    Notes: Abstract An important factor in determining species rarity is the geographic distribution of species. Estimates were made of the level of endemism of the flora of two sites in the southern Bahian wet forest zone. Estimates were made for endemism in the Atlantic forest biome and for the much more restricted area of southern Bahia and northern Espi´rito Santo and are derived from analyses of the distributions of the species known from each area. The species checklist for each area is based on identified specimens resulting from intensive collecting in a forest near Serra Grande (40km north of Ilhe´us) and the Una Biological Reserve (40km south of Ilhe´us). Slightly less than half of the species (45.2% at Una and 47.7% at Serra Grande) have widespread distributions and 7.4% at each site are disjunct between the coastal forests and Amazonia. In the Una Reserve, 44.1% of the species are endemic to the coastal forest and 28.1% endemic to southern Bahia and northern Espi´rito Santo. At Serra Grande, 41.6% of the species are endemic to the coastal forest and 26.5% endemic to southern Bahia and northern Espi´rito Santo.

    Notes: Die endotracheale Intubation zählt zu den etablierten Methoden zur Sicherung der Atemwege von Notfallpatienten im präklinischen und klinischen Bereich [6]. Im Rettungsdienst erfolgt die Intubation nicht nur bei manifester respiratorischer Insuffizienz, sondern bei einer Vielzahl von Krankheitsbildern. Sie gewährleistet sicheren Schutz vor Aspiration, erleichtert die Korrektur einer vorbestehenden Hypoxie oder Hyperkapnie wie auch das Absaugen des Tracheobronchialsystems und ermöglicht die Applikation kardiovaskulär wirksamer Medikamente. Intubationsschwierigkeiten im Rahmen der präklinischen Notfallmedizin treten in aller Regel unvermittelt auf und können sich rasch zu einer akuten vitalen Bedrohung des Patienten entwickeln, die eine zerebrale Schädigung oder den Tod des Patienten zur Folge hat. Von einer schwierigen Intubation spricht man, wenn die direkte Laryngoskopie, die Plazierung oder das Einführen des Endotrachealtubus durch die Stimmritze problematisch oder unmöglich ist [8]. Der Algorithmus der endotrachealen Intubation sowie Alternativen zur Sicherung der Atemwege müssen allen im Rettungsdienst tätigen Personen vertraut sein (Abb. 1). Hierzu zählen die Oxygenierung des Patienten über eine Gesichtsmaske, die Anwendung der Larynxmaske [4, 7] und des Kombitubus © [12]. Zu den invasiven Zugangsmöglichkeiten zur Sicherung der Atemwege zählen die Koniotomie und Tracheotomie [11].

    Notes: Abstract Aim of the study. To evaluate in a prospective trial three courses of an ABVD-like chemotherapy (CT) regimen given before radiation therapy (RT) (subtotal nodal irradiation (STNI)) in favorable stage Hodgkin’s disease (HD). The efficacy, risk factors and medium-term toxicities are reported. Patient characteristics. Stage I or II with at least one of the following factors, mediastinal involvement, histological type 3, age 〉 40 years, ESR rate 〉 50 mm, or stage IIIA. 189 patients with newly diagnosed HD were treated between 01/86 and 01/94 (stage I, n = 33; stage II, n = 129, stage IIIA, n = 27). Three courses of an ABVD-like regimen (adriamycin 25 mg/m2, bleomycin 10 mg, vindesine 2 mg/m2 and dacarbazine 250 mg/m2 day 1 and 8) were given before STNI at 36/40 grays. At diagnosis, prognostic factors were distributed as follows: B symptoms (n = 54), bulky mediastinum (n = 41), hemoglobin 〈 12 g/dl (n = 37), ESR 〉 50 (n = 65), age 〉 45 (n = 24). Results. After chemotherapy, 90% had an objective response (partial response 〉 75%) and 98% were in complete remission (CR) at the end of RT. Three patients had primary refractory disease and 13 patients (7%) relapsed, 3 at the initial site, 4 at previously uninvolved sites and 6 at both. With a median follow-up of 60 months, 170 patients are in 1st CR, 5 in 2nd or greater CR and 11/14 patients have died from HD. Bulky mediastinum (p = 0.009), age 〉 45 years (p = 0.03) and ESR 〉50 mm (p = 0.05) were adverse prognostic factors for survival. Bulky mediastinum (p = 0.009) was the only prognostic factor for freedom from progression. Toxicities. Two patients died from treatment related toxicity and one patient died with an osteogenic sarcoma. No secondary leukemia has so far been detected. 24 pregnancies were reported. Cardiopulmonary toxicity was always 〈 grade 1 (WHO) in 95 patients evaluated. Two patients over 45 years old had a myocardial infarction. Conclusion. With an acceptable medium-term toxicity, this treatment achieved 85% survival at 5 years.

    Notes: Abstract Background: Despite high-dose therapy and ASCT some patients with aggressive HD fail to achieve long-term survival. Patients and methods: Forty-three patients with induction failure (n = 19) or very unfavorable (UF) relapse (n = 24) from HD were included in a multicentric study of tandem ASCT. They planned to receive two courses of IVA75 with GCSF and blood stem-cell collection. ASCT1 was conditionned with CBV + mitoxantrone (30 mg/m2) and ASCT2 (cytarabine 6 g/m2, melphalan 140 mg/m2 and total body irradiation at 12 Gy or busulfan 16 (n = 4) than 12 mg/kg). After salvage therapy, response 〉50% was observed in 63% of the patients (six patients were included for refractory relapse). Four patients had no ASCT for disease progression; seven patients had only ASCT1 (disease progression, n = 3) and thirty-two patients (74%) received the two ASCT. Results: Hematologic recovery was normal after ASCT1 but delayed platelet recovery was observed after ASCT2 with busulfan in the conditioning regimen. Two VOD with one fatal occured with busulfan at 16 mg/kg and one hemorragic cystis, no further grade 4 toxicity was observed with the reduced doses of busulfan (12 mg/kg). After ASCT2, 83% of these UF patients were in remission and 20% relapsed within the first year. On an intent-to-treat analysis, 22 of 43 patients are in continuous CR (including 8 patients with induction failure). For the whole population (n = 43) and for patients receiving the two ASCT (n = 32), the two-year survival from the date of progression were respectively at 65% and at 74%. Conclusion: double ASCT is feasible in very UF relapse from HD and may lead to some prolonged remission.
