    Notes: Previous studies have shown that type I diabetes (IDDM) increases the risk of developing periodontitis by 2-3-fold. IDDM patients exhibit destruction of the pancreatic beta cells, most probably caused by an autoimmune reaction. Evidence is accumulating to support the role of the autoimmune response in periodontal pathogenesis. A cytokine, interleukin (IL)-10, has been reported to selectively promote the expansion of a B lymphocyte lineage (CD5/LY1/B1) which has the propensity for secreting high levels of autoantibody. Therefore, the purpose of this project was to evaluate IL-10 production, percentage of CDS B cells and the frequency of anti-collagen secreting cells in peripheral blood mononuclear cells of age, gender and race matched IDDM patients and controls. IL-10 production was evaluated by an ELISA using the supernatant of adherent peripheral blood cells cultured for 24 h in the presence of Porphyromonas gingivalis lipopolysaccharide (LPS). In 8 of 31 patients, IL-10 levels were significantly increased in IDDM compared to controls and a higher percentage of CD5 B cells was also observed by flow cytometry. In addition, these patients exhibited a higher frequency of anti-collagen secreting cells as elucidated by an ELISPOT. Moreover, treatment with a neutralizing anti-IL-10 antibody diminished the anti-collagen antibody response by 70%. These findings support the concept that a subset of IDDM patients possess an extremely robust IL-10 response following exposure to Gram-negative LPS, which could predispose them to the development of periodontitis through a heightened autoimmune mechanism.

    Notes: Abstract. The objective of this research was to determine the effectiveness of a biochemical assay which measures proteolytic enzyme activity in gingival crevicular fluid (GCF) and to relate this enzyme activity to clinical parameters traditionally utilized for periodontitis detection. A clinical trial was conducted on 8 periodontitis subjects with ≥4 sites exhibiting a loss of attachment of ≥5 mm and probing depths of ≥5 mm with bleeding on probing. On each subject, a plaque index was performed, followed by GCF sampling at those sites which exhibited a loss of attachment and probing depths. GCF was analyzed for activity against benzoyl-L-arginine-p-nitroanilide in the presence (BAPNA w/gly-gly) and the absence (BAPNA w/o gly-gly) of glycyl-glycine and against MeOSuc-Ala-Ala-Pro-Val-pNA and Suc-Ala-Ala-Pro-Phe-pNA for neutrophil serine proteinases activity (elastase and cathepstn G, respectively). Subsequently, a gingival index was performed, attachment levels and probing depths were recorded using a constant force probe with bleeding on probing being noted. A split-mouth design was employed and half mouths were randomly assigned to the following treatment groups: group A, half of the mouth received scaling/root planing and polishing: group B, half of the mouth received no treatment (control). Subjects were treated, then instructed on toothbrushing and interdental cleaning. After 4 weeks, subjects returned to receive a plaque index; GCF sampling, gingival index, attachment levels, probing depths and bleeding on probing as described above. Using a patred Student t-test, the findings suggest that BAPNA w/gly-gly was significantly less in treatment sites than in non-treated control sites (p=0.05). No such correlation was found for other activities, including neutrophil serine proteinases which were shown to occur in GCF in free, proteolytically active forms. In addition, significant treatment effects were detected for probing depths (p= 0.03) which reduced by 1.3 mm and attachment levels (p=0.02) which gained 0.7 mm. The reduction of P. gingivalis from treated periodontitis sites as detected by a significant decrease in BAPNA w/gly-gly may prove to be a valuable marker for periodontal disease activity.

    Notes: Abstract Hemoperitoneum in the newborn is an uncommon event, occurring most often in the setting of traumatic delivery. Hemoperitoneum resulting from antenatal hemorrhage into an abdominal mass is rare. We present a case of neonatal hemoperitoneum secondary to antenatal hemorrhage into a retroperitoneal lymphangioma. The differential diagnosis of neonatal hemoperitoneum is discussed.

