    Notes: Aim - To determine whether house dust mite (HDM) exposure in living rooms and bedrooms is higher in asthmatic children than in those of age and sex matched healthy children, living in the same area. Methods - Types of floor-coverings were recorded and dust samples were collected by vacuum cleaning the total area of living rooms and bedrooms; Der p I and Der p II per gram fine dust concentrations were assessed. Twenty-five asthmatic children (RAST HDM 〉= class 3, age 6–12 years) and 25 healthy children participated in the study. Results - The frequency of cleaning and prevalence of smooth floor-coverings in bedrooms of asthmatic children were significantly higher. There were no differences in living rooms in this respect. The amount of fine dust/m2 floor space was significantly lower in bedrooms of asthmatic children. Concentrations of HDM were low and no differences in Der p I and Der p II concentrations were observed between the two groups (asthmatic children: Der p I living room: 1.1 (0.04 - 59.4 μg/ g), bedroom: 0.5 (below detection - 19.3 μg/g); nonasthmatic children: Der p I living room: 1.4 (below detection - 27.5 μg/g), bedroom: 0.9 (below detection - 68.8 ug/g. Smooth floor coverings contained significantly less fine dust, Der p I, and Der p II than carpeted floors. Conclusions - Low HDM concentrations are a general finding in Dutch dwellings in the present generation of children.We observed a higher cleaning frequency, and more smooth floor coverings in bedrooms of asthmatic children than of healthy children, yet HDM concentrations were not significantly different. The latter can be explained by the observation that only 40% of the asthmatic children had smooth floor coverings in their bedrooms. Smooth floor coverings contain less fine dust and lower concentrations of Der p I and Der p II than carpeted floors.

    Notes: Background The degree of airway hyperresponsiveness in allergic asthmatic patients may be influenced by changes in environmental exposure to inhalant allergens.Objective This study investigates the relationship between seasonal changes in exposure to house dust mite (HDM) allergens and non-specific airway hyperresponsiveness in asthmatic patients with multiple sensitizations to inhaled allergens.Methods In 43 asthmatic patients sensitized to several inhalant allergens, lung function (FEV1), airway hyperresponsiveness (PC20 histamine), serum total IgE, house dust mite (HDM) specific IgE and number of peripheral blood eosinophils were measured during autumn 1990 (September-November) and spring 1991 (March—May). During each season. floor dust samples were collected twice from living- and bedrooms and the concentration of the HDM allergens Der p 1 and Der p 2 determined.Results More severe airway hyperresponsiveness (lower PC20 histamine) during autumn was only found in patients sensitized to HDM(n= 32; autumn: 2.05mg/mL, spring: 4.51mg/mL (geometric means), P 〈0.0 1), whereas in patients not sensitized to HDM (n= 11) similar values were observed in both seasons (3.44 and 4.52 mg/mL. respectively, P= 0.56). More severe airway hyperresponsiveness of HDM sensitized patients in autumn was significantly associated with higher Der p 1 concentrations in floor dust. Aside from airway hyperresponsiveness, seasonal changes in serum total IgE and number of peripheral blood eosinophils were seen in patients sensitized to HDM,Conclusions In allergic asthmatic patients, airway hyperresponsiveness may increase during autumn, depending on sensitization to HDM and an increase of exposure to HDM allergen.

    Notes: The prevalence of sensitization to the storage mites Acarus siro (AS), Tyrophagus putrescentiae (TP), and Lepidoglyphus destructor (LD) was studied in 250 sera of patients with different degrees of sensitization to the house-dust mite Dermatophagoides pteronyssinus (DP) by measuring IgE binding to extracts of the storage mites. Additionally, allergenic cross-reactivity between DP and the storage mite species was studied by RAST inhibition with five individual sera (and a pool of these sera) with moderate IgE levels to all three storage mites and to DP. Increased serum IgE to storage mites was found in 46% of the 200 patients sensitized to DP. Increased prevalence rates of IgE titers to storage mites were associated with higher IgE levels to DP In 50 sera without sensitization to DP, only five sera showed increased IgE to one of the storage mites. Extracts of TP almost completely inhibited the IgE binding to AS, and vice versa. DP inhibited IgE binding to all storage mites up to 60%, whereas IgE binding to DP was only minimally inhibited by extracts of storage mites. In conclusion, cosensitization to storage mites is a frequent finding in patients sensitized to DP. Although this is largely the result of cross-reactivity between different mite species, it may nevertheless be of clinical significance in patients exposed to storage mites.

    Notes: This double-blind, placebo-controlled study investigated whether the application of an acaricide (Acarosan®) on mattresses and on textile floor coverings in living rooms and bedrooms can contribute to improvement in lung function and airway hyperresponsiveness in 40 adult asthmatic patients sensitized to house-dust mite. In a second group of 19 patients who refused chemical intervention, the clinical effects of application of allergen-impermeable mattress encasings were studied. In all three treatment groups, Der p 1 levels in mattress dust were statistically significantly decreased after 12 months. However, this decrease was much greater in the group who received mattress encasings (final mean level 430 ng/g) than in groups with acaricide- or placebo-treated mattresses (final mean levels 1730 and 2100 ng/g, respectively). Treatment of textile floors with either Acarosan or placebo chemical caused a statistically significant decrease in the level of the house-dust-mite allergen Der p 1 in floor dust. In the group with mattress encasings, no significant changes of floor dust Der p 1 were found. Airway hyperresponsiveness (as measured by the PC20 histamine) improved significantly in the mattress cover group after 6 months. The Acarosan group also showed a small but statistically significant improvement after 12 months.

