    Notes: Abstract:  Matrical differentiation in basal cell carcinoma (BCC) is rare. Only nine cases have been described that showed typical diagnostic features of BCC, in addition to shadow cells indicating hair-matrix differentiation.1–5 These cases often present a diagnostic challenge due to confusion with pilomatrixoma or pilomatrix carcinoma. We present a case of BCC with matrical differentiation in a 78-year-old man. Immunohistochemical and molecular methods are used to differentiate this lesion from benign or malignant forms of pilomatrixoma.

    Notes: From the mid-1940s through the 1980s, large volumes of waste water were discharged at the Hanford Site in southeastern Washington State, causing a large-scale rise (〉20 m) in the water table. When waste water discharges ceased in 1988, ground water mounds began to dissipate. This caused a large number of wells to go dry and has made it difficult to monitor contaminant plume migration. To identify monitoring wells that will need replacement, a methodology has been developed using a first-order uncertainty analysis with UCODE, a nonlinear parameter estimation code. Using a three-dimensional, finite-element ground water flow code, key parameters were identified by calibrating to historical hydraulic head data. Results from the calibration period were then used to check model predictions by comparing monitoring wells’ wet/dry status with field data. This status was analyzed using a methodology that incorporated the 0.3 cumulative probability derived from the confidence and prediction intervals. For comparison, a nonphysically based trend model was also used as a predictor of wells’ wet/dry status. Although the numerical model outperformed the trend model, for both models, the central value of the intervals was a better predictor of a wet well status. The prediction interval, however, was more successful at identifying dry wells. Predictions made through the year 2048 indicated that 46% of the wells in the monitoring well network are likely to go dry in areas near the river and where the ground water mound is dissipating.

    Notes: Background:  Apoptosis is important for maintenance of tissue homeostasis and often dysregulated in cutaneous neoplasms. The apoptosis inhibitor survivin is expressed in melanoma and non-melanoma skin cancers and benign keratinocytic lesions. Its expression has not been studied in melanocytic nevi.Objective:  We determined the expression pattern of survivin in benign melanocytic nevi in comparison to markers of proliferation and apoptosis.Methods:  Six cases of each of the following melanocytic nevi were retrieved from a dermatopathology archive: compound dysplastic nevus, intradermal nevus, compound nevus, neurotized intradermal nevus, and Spitz nevus. Survivin expression was evaluated by in situ hybridization. Apoptotic and proliferation indices were calculated by counting immunoreactive cells in terminal deoxynucleotidyl transferase (TdT)-mediated dUTP nick end labeling and proliferating cell nuclear antigen immunostained sections, respectively.Results:  All nevi, regardless of histologic type, expressed survivin. Compound melanocytic lesions expressed survivin in both epidermal and dermal compartments. The apoptotic rate was low for dysplastic, compound, and Spitz nevi, and apoptotic cells were not identified in any neurotized nevus. The proliferative index was highest for Spitz nevi, while all other nevi demonstrated rare positive cells.Conclusions:  Survivin is consistently expressed in benign melanocytic lesions, while apoptotic cells are rarely identified, suggesting the dysregulation of apoptotic pathways with the accumulation of cells in these neoplasms.

    Notes: Tattoo pigment in sentinel lymph nodes in melanoma patients is a clinical challenge. A 77-year-old man presented with a pink nodular lesion of the right upper arm. Biopsy revealed a 1.9 mm thick, Clark level 4 nodular melanoma. A wide local excision with 2 cm margins and sentinel lymph node (SLN) mapping was planned. Pre operative lymphoscintigraphy using technetium-99 m demonstrated a single strongly pigmented SLN in the right axilla. A prominent tattoo was located on the patient’s upper arm in the area of the melanoma. The clinical impression was highly suspicious for metastatic disease and the tissue was sent to pathology for examination. Frozen section showed a lymph node with benign histiocytes. There were black non-polarizable coarse granules in the subcapsular, sinusoidal areas and also in the histiocytes. Review of permanent hematoxylin and eosin sections as well as immunohistochemical analysis with MART-1 and HMB-45 showed no evidence of metastatic melanoma in the SLN. Pigmentation of lymph nodes is not pathognomic of malignant melanoma. Black pigmented lymph nodes can occur by carbon deposits of tattoo pigment as in our case. Histological confirmation of metastatic malignant melanoma in the SLN is imperative before proceeding to complete regional lymph node dissection.
