    Notes: Here we report an unusual case of primary systemic amyloidosis. The cutaneous lesions were polymorphic and included involvement of both external auditory canals. The visceral involvement was covert. Mapping of amyloid deposits was performed using scintigraphy with technetium-99m (V) dimercaptosuccinic acid ([99mTc (V)] DMSA). Therapy with melpha-lan, prednisone and colchicine resulted in considerable improvement.

    Notes: Abstract This paper considers a stochastic frontier production function which has additive, heteroscedastic error structure. The model allows for negative or positive marginal production risks of inputs, as originally proposed by Just and Pope (1978). The technical efficiencies of individual firms in the sample are a function of the levels of the input variables in the stochastic frontier, in addition to the technical inefficiency effects. These are two features of the model which are not exhibited by the commonly used stochastic frontiers with multiplicative error structures. An empirical application is presented using cross-sectional data on Ethiopian peasant farmers. The null hypothesis of no technical inefficiencies of production among these farmers is accepted. Further, the flexible risk models do not fit the data on peasant farmers as well as the traditional stochastic frontier model with multiplicative error structure.

    Notes: Abstract Background: Dose intensification and autologous stem cell transplantation as front-line therapy in non-Hodgkin's lymphoma patients (NHL) is a matter for debate, although preliminary data suggest a role for it in patients at high risk of resistance or relapse according to the international prognostic index (IPI). Purpose and study design: To compare retrospectively the clinical outcome of two cohorts of NHL patients with high-risk IPI treated with MACOP-B for 12 weeks (38 patients) or high-dose chemotherapy (44 patients) including eight weeks of MACOP-B, one or two intensification cycles with mitoxanthrone, dexamethasone, high-dose ara-C and finally BEAM chemotherapy with autologous hemopoietic progenitor cell transplantation. Results: The actuarial estimate of event (progression, relapse or death)-free survival (EFS) at three years was better (58% vs. 41%, P = 0.08) for patients treated with the intensive regimen even though the overall survival did not show a statistically significant difference (63% vs. 50%, P = 0.27). Multivariate analysis showed that the high-dose chemotherapy program was the only independent variable correlating with a reduction in the event rate. Conclusion: Early autologous stem-cell transplantation might improve the clinical outcome of high-risk patients according to IPI.

    Notes: Abstract We report the case of a patient who complained of arthralgias and arthritis 1 month before the onset of fever or other signs of infective endocarditis. In 2 months she developed an additive, asymmetrical polyarthritis with fever (febrile polyarthritis). Splenomegaly was present. Two-dimensional echocardiography showed no vegetations or other findings suggesting endocardial involvement. Initially, four blood cultures showed no microorganisms, then six of nine subsequent blood cultures grew highly gentamicin-resistantEnterococcus faecalis.

    Description / Table of Contents: Zusammenfassung Zur Behandlung der Leishmaniasis mit pentavalenten Antimon in der Schwangerschaft liegen nur spärliche Daten vor. Bei einer 39 Jahre alten Frau trat im zweiten Schwangerschaftstrimester eine viszerale Leishmaniasis auf. Die Patientin wurde in schlechtem Zustand mit hohem Fieber und Panzytopenie, die schon über 6 Wochen bestanden hatten, stationär aufgenommen. Im Knochenmarkpunktat fanden sich zahlreiche Amastigoten, die Serodiagnose war hochtitrig-positiv fürLeishmania. Die Patientin wurde erfolgreich mit Meglumin Antimonat mit einer täglichen Dosis von 850 mg Antimon behandelt. Diese Dosis liegt unterhalb der derzeit empfohlenen Dosis, doch scheint sie wirksam und nicht fetotoxisch zu sein, wenn sie im zweiten Schwangerschaftstrimester verabreicht wird.
    Notes: Summary Data on the efficacy and safety of pentavalent antimony in the treatment of visceral leishmaniasis during pregnancy are scanty. A case of visceral leishmaniasis in a 39-year-old woman in the second trimester of pregnancy is reported here. The patient was hospitalized in poor condition with high fever and pancytopenia which had lasted for 6 weeks. A bone marrow aspirate revealed numerous amastigotes and serodiagnosis forLeishmania was positive at a high titer. The patient was successfully treated with meglumine anti-moniate at a daily dose of 850 mg of antimony for 20 days. She delivered at term a healthy female baby who remains in good condition at 18 months of age. Thus a dose of 850 mg of antimony, which is lower than that presently recommended, seems to be effective and non toxic to the fetus when administered at the second trimester of pregnancy.
