    Notes: Objective To assess the relative efficacy of three disposable plastic instruments in the retrieval of ‘missing’ IUCD threads.Design A prospective randomised comparative single centre study.Setting Family Planning Clinic in London, UK.Subjects 217 of 350 IUCD users referred to the research team with ‘missing’ IUCD threads entered the study.Intervention All women initially underwent exploration of the endocervical canal with Spencer Wells forceps. When this procedure did not retrieve the threads, the patients were entered into the study. A maximum of two randomly chosen plastic IUCD thread retrievers were then used in any one patient to explore the uterine cavity and capture the ‘missing’ threads. The order in which the two retrievers were employed was also determined at random. Four separate entries into the uterine cavity were permitted with each instrument, the endocervical canal being explored with Spencer Wells forceps after each retraction of the instrument to identify the possible descent of the threads.Main outcome measures Threads brought down beyond the external cervical os, or threads brought to within the endocervical canal and then grasped by Spencer Wells forceps.Results In approximately 40% of all patients, the threads were retrieved with Spencer Wells forceps alone and a further 40% with the disposable plastic retrievers. About 5% had no retrievable threads, and only 2.5% of the referred patients required general anaesthesia for removal of their IUCD. The analysis of the comparative trial was confined to the 197 patients with retrievable threads which could not be brought below the external os with Spencer Wells forceps. The first plastic retriever used was successful in 50% of patients. The Retrievette (59%) and the Emmett (53%) performed better than the Mi-Mark Helix (37%) in this study. The difference was statistically significant (P=0.03) and the 95% confidence interval for the difference of the Mi-Mark Helix from the other two retrievers was 4% to 33%. This retrieval rate for the Mi-Mark Helix was much worse than in previously reported studies, though one doctor did have a better success rate with this retriever. The success rates, using a second plastic retriever randomly chosen from the two not used in the first attempt, were almost identical to those observed with the first retrievers: 63%, 56% and 36%. The success rate did not appear to be influenced by the length of thread, day of cycle, device type or parity. The success of the second retriever tried did not seem to be influenced by the retriever that had failed previously.Conclusions Based on our experience, the initial exploration of the endocervical canal with Spencer Wells forceps is invaluable. If this simple manoeuvre fails to retrieve the ‘missing’ threads, either the Retrievette or the Emmett thread retrievers are useful tools in general practice or in the family planning clinic setting.

    Notes: This study investigated the phenotype and function of different antigen-presenting cells (APC) present within the epidermis of patients with cutaneous T-cell lymphoma (CTCL). Involved epidermis of CTCL compared with uninvolved was found to contain increased numbers of CDI + DR+ APC. This population was heterogeneous and comprised both leucocytes of a novel CD1+DR+CD36 (OK.M5)+ phenotype and CD1+DR+CD36− indeterminate/Langerhans cells. The CD1+DR+CD36+ leucocytes did not express TcR-1, CD5, CD 15. or CD22, and only a minor population expressed CD11, demonslrating that they were neither T nor B cells, and did not belong lo the major CD11+ (OKM1+) blood monocyte population. Electron microscopy of purified CD36+ lesional epidermal cells (EC) demonstrated that they lacked Birbeck granules found on CD1+-selected Langerhans cells, and most cells exhibited features of indeterminate cells or macrophages.The capacity of EC from involved epidermis to present alloanttgens was found to be increased relative to uninvolved epidermis in all patients tested, and this capacity was critically dependent upon the presence of CD45+ DR+ bone marrow-derived cells but not on the presence of CD45+ DR+ keratinocytes. Positive selection using MoAb against CDI and CD36 demonstrated that both cell populations exhibited the capacity to stimulate T cells. The results indicate that a novel antigen-presenting cell population with a unique phenotype is present within involved skin of patients with mycosis fungoides. These cells express CD36 in addition to CD1 and have an ultrastructural appearance consistent with a dendritic antigen-presenting cell derivation.

    Notes: Summary Production of inhibitor(s) of IL-1 activity can be induced in keratinocytes by exposure to UVB. We describe in this study the characterization of an endogenous constitutively expressed IL-1 inhibitor which is present in extracts of human psoriatic epidermal keratome biopsies. Size-fractionated extracts of normal human epidermis did not reveal IL-1 inhibitory factor(s) activity in normal epidermis. Psoriatic epidermal extracts, however, contained virtually no IL-1 bioactivity and inhibited the activity of recombinant human IL-1β. This IL-1 inhibitor has a molecular weight of approximately 30 kDa and a pI of 5.3, as revealed by fast protein liquid chromatography size fractionation and chromatofocusing of psoriatic epidermal extracts. IL-1 inhibitory activity was not blocked by neutralizing anti-TGFβ monoclonal antibody. It did not have any inhibitory effect upon normal cellular proliferation but could block the IL-1 induction of IL-2 production by LBRM.33 cells as late as 4 h after exposure of LBRM.33 cells to IL-1. Thus, in vivo human psoriatic epidermis expresses an IL-1 inhibitor that specifically inhibits IL-1 activity but which appears distinct from previously described UV-induced epidermal IL-1 inhibitory activity or TGFβ.

