 h in UW (4 °C) immediately after harvesting and reperfused for 3 h at 37 °C. Control livers subjected to a 24-h cold ischemia were compared to livers subjected to preconditioning (defined as a 5- or 10-min clamping of the hepatic pedicle followed by a 10-min reperfusion before liver harvesting) prior to the definitive 24-h cold ischemia. While there was no difference in bile production between the preconditioned groups and the controls, transaminases and LDH release was significantly increased, vascular resistance was enhanced, and preservation injury was more extensive in both preconditioned groups as compared to controls. In contrast to the beneficial effect reported on prolonged warm ischaemia, preconditioning has a deleterious effect on hepatic tolerance to an extended cold ischemia.

    Notes: Abstract Purpose. The amount and intracellular distribution of DNA fragments (491 -bp) was characterized after transfection in vitro with a commercially available cationic lipid. Localization of fragment to the nucleus, its subcellular distribution, and integrity within the cells was determined for various times after transfection. Methods. Cystic fibrosis (CF) airway epithelial cells were transfected with 32P and FITC labeled single-stranded (ss) or double-stranded (ds) DNA fragments complexed with Lipofectamine® at various charge ratios. Results. A 5/1 (+/−) charge ratio was found to be the optimal ratio for transfection of both ss-and dsDNA. After a 5 h exposure, 7.51 ± 0.89% of the radioactivity was associated with the nuclear fraction whereas only 1.07 ± 0.23%, was found in the nuclear fraction when dsDNA was used. The nuclear radioactivity detected after a 24 h exposure was only 1/3 of that after 5 h. Analysis of fragment stability in the cytosolic and nuclear fractions showed the presence of intact fragment in each subcellular compartment. No intranuclear/intracellular fragment could be detected in control experiments with naked DNA. Conclusions. The results from these experiments indicate that small fragments of DNA can be efficiently and rapidly transferred intact to the cell nucleus using cationic lipids and that ssDNA fragments are more effective than dsDNA fragments for nuclear delivery.

    Notes: Abstract The morphology and ultrastructure of the digestive gland ofBithynia siamensis goniomphalus and its alteration by infection withOpisthorchis viverrini were investigated by light and electron microscopy. The digestive gland ofB. s. goniomphalus was composed of three different cell types: digestive cells, excretory cells, and narrow cells. In infected animals the number of excretory cells increased dramatically. Cellular injury in digestive cells as well as in excretory cells following the infection could be observed at the ultrastructural level.
