    Notes: SUMMARY: Hypertension is an important and well-established risk factor for both cardiovascular and cerebrovascular disease. Hypertension is much more common in patients on renal replacement therapy than in the general population. Up to 80% of patients on renal replacement therapy are hypertensive and about 50% of dialysis patients die from cardiovascular causes. Salt and water overload are major factors exacerbating hypertension in the dialysis population. This was a prospective crossover study of 10 patients examining the effect of haemodialysis for 2 weeks using usual (Na+ 138–140 mmol/L) sodium dialysate with a 2-week period of low (reduced by an average of 5 mmol/L Na+ to 133 mmol/L on average) sodium dialysate on inter-dialytic ambulatory blood pressure (ABPM) and trans-thoracic bioimpedance (TTB). Ten patients, mean age 67 years, completed the study (two women and eight men). No patient became severely hyponatraemic during the study period. Mean 48 h inter-dialytic blood pressure (BP) fell from 141/83 to 133/78 (P 〈 0.01). Mean arterial BP measured immediately prior to TTB fell from 92.8 mmHg to 87.5 mmHg (P 〈 0.01) during the low-sodium haemodialysis period. Afterload (systemic vascular resistive index – SVRI) measured by TTB fell significantly during the low-sodium haemodialysis period (SVRI on Na+-140 = 3426 cf. Na+-134 = 2281; P = 0.01). Dialysate sodium reduction without extra fluid removal had a beneficial effect on inter-dialytic 48-h blood pressure in chronic stable haemodialysis patients. Lowering dialysate sodium reduced the systemic vascular resistance index as measured by TTB. Reduction of dialysate sodium was well tolerated, although mild dizzines and cramps did occur. These data suggest that sodium overload and water overload may have independent effects on BP and that simple-to-achieve and modest changes in dialysate sodium could usefully augment the action of antihypertensives in dialysis patients.

    Notes: Muscle (body wall, buccal mass, heart) and neural tissue of the marine mollusc Aplysia californica was analysed for calcium-binding proteins using transblot45Ca overlay, Western blotting and two-dimensional polyacrylamide gel electrophoresis, and several low molecular weight calcium-binding proteins were identified. Our results show that Aplysia muscle contains an abundant protein with a Mr of ∼20 000 with strong 45Ca2+-binding ability and cross-reactivity to antibodies against the sarcoplasmic calcium-binding protein isoform II (SCP II) from Amphioxus. Immunocytochemical studies revealed that isoforms of SCP are distributed in a tissue-specific manner, SCP II-like protein is exclusively present in muscle fibres closely associated with the contractile machinery, whereas the isoform I (SCP I-like protein) is exclusively present in a subset of neurons, suggesting a function in their calcium regulation. In addition, a novel 45Ca2+-binding protein of Mr 43 000, pl 4.7, was found in muscle and in neurons. A third protein of Mr 40 000, pl 4.8, cross-reacts with anti-parvalbumin and anti-calbindin D-28K antibodies.

    Notes: Summary Genital warts are one of the most common sexually transmitted infections, and are caused by human papillomavirus (HPV) types 6 and 11. Standard treatments for genital warts include cryotherapy, laser therapy, trichloroacetic acid and podophyllotoxin. We report the case of a 21-year-old female with extensive genital warts. A patient-applied, non-destructive therapy was considered to be the most appropriate treatment in this case, due to the extent of the disease and the resulting psychological distress experienced by the patient. She applied imiquimod 5% cream three times per week for a period of 5 weeks, which resulted in complete clearance of all the warts. Minor inflammatory changes were observed during treatment; however, no significant pain was by reported the patient. No recurrences were reported during 2 years of follow-up.

    Description / Table of Contents: Résumé On a réalisé des biopsies du triceps chez 11 sujets qui avaient présenté une rupture spontanée du tendon d'Achille au cours d'un exercice sportif. Toutes les pièces montraient d'importantes modifications histologiques et ultra-microscopiques, dont la plus frappante était la présence d'une grande quantité de gouttelettes lipidiques à l'intérieur des fibres musculaires. Une période d'inactivité précédant la reprise du sport constituait un facteur commun à l'ensemble des malades. Un entraînement préalable peut améliorer l'adaptation musculaire, ce qui peut diminuer le risque de rupture du tendon d'Achille.
    Notes: Summary Biopsy of the triceps surae muscle was carried out in 11 patients who had sustained a spontaneous rupture of the Achilles tendon during sports. All the specimens showed important histological and ultrastructural changes, the most striking being the presence of large quantities of lipid droplets within the mucle fibres. A factor common to every patient was a period of inactivity before resuming sports. Preliminary training may be valuable in producing positive adaptive changes in the muscles, and the consequent improvement in function may reduce the risk of rupture of the Achilles tendon.

    Notes: Summary We report the post-mortem findings of the case of a 31-year-old male who, through sexual contacts with a female drug addict, was found to be HIV-positive and developed the acquired immunodeficiency syndrome (AIDS) 2 years later. He was treated for various opportunistic infections over the next 7 years when he presented with cardiac abnormalities and multiple cerebral lesions which were responsible for his death. The results revealedAspergillus fumigatus endocarditis and myocarditis with mycotic thromboembolic extension to the brain, spleen, kidney and pancreas. We review the literature ofAspergillus infection in patients with AIDS and more specifically cardiac involvement with this pathogen.