    Notes: We measured the ventilatory pattern and additional work of breathing (WOBadd) at three different levels of inspiratory pressure support [IPS 5, 10, 15 mbar above positive end-expiratory pressure (PEEP)] and in a new ventilatory mode, automatic tube compensation (ATC), in nine operative patients without lung injury nine patients ventilated for several following acute respiratory insufficiency (ARI). In ATC, endotracheal tube resistance is compensated automatically by means of closed-loop control of the calculated tracheal pressure. Pressure support in this mode, i. e. airway pressure above PEEP, is equal to the actual flow-dependent pressure drop across the endotracheal tube (ETT). Airway pressure rises at the beginning of inspiration and falls towards the end. As the tube resistance of ETT seriously hinders expiration and can cause desynchronization between ventilator and patient, airway pressure is reduced below PEEP during expiration in the same way as it is increased during inspiration. The result is a near-constant tracheal pressure at PEEP both during inspiration and during expiration. This mode could be best termed as ``electronic extubation''. The most striking difference between the postoperative patients and the ARI patients was their minute ventilation (17.8±1.85 l/min in ARI patients vs 7.3±3.1 l/min in the postoperative patients). In the postoperative patients augmentation of IPS from 5 to 15 mbar induced a steady increase in tidal volume (VT) and a consecutive decrease in respiratory rate (rr) compared with ATC (VTATC,postop= 463±78 ml; rrATC,postop =16±4 min–1; VTIPS5,postop = 505±79 ml; rrIPS5,postop = 15±4 min–1; VTIPS10,postop = 562±86 ml; rrIPS15,postop =14±4 min–1; VTIPS15,postop=660± 151 ml; rrTPS15, postop = 12±4 min–1), whereas the augmentation of IPS of 5 and 10 mbar in the ARI patients could not compensate for the increase in rr and the decrease in VT, after switching from ATC to IPS (VTATC,ARI 724±308 ml, rrATC,ARI = 24±6 min–1; VTIPS5,ARI = 649±315 ml; rrIPS 5,ARI = 27±8 min–1; VTIPS10,ARI=653±353 ml; rrIPS10,ARI = 25±8 min–1: Even IPS 15 was not able to reestablish VT at the values observed during ATC (VTIPS15,ARI =680±312 ml). During ATC WOBadd was small in both postoperative and ARI patients (WOBadd,ATC,postop = 93±36 mJ/l, WOBadd,ATC,ARI =116 ±72 mJ/l). In the postoperative patients, an inspiratory pressure support of 5 mbar was not sufficient to compensate WOBadd compared with ATC. However, IPS 10 and 15 mbar were able to compensate for WOBadd (WOBadd.ATC5,postop WOBadd,IPS5,postop =189±77 mJ/l; WOBadd,IPS10,postop = 55±30 mJ/l; WOBadd,IPS15,postop = 21±11 mJ/l). In the ARI patients an IPS 5, 10 or 15 mbar was not sufficient to compensate for WOBadd (WOBadd,IPS 5,ARI = 1126±262 mJ/l; WOBadd,IPS 10,ARI 863±253 mJ/l; WOBadd,IPS 15,ARI 763±298 mJ/l). Under ATC, WOBadd was only 15% of WOBadd under IPS of 15 mbar. All but two patients were successfully extubated after the investigation. These two patients were not extubated because they were dependent on an FIO2 〉0.5. Our results strongly indicate that ventilatory dependence in ARI patients may be caused by the ETT rather than by mechanical dysfunction of the lung. ATC is a very helpful mode to use in distinguishing between ventilatory failure caused by ETT and real ventilatory dependence.

    Notes: Abstract Approximately 300,000 patients each year in the United States undergo laminectomy for disabling lumbar disc herniation. Post-laminectomy hospitalization is 3–7 days and convalescence may be prolonged. As an alternative to laminectomy, we have developed a technique for performing L5-S1 lumbar diskectomy laparoscopically. Using an anterior approach, the intervertebral disc space is opened and the diskectomy is performed under direct videolaparoscopic imaging. After pneumoperitoneum is established, the patient is placed in a steep Trendelenburg position. The small bowel is retracted cephalad and the colon is moved to the left. The iliac vessels are identified visually and by Doppler probe, and the presacral space is dissected in the midline to expose the L5-S1 disc. In the case presented, the disc annulus was opened with the Nd:YAG contact laser, and diskectomy was performed under direct videolaparoscopic vision using standard neuro-surgical instruments modified for laparoscopy. The posterior longitudinal ligament can be visualized directly to define the posterior limits of the completed diskectomy. In the case described, pain relief was confirmed immediately after the procedure. The patient was discharged after 2 hospital days, and returned to normal activity in 8 days.

    Notes: Trianthrylene compounds with film-forming capabilities were synthesized in a three-step procedure by means of transition-metal catalyzed Grignard and Suzuki coupling reactions. Upon UV exposure, photopolymerization of the model compound 1,11-di-(9-anthryl)undecane was observed in solution as well in the film due to [4+4] cycloaddition of the anthracene moieties. The soluble photopolymers were characterized by NMR spectroscopy and GPC analysis. As expected, irradiation of solutions of the trichromophoric hydrocarbons afforded crosslinked photoproducts which precipitated from solution. In the case of an analogous trianthryl derivative with tetraoxyethylene spacers, formation of a gel was observed due to cycloaddition of the anthracene groups. The film-forming capabilities of trianthryl compounds in conjunction with UV-induced photocrosslinking can be useful for photoresist applications and will be discussed in the following paper [1].
