    Notes: The U.S. Geological Survey computer program TENSOR2D may be used to compute the directional components of the anisotropic transmissivity tensor of two-dimensional ground-water flow. In this note, modifications to the original computer code, developed in 1987, are described. Because of the modifications, TENSOR2D now conforms to the Fortran 77 Standard and can be run on most personal computers without user modification. Additionally, a new routine generates the polar coordinates of the computed theoretical transmissivity (or diffusivity) ellipse.

    Notes: : Simulation of ground-water flow and fate of contaminants in the subsurface environment constitutes a major phase of most environmental assessment and site remediation studies. These simulation studies yield information on spatial and temporal distributions of contaminants in the subsurface media. An important use of this information is to conduct exposure assessment studies. Spatial and temporal distributions of both chemical concentrations and exposed populations render this integrated exposure analysis task rather difficult. Geographic Information Systems (GIS), on the other hand, provide a platform in which layered, spatially distributed databases can be manipulated with ease, thereby simplifying exposure analysis tasks significantly. In this paper, we describe procedures that combine the simulation models and demographic databases under a GIS platform to automate the exposure assessment phase of a typical health assessment study. Procedures developed herein significantly simplify the post-processing phase of the analysis, and render the overall task more ‘user friendly.’ A site-specific application is included as a demonstration of the proposed process.

    Notes: : Runoff and sediment production was measured under simulated and natural rain from 1×5 m plots established on a cutover and burned mixed pine-hardwood site in the Georgia Piedmont. Trees on the study site were cut and removed without mechanical disturbance. Slash was removed, kiln dried and replaced on the slope, and burned prior to plot installation. Three slopes, two rainfall intensities, three rainfall simulations representing three soil moisture conditions, and two replicate plots were used. The experiment was repeated four times during the period July 1989-July 1990 to investigate the effects of temporal changes in surface conditions and particularly root mat and residual forest floor decemposition. Runoff and sediment production from natural rainfall events was also measured from these plots during the period February-October 1990. Results of all measurements indicate that runoff and sediment production were generally low because of the protection afforded by the residual forest floor following burning. However, temporary hydrophobic conditions caused by a dry organic layer produced relatively high runoff rates and high sediment for the first few minutes of runoff for some of the simulated rainfall applications.

    Notes: Abstract Dysspondyloenchondromatosis is a rare form of generalised enchondromatosis with hypoplastic/dysplastic changes in the lower thoracic and upper lumbar spine. The disease presents at birth as neonatal dwarfism and is characterised later in life by marked shortening of stature, unequal length of the extremities and early development of kyphoscoliosis. We report four newborn babies — three boys and a girl — with dysspondyloenchondromatosis, who had skeletal survey performed shortly after birth. The condition can be established in the newborn, as the radiographic examination (skeletal survey) shows diagnostic radiographic findings.

    Notes: Abstract Ossifying fibromas of the long bones of the leg are benign lesions occurring in the pediatric age group identical in histological appearance to the similarly named tumor of the jaw in adults. Most frequently presentation occurs after minor trauma with symptoms of a swelling of the tibia or fibula which may be painful. Pathological fracture or limp are also occasional presentations. Congenital cases are extremely rare. We describe an otherwise normal male neonate who presented at birth with a bowed right lower leg. The limb was 1 cm shorter than the other side, with tibia vara and a firm mass situated anteriorly. X-ray showed a mixed lytic and sclerotic lesion in the proximal metaphysis of the tibia. Biopsy showed collagenous stroma containing spindle cells and irregular trabeculae of woven bone rimmed by plump osteoblasts. As the appearances were typical of an ossifying fibroma (osteofibrous dysplasia) no surgical treatment was given. The patient was well with no growth of the tumor and with radiological evidence of healing at 1 year follow up. This case is presented to draw attention to the clinicopathological features of this unusual lesion which must be considered in the differential diagnosis of congenital lesions of the tibia.

    Notes: Summary In a community-based study of second-generation Japanese-American men known to have a high prevalence of both Type 2 (non-insulin-dependent) diabetes and impaired glucose tolerance, there was a highly significant association of coronary heart disease with glucose intolerance in a study sample of 219 men. Intra-abdominal cross sectional fat area determined by computed tomography was significantly elevated in men with coronary heart disease even after adjustment for glucose intolerance and body mass index (p=0.026). Other differences that were significantly related to coronary heart disease after adjustment for glucose intolerance were lower high density lipoprotein cholesterol levels (p=0.001), elevated total triglyceride and very low density lipoprotein triglyceride (p〈0.001), and elevated fasting insulin and C-peptide levels p=0.001. When these variables were tested in a stepwise multiple logistic regression model, significant independent associations with coronary heart disease were found only for total triglyceride and fasting C-peptide after adjustment for glucose tolerance status. Variables identified to be associated with coronary heart disease were interpreted as representing or manifesting an insulin resistant state. Thus, insulin resistance may be the underlying risk factor aetiologically linking glucose intolerance with coronary heart disease.

    Notes: Summary Since second-generation (Nisei) Japanese Americans are prone to develop the insulin resistance syndrome, younger third-generation (Sansei) Japanese Americans from a cross-sectional 10 % volunteer sample of Sansei men (n=115) and women (n=115) 34 years or older in King County, Washington with normal glucose tolerance or IGT were examined for metabolic and adipose risk factors associated with this syndrome. After an overnight 10-h fast, blood samples were taken for measurement of glucose, insulin, C-peptide, lipids, and lipoproteins, followed by a 3-h 75-g oral glucose tolerance test with blood samples taken for glucose, insulin, and C-peptide measurement. BMI (kg/m2), skinfolds, and body fat areas (by computed tomography) were measured. IGT was diagnosed in 19 % of the men and 31 % of the women. Men with IGT had more adiposity, both overall and in thoracic and visceral sites, had higher fasting plasma insulin and C-peptide, and tended to have higher fasting triglyceride and lower HDL cholesterol than men with normal glucose tolerance. Women with IGT had more thoracic subcutaneous fat and intra-abdominal fat and lower fasting HDL cholesterol than women with normal glucose tolerance, and tended to have higher fasting triglyceride and LDL cholesterol. Women with IGT also had higher fasting plasma insulin than women with normal glucose tolerance but tended to be less hyperinsulinaemic than men. Differences in fasting insulin, C-peptide, and lipids were best predicted by intra-abdominal fat. Thus metabolic (higher fasting insulin and a tendency to higher triglyceride and lower HDL cholesterol) and adipose (visceral adiposity) risk factors associated with the insulin resistance syndrome are identifiable among Sansei men and women with IGT, who may therefore be at increased risk of future development of NIDDM and CHD.
