    Notes: Abstract Purpose: Oblique radiographs of the chest were added to our skeletal survey for nonaccidental trauma to determine if these views improved our ability to diagnose rib fractures accurately. Methods: Seventy-three patients were evaluated with skeletal surveys for suspected child abuse. The chest films were reviewed independently by three radiologists as two separate sets, one with the frontal and lateral views only and one including the two oblique views. At the end of the study, consensus readings of all two- and four-view sets were reached and used as the true interpretations. Results: In 14 patients, positive rib fractures were identified by consensus. The accuracy, sensitivity, and specificity for each radiologist were greater with the four-view sets than with the two-view sets. In some children, rib fractures considered equivocal by two-view evaluation were called confidently as positive or negative with the additional views. Conclusion: The addition of oblique radiographs improves rib fracture evaluation in children suspected of having suffered nonaccidental trauma, increasing sensitivity, specificity, and accuracy as well as the radiologists' confidence.

    Notes: Abstract Congenital heart failure in the neonate supported by classic imaging findings may allow the implementation of medical therapy for presumed hemangioendothelioma without obtaining a tissue diagnosis. This case report describes a neonate with these classic clinical and radiographic findings but who underwent surgery for failing medical treatment and was diagnosed as having a hepatoblastoma by pathology. This case supports the need to obtain tissue confirmation before beginning medical therapy.

    Notes: Abstract Aim/hypothesis. Numerous studies have suggested a relation between sex hormones and insulin sensitivity but the ability of sex hormones to directly influence insulin action in peripheral tissues has not been investigated.¶Methods. We have examined the effects of estriol, estradiol and estrone on insulin action in cultured 3T3-L1 adipocytes, a useful model of adipocytes.¶Results. Treatment of these cells with each of these sex hormones resulted in a statistically significant reduction in the ability of insulin to stimulate glucose transport independently of a reduction in total cellular GLUT-4 content. This diminished ability of insulin to stimulate glucose transport was accompanied by a reduction in the total cellular content of insulin receptor substrates –1 and –2 and the p85α subunit of phosphatidylinositol 3'-kinase. By contrast, cellular content of protein kinase B was unchanged by hormone treatment but the magnitude of insulin-stimulated kinase activity was statistically significantly reduced after incubation with each of the sex hormones tested. We have further shown that treatment of 3T3-L1 adipocytes with these hormones alters the subcellular distribution of insulin receptor substrate proteins such that the particulate and soluble pools of these proteins were differentially affected by hormone treatment.¶Conclusion/interpretation. These data show that sex hormones can directly induce a state of insulin resistance in 3T3-L1 adipocytes in culture. The mechanism of this defect seems to be at least in part due to decreased cellular content and altered subcellular distribution of insulin receptor substrate proteins which in turn results in a reduction in proximal insulin-stimulated signalling cascades.     Notes: Abstract  An understanding of the molecular mechanisms that are responsible for increased oleic acid accumulation would open avenues to alter peanut fatty acid composition and allow detection of polymorphic regions which can be used for marker assisted selection (MAS). Δ12-Fatty acid desaturase (FAD) was isolated and characterized from genotypes having a low or high oleic to linoleic acid O/L ratio – genotypes, Tamspan 90 (T-90) and F435–2-2 (F435), respectively. Southern blots showed three to four copies per haploid genome, and no major differences in organization between the two parental lines. Approximately 3525 bp was isolated from both genotypes, including a genomic walk toward the promoter region. The Δ12-Fad contains a putative intron, the coding region at the 3′ end, and an open reading frame (ORF) of 1140 bp encoding 379 amino acids. Comparisons of the coding sequences from the high and low oleic acid genotypes revealed several single nucleotide polymorphisms (SNPs). Two polymorphisms appear to be associated with the high O/L trait. The first is an ”A” insertion 442 bp after the start codon. The ”A” insertion shifts the amino acid reading frame, probably resulting in a truncated, inactive protein and the loss of one of three histidine boxes believed to be involved in metal ion complexation required for the reduction of oxygen. Another polymorphism at 448 bp from the start codon results in an amino acid change. The region containing the polymorphisms was amplified from leaf tissue of several independently derived backcross lines (IDBLs). Most high O/L lines had either the ”A” insertion or the amino acid substitution.
