    Description / Table of Contents: Summary Fibromyalgia is present in 2% of the general population and leads to impairment by chronic pain and fatigue. It does not improve without therapy directed at the symptoms of fibromyalgia. We describe our interdisciplinary group treatment for patients with fibromyalgia. They received a physical examination, ergometry and psychometric tests both at admission and before discharge, and they were questioned to the degree and localization of their pain, to fatigue, sleeping disorders and functional symptoms. Therapy included information about fibromyalgia, learning of coping strategies, relaxation and endurance training. Our results show that our interdisciplinary group treatment is effective for fibromyalgia and improves anxiety, depression and well being after a period of 5 weeks of in-patient rehabilitation.
    Notes: Zusammenfassung Die Fibromyalgie betrifft 2% der Bevölkerung und führt bei den Betroffenen zu einer deutlichen Beeinträchtigung durch chronische Schmerzen und Müdigkeit. Ohne gezielte Therapie bessern sich die Symptome der Fibromyalgie nicht. Wir behandelten Fibromyalgiepatienten interdisziplinär in Gruppen. Dabei wurden die Patienten bei Aufnahme und vor Entlassung ärztlich, leistungsphysiologisch und psychometrisch untersucht und zur Ausprägung und Lokalisation ihrer Schmerzen, zu ihrer Müdigkeit, Schlafstörungen und funktionellen Symptomen befragt. Die Behandlung schloß Informationen zum Krankheitsbild, Erlernen von Bewältigungsstrategien, Entspannungstechniken und Ausdauertraining ein. Unsere Untersuchungen zeigen, daß die ärztlich und psychologisch geleitete interdisziplinäre Gruppentherapie in der Behandlung der Fibromyalgie effektiv ist und am Ende der 5-wöchigen Behandlungsdauer Angst, Depression und Befindlichkeit der Fibromyalgiepatienten bessert.

    Description / Table of Contents: Zusammenfassung Bei der Fibromyalgie beobachtet man in gleicher Weise wie bei unspezifischen chronischen Lumbalgien klinisch und röntgenologisch sehr häufig Haltungsstörungen der Wirbelsäule, insbesondere Skoliosen. Darüber hinaus lassen sich ebenfalls bei beiden Erkrankungen in einem hohen Prozentsatz segmentale oder ausgedehnte Funktionsstörungen des Achsenorgans nachweisen. Vermutlich sind die Wirbelsäulenveränderungen oft für die Erstmanifestation der Fibromyalgie verantwortlich, die sich ja meist monolokulär vor allem in der Lumbal- und Cervicalregion findet. Die spätere Ausbreitung der Schmerzen erfolgt wahrscheinlich auf neuralen und muskulären Wegen, wobei psychische Faktoren akzentuierend wirken dürften.
    Notes: Summary In fibromyalgia as well as in low back pain we frequently find disturbances of the posture of vertebral column clinically and radiologically. Also, reduction in the mobility of whole spine and localized movement impairments in both conditions was present. It is likely that the disturbances are responsible for the first manifestations of fibromyalgia in a single localization, especially in lumbar and cervical regions.

    Notes: Abstract   Introduction: Inhaled nitric oxide (NO) may be beneficial in the treatment of pulmonary hypertension, both of the newborn and in the adult respiratory distress syndrome. Up to now, serious systemic side effects have not been reported. Objective: The effect of inhaled NO on superoxide anion production by neutrophils. Design: Prospective study of a consecutive series of 15 neonates and infants. Setting: Neonatal and paediatric ICUs with a total of 17 beds (university hospital). Measurements and results: Superoxide anion production was determined by a flow cytometric method using dihydrorhodamine 123 (DHR) as an oxidative probe after the priming of neutrophils with N-formyl-methionyl-leucyl-phenylalanine (fMLP) or with Escherichia coli. The generated fluorescence was expressed as relative fluorescence intensity (RFI). Inhalation of NO for more than 24 h reduced the superoxide anion production by neutrophils stimulated with E. coli to below baseline values before NO inhalation (mRFI=158±25 vs 222±24; P=0.03). This decrease was more pronounced after more than 72 h (mRFI=133±17). At this time, superoxide anion production by fMLP-stimulated neutrophils was also decreased (mRFI=40±3, vs 57±5; P=0.03). The reduced capacity of superoxide production persisted throughout therapy with NO and lasted up to more than 4 days after the end of NO inhalation. Conclusion: The results suggest that inhalation of NO in patients with pulmonary hypertension causes reduced superoxide anion production by neutrophils stimulated with E. coli or with fMLP. To determine the clinical importance of this systemic side effect with respect to bacterial infections, a randomized controlled study is necessary.