    Notes: Abstract We have measured body temperatures and serum iron concentrations of sheep in the peripartum period following administration of endotoxin and Staphylococcus aureus cell walls. Both the rise in rectal temperature and the fall in serum iron concentration following intravenous injection of S. aureus were the same immediately pre- and postpartum as they were 5 weeks after parturition. The rise in rectal temperature following intravenous endotoxin injection immediately pre- and postpartum was significantly less than that of the same ewes 5 weeks later. However, the fall in serum iron concentration following endotoxin injection was significantly suppressed only prepartum. We conclude that fever is not suppressed in sheep in the peripartum period, but the response to endotoxin is suppressed, through complex processes incidental to the mechanisms responsible for the maintenance of gestation and induction of labour.

    Notes: Abstract When reactive metals such as molybdenum and tungsten are shaped by electrical discharge machining in the kerosene-based electrolytes generally employed for the process, they become covered with a thin, multiply-melted surface layer which contains carbide phases of variable composition. This surface layer is sometimes undesirable because its chemical and mechanical properties are different from the base metals. It has been demonstrated that the carbide can be removed by a high-temperature anneal in a reducing atmosphere. The anneal also removes surface cracks on molybdenum introduced by the machining, but the heat treatments employed here did not remove surface cracks on tungsten. The high-temperature anneal had the unfortunate side effect of inducing a significant degree of grain coarsening in the metals.

    Notes: Abstract Enterocytes from the winter flounder (Pseudopleuronectes americanus) were isolated by collagenase digestion and maintained in flounder Ringer's solution. Whole cell currents were studied using the amphotericin-perforated whole-cell patch clamp technique. The mean resting membrane potential and capacitance values or dissociated cells were-45±7 mV and 5±0.4 pF, respectively. Enterocytes held at-20 mV and treated with 1 μmol·l-1 ionomycin exhibited outward currents when cells were stepped through a series of voltages from-60 to +110 mV. The reversal potential of this current in flounder Ringer's solution was-55 mV and the voltage at which half-maximal activation occurred was +20 mV. Voltage-dependent inhibition of outward current was observed at +60 mV and above. When cells were bathed in symmetric K Ringer's solution the reversal potential shifted to zero mV and no inhibition of current was observed at voltages between-60 and 140 mV. When the holding potential of the cell was changed from-20 to-80 mV and stepped from-60 to +110 mV, a second [previously characterized, O'Grady et al. (1991)] K current with delayed-rectifier properties was identified. This observation demonstrated that the delayed rectifier K channel and the Ca2+-activated K channel described in this study exist in the same cell. Extracellular addition of 2 mmol·l-1 Ba2+ to cells bathed in symmetric K Ringer's solution resulted in nearly complete inhibition of outward current. Charybdotoxin produced only minor effects on this current. Addition of 8-Br cGMP to the bathing solution also inhibited outward current and this effect could be partially reversed following washout of 8-Br cGMP from the bathing solution. The results of this study indicated that a Ca2+-activated K conductance in winter flounder enterocytes is potentially inhibited by agents that increase intracellular cGMP. A similar effect of cGMP on a delayed rectifier K channel in flounder enterocytes was previously demonstrated.

    Notes: A new method is presented for solving elliptic partial differential equations over two-dimensional irregular regions. The scheme imbeds the irregular region in a rectangle, and then uses an alternating direction iteration to solve the resulting system of linear equations. Collocation with cubic Hermite splines is used for discretization. The method is shown to be equivalent to a multiboundary alternating direction method. A theory of convergence for a simplified case is given, details of implementation are discussed, and two numerical illustrations are presented. © 1993 John Wiley & Sons, Inc.

    Notes: A new method for solving the first biharmonic equation via a double splitting into coupled systems of ordinary differential equations is presented. The first splitting reduces this fourth order partial differential equation into the coupled system uxx + uyy = v and vxx + vyy = f, where u carries both Dirichlet and Neumann boundary data and v carries no boundary data. The pair is then iteratively solved using coupled alternating direction collocation on the resulting systems of ordinary differential equations. This can also be viewed as an alternating direction method of lines for a system of partial differential equations. Although there is no separation of variables underlying the splitting, the method yields a convergent sequence of iterates for a variety of examples under a restricted range of acceleration parameters, and possesses O(h4) accuracy. Desirable features of the algorithm are discussed together with the reduction of bandwidth of the associated collocation matrices under intersticing of u and v variables. Interesting open questions are also discussed.
