    Notes: A cylindrical arrangement of electrodes, together with control electronics, is described that provides uniform electric fields over cm3 volumes in a vacuum, whose angle and magnitude can be switched on a μs time scale. Full 360° field rotation is achieved, and the device allows access for particle or light beams from all sides. Both numerical and analytic descriptions of the general fields are given. Extension to full three-dimensional field control and other variations are described. The device, which was originally designed for crossed-beam collision experiments with keV energy ions and laser-excited Rydberg atoms in field-directed Stark states or coherent-elliptic states, may be more generally useful in atom trapping and cold collisions or in materials and surface science. © 1998 American Institute of Physics.

    Notes: We evaluated the long-term results of three total hip and nine total knee arthroplasties in nine haemophilic patients with disabling pain and end-stage arthropathy. These patients have been followed over a period of 2–12 years (mean 6.9). One patient had a post-operative haematoma, which was evacuated; two patients required manipulation of the knee because of a limited range of motion. There were no infections. At follow-up, all but two patients were completely free of pain; two patients had occasional pain. The functional improvement was impressive with an average increase in the knee score from 37 to 80 points post-operatively. The hip score increased from 36 to 85 points. The range of motion was increased in seven joints, unchanged in two joints and decreased in three. One total hip arthroplasty was revised after 9 years for aseptic loosening. One total knee demonstrated nonprogressive radiographic evidence of aseptic loosening. Median consumption of coagulation factor concentrate in severe haemophiliacs decreased from 47.00 U yr−1 (range 16.000–144.000) before surgery, to 25.000 U yr−1 (range 18.000–132.000) at 2 years after surgery. Conclusion: hip and knee arthroplasty is a valuable option in symptomatic haemophilic patients with disabling arthropathy, in order to obtain pain relief and functional improvement. It is associated with a low rate of complications and may reduce the need for substitution of factor VIII and IX.

    Notes: We report the case of a patient with a haeophilic pseudotumour of the ilium who developed chronic fistulation, 6 months after en-bloc resection. During the initial resection, the large defect in the iliac wing was filled with bone cement, which may have contributed to fistula formation. A second surgical procedure consisted of excision of the fistula and bone cement and the dead space was obliterated by bringing the gluteus medius muscle into the defect. The fistula recurred, however. Re-excision of the fistula and obliteration of the dead space by a pedicled rectus abdominis muscle flap resulted in eradication of the fistula. We emphasize the importance of obliteration of dead space, resulting from large pseudotumour resection. The use of bone cement is not advocated. If a fistula does occur, a pedicled rectus abdominis muscle flap may be considered.

    Notes: Abstract Twelve patients are reported in whom a unicameral bone cyst was found to involve the epiphysis or apophysis across an open growth plate. Nine patients showed pathologic fracture. In four patients a growth disturbance was found. A varying amount of varus angulation was present in most cases. No patient had functional impairment.

    Description / Table of Contents: Résumé Cinquante-six prothèses d'humérus ont été mises en place de 1974 à 1984 pour tumeurs primitives de l'épaule. Dans 40 cas on a pratiqué une résection de la partie proximale de humérus et dans 16 une intervention de Tikhoff-Linberg. La longueur de l'humérus réséqué a varié entre 9 et 24 cm. Les auteurs analysent en détail les résultats oncologiques et fonctionnels. La résection a été large dans 42 cas (75%), marginale dans 12 (21,5%) et intra-lésionnelle dans 2 (3,5%). Il y a eu 8 récidives locales. Les résultats fonctionnels ont été satisfaisants, les malades avaient un bon degré de rotation active et étaient capables d'effectuer un travail sédentaire. Présentation d'une nouvelle prothèse modulaire d'humérus utilisée depuis 1984.
    Notes: Summary Fifty six humeral prostheses were inserted from 1974 to 1984 for primary bone tumours of the shoulder. In 40 patients a proximal humeral resection was performed and in the remaining 16 cases a Tikhoff-Linberg procedure was employed. The length of humeral resection ranged from 9 cm to 24 cm. A detailed analysis of the oncological and functional results was made. The resection was well clear in 42 patients (75%), marginal in 12 (21.5%), and through tumour in 2 (3.5%). Eight local recurrences were noted. The functional results were satisfactory, the patients having a wide range of active rotation and being able to do sedentary work. A new modified humeral modular prosthesis was introduced in 1984 and is presented.

    Notes: Abstract Although the structural changes occurring in the scoliotic spine have been reported as early as the 19th century, the descriptions and biomechanical explanations have not always been complete and consistent. In this study, three-dimensionally rendered CT images of two human skeletons with a scoliotic deformity and two patients with serious scoliosis were used to describe the intrinsic vertebral and rib deformities. The pattern of structural deformities was found to be consistent. Apart from the wedge deformation of the apical vertebrae, a rotation deformity was found in the transversal plane between the vertebral body and the posterior complex: the vertebral body was maximally rotated towards the convexity of the scoliotic curve, whereas the tip of the spinous process was pointed to posterior. The rib deformities at the convex side of the scoliotic curve showed an increased angulation of the rib at the posterior angle, whereas the rib curve on the concave side was flattened. The observed vertebral deformities suggest that these are caused by bone remodelling processes due to forces in the anterior spinal column, which drive the apical vertebral body out of the midline, whereas forces of the musculo-ligamentous structures at the posterior side of the spinal column attempt to minimize the deviations and rotations of the vertebrae. The demonstrated rib deformities suggest an adaptation to forces imposed by the scoliotic spine.
