Abstract In a complex genital prolapse there are usually at least two of three pelvic floor compartments involved – this being through discomfort, functional disorders, or organ pathology. How these factors affect the individual patient will regulate indication for surgery. What is important is the separate examination of each compartment in order to determine the effect of a dominant herniation on the neighboring area (e.g., masking of stress urinary incontinence). Where proctological problems are also the case, working together with an experienced surgeon is helpful. Increased age and lack of enthusiasm in maintaining sexual relations are no argument against preserving the function of the vagina. Pessaries are a sensible alternative where contraindications to surgery or the wish of the patient is involved. Cystoceles due to a central fascia defect can be corrected with an anterior colporrhaphy; with a lateral defect, the so-called “lateral repair” would be the operation of choice. The sacrospinous fixation and abdominal sacrocolpopexy/uteropexy are tried and tested methods of repairing a prolapsed vaginal cuff or uterus, although no long-term data are available for endoscopic intervention. The prolapsed posterior compartment is treated mainly by posterior colporrhaphy with colpoperineoplasty. However, as yet, there are hardly any prospective long-term results. For deep rectoceles, the surgical alternative is transanal correction. A more defect-related procedure has been described by Richardson.

Zusammenfassung Der Leidensdruck der Patientin bestimmt die Indikation zur Operation. Wichtig ist die getrennte Untersuchung aller 3 Kompartiments, um die Bedeutung eines dominierenden Bruchsackes für das benachbarte Kompartiment (z. B. Larvierung einer Stressharninkontinenz) zu erkennen. Die Zusammenarbeit mit einem erfahrenen Chirurgen ist bei proktologischen Beschwerden hilfreich. Höheres Alter und fehlender Wunsch nach Erhalt der Kohabitationsfähigkeit sind kein Argument gegen funktionserhaltende Operationsverfahren. Pessare sind bei Kontraindikation für eine Operation oder Wunsch der Patientin eine sinnvolle Alternative. Zystozelen aufgrund eines zentralen Fasziendefekts werden zumeist durch die Kolporrhaphia anterior behoben, beim Lateraldefekt bietet sich sog. “lateral repair” an. Die sakrospinale Fixation und abdominale Uteropexie bzw. Sakrokolpopexie sind zur Fixierung des prolabierenden Uterus bzw. Scheidenstumpfs etablierte und nahezu gleichwertige Verfahren. Das prolabierte hintere Kompartiment wird hierzulande zumeist durch die Kolporrhaphia posterior mit Kolpoperineoplastik behandelt. Es gibt jedoch kaum prospektiv erhobene Langzeitresultate. Ein mehr defektbezogener Ansatz wurde von Richardson beschrieben.