    Notes: Exposure to house-dust-mite allergens is an important cause of allergic reactions in sensitized patients. In community-based studies, sensitization to house-dust mites, as ascertained by a positive skin test or by an increased allergen-specific IgE level in serum, is associated with both diminished lung function and enhanced airway hyperresponsiveness. Sensitization to house-dust mites is an independent risk factor for the development of asthma, especially in areas which favor the growth of house-dust mites. In studies with asthmatic patients, however, the relationship between sensitization to mites and lung function or airway hyperresponsiveness is less clear. The selection of asthmatic individuals (with sensitization to other inhalant allergens) and the use of medication such as inhaled steroids may interfere with the demonstration of a possible association between mite allergy and lung function or airway hyperresponsiveness in asthmatic patients.

    Description / Table of Contents: Zusammenfassung Ein 22-jähriger Fußballspieler wurde gefoult und erlitt eine Thoraxprellung. Kurz nach dem Spielende wurde ihm plötzlich übel. Etwa 2 h später verstarb er. Die Obduktion erbrachte neben geringgradigen Zeichen stumpfer Gewalteinwirkung eine schwere Sklerose und Thrombose des Ramus interventricularis der A. coronaria sinistra. Die postmortale Lipoproteinbestimmung ergab einen nahezu dreifach erhöhten Triglyceridwert (6,1 mmol/l). Prospektiv wurden 4 weitere Fälle mit erheblicher juveniler Koronararteriensklerose überprüft. In allen Fällen fand sich eine deutliche Erhöhung des Triglyceridwertes, zum Teil kombiniert mit erniedrigter HDL-Cholesterin- bzw. erhöhter Gesamtcholesterolfraktion. In 10 Vergleichsfällen ohne Koronararteriensklerose waren die Triglyceridwerte postmortal zumeist auch erhöht, eine Überschreitung des doppelten vitalen Normalwertes (〉 4,6 mmol/l) war jedoch in keinem Fall feststellbar. Die Kombination vom morphologischen Befund einer erheblichen juvenilen Koronararteriensklerose und deutlicher Veränderung der Serumlipidfraktionen spricht demnach bei einem postmortalen Intervall von nur wenigen Tagen für eine pathologische Dyslipoproteinämie zu Lebzeiten. Diese Schlußfolgerung berechtigt nicht nur zu entsprechenden Kausalitätsaussagen, sondern sie sollte auch Anlaß sein, die Angehörigen über die eventuell bestehende familiäre Disposition aufzuklären.
    Notes: Abstract A 22-year-old soccer player was fouled and suffered from a bruised thorax. Shortly after the game he suddenly felt sick and about 2 h later he died. Apart from signs of minor injuries caused by blunt force, the autopsy revealed severe sclerosis and thrombosis of the ramus interventricularis of the arteria coronaria sinistra. The postmortal lipoprotein analysis showed a triglyceride level (6.1 mmol/l) almost three times higher than normal. In four other cases of substantial juvenile arteriosclerosis the triglyceride values were clearly elevated partly combined with lower HDL cholesterol or a raised total cholesterol fraction. In ten similar cases without coronary arteriosclerosis the triglyceride values were also higher postmortem. But in none of the cases was the level more than twice the normal level (4.6 mmol/l). After a postmortem interval of only a few days the morphological state of a substantial juvenile coronary arteriosclerosis combined with a clear change in the serum lipid fractions indicate a pathological dyslipoproteinaemia during the person’s lifetime. This combination of factors justifies not only the diagnosis, but should also be a reason for informing the relatives of the patient about a possible predisposition within the family.

    Notes: Abstract After collision of a car with the left rearside against a steel mast the 19-year-old front seat passenger was found comatose on the seat. CT imaging showed a depression fracture parietal on the left with an intracerebral haemorrhage on the opposite side. The cause of the injury was unknown to the surgeons at the time of operation. Despite neurosurgical intervention the patient died 24 h after the accident. The post-mortem showed an additional depression fracture at the base of the skull in the right temporal region arousing suspicion of an impalement injury. Only inspection of the car by the forensic pathologists revealed the gas pressure telescopic shock absorber to be the cause of the head injury.

    Description / Table of Contents: Zusammenfassung Eine 34jährige, von ihrem Ehemann getrennt lebende Frau, wurde von ihrem Mann durch Erwürgen und durch mehrere Bruststiche getötet. Die Leichenschau erfolgte etwa fünf Stunden nach der Tat. Bereits am Fundort wurden durch den Rechtsmediziner zwei Abstriche (rechte und linke Halsseite) mit angefeuchteten Wattestieltupfern durchgeführt. Zusätzlich wurde ein Abstrich von der Kragenaußenseite des Rollkragenpullovers, den das Opfer trug, angefertigt. Es war nämlich auffällig, daß sich auf der Halshaut unterhalb des Kehlkopfes ein Gewebemuster markierte, das darauf hindeutete, daß sich textiles Gewebe während des Würgevorgangs zwischen Täterhänden und Opferhals befunden hat. Die DNA-Typisierung (Short tandem repeat-PCR) dieser Abstriche ergab sowohl für die Halsabstriche als auch für den Kragenabstrich eine Mischmusterkonstellation, die mit den genetischen Merkmalen von Täter und Opfer übereinstimmte.
    Notes: Abstract A 34-year-old woman who was separated from her husband, was killed by him by strangulation and several stab wounds in the chest. The post-mortem examination was carried out approximately 5 h after death. At the place of discovery one swab of her right and one of her left neck side were taken. Additionally, a swab was taken out from the front side of the collar of her polo-neck pullover. It was obvious that the skin of the neck below the larynx showed a fabric pattern leading to the assumption that the collar was temporarily pressed between the hands of the suspect and the neck of the victim. DNA typing (short tandem repeat-PCR) of these swabs led to the detection of mixed patterns for the swabs of the neck and the collar which matched the patterns of the victim and the suspect.
