    Notes: Abstract Previous studies have suggested that CT examinations of the brain in children soon after near-drowning incidents are not helpful in predicting clinical outcome and are not necessary. The clinical and CT findings of 19 pediatric near-drowning victims were reviewed for correlation with clinical and neurologic outcome. As expected, a normal initial CT scan was poorly predictive of outcome, whereas an abnormal CT scan in the initial 36 h following an immersion incident was associated with a dismal prognosis. Three children with abnormal initial CT examinations were identified and all died within 3 days of admission. A CT scan performed in the immediate neardrowning period, therefore, may be helpful in identifying some patients who have sustained severe neurologic injury.

    Notes: Summary Our study included 27 polyurethane foam workers exposed to MDI only at low concentrations (ranging from 0.0005 to 0.001 ppm) and 27 clerks from the same factory matched by age. Respiratory function tests were performed on a Monday and Friday of the same week at shift onset, 4 h later and at shift end. The subjects under study were asymptomatic for asthma. The two groups had quite similar spirometric values with minimal functional impairment. A statistical analysis was carried out by Student's t-test for matched pairs and two-way analysis of variance (ANOVA), in order to take into account both occupational exposure and smoking habits. No significant differences between the two groups were observed with paired t-tests in the respiratory parameter trend during both the Monday and Friday work shift. Nor were differences observed within the two groups when Friday's and Monday's results were compared. No significant differences between the two groups were found in paired comparisons between Friday and Monday for respiratory parameters. ANOVA demonstrated that the FEV1 and FEF25-75 reduction present on Friday, when compared to Monday, was related to smoking and not to occupational exposure. In conclusion our findings showed no short-term respiratory changes in subjects exposed to low MDI concentration.

    Notes: Abstract The effect of pressure on titanium coordination in glasses, with composition K2TiSi4O11, quenched isobarically from liquids equilibrated at high pressure (5, 10, 15, 20, 25, 30 kbar respectively) and T=1600° C has been investigated by X-ray absorption spectroscopy (XAS). The XANES spectra collected at the Ti K-edge clearly show a variation with pressure that is related to changes in the geometrical environment around the Ti atoms. By comparison with spectra of standard materials, the XANES spectra of the glasses suggest a relatively low average coordination number (near 5) in samples quenched at low pressure and a higher coordination number (near 6) in samples quenched from the highest pressure. The combination of XANES data with density and compressibility measurements supports the idea that a mixture of 6- and lower coordinated (4- and/ or 5-coordinated) Ti geometries are present in the 1 bar glass, and an increasing proportion of 6-coordinated Ti occurs in the glasses synthesized at progressively higher pressures.

    Notes: Abstract Ti K-edge XANES spectra have been collected on a series of Ti-bearing silicate glasses with metasilicate and tetrasilicate compositions. The intensity of the preedge feature in these spectra has been found to change with glass composition and varies from 29 to 58% (normalized intensity) suggesting a variation in structural environent around the absorbing atom. The pre-edge peak intensity increases for the alkali titanium tetrasilicate glasses from 35% to 58% in the order Li 〈 Na 〈 K 〈 Rb, Cs whereas for the metasilicate compositions there is a maximum for the K-bearing glass. The pre-edge peak intensity remains constant for the alkaline earth titanium metasilicate glasses, Ca and Sr (34%) but increases slightly for Ba (41%). As the intensity of this feature is inversely correlated with coordination number, a comparison of the pre-edge intensity data for the investigated glasses with those of materials of known coordination number leads us to establish a regression equation and to infer that the average coordination number of Ti in these glasses ranges from 4.8 to 5.8. Large alkali cations appear to stabilize a relatively low average coordination number for Ti in silicate melts. The Ti structural environment results appear also to vary as a function of SiO2 content within the K2O-TiO2-SiO2 system. A number of physical properties of the melts from which these glasses were quenched and of other Ti-bearing silicate melts, have been determined in recent years. Clear evidence of a variable coordination number of Ti, consistent with the interpretation of the present XANES data is available from density measurements. These and other property determinations are compared with the present spectroscopic observations in an attempt to relate structure and properties in these melts which contain a major component with variable coordination number.

    Notes: Abstract We describe a patient with methylmalonic aciduria and homocystinuria due to a defect in cobalamin metabolism of the Cbl-C type mutant (McKusick 277400). Our case was diagnosed within the first 2 months of life by amino acid analysis (ion-exchange chromatography) and by biochemical studies in cultured fibroblasts ([14C]propionate incorporation, methionine and serine formation). We discuss the clinical course and the biochemical evolution after 2 years of hydroxycobalamin treatment that led to an improvement in general clinical condition and neurological performance.

    Notes: Abstract Familial defective apolipoprotein B-100 (FDB) is a genetic disorder presenting with hypercholesterolemia and abnormal low density lipoprotein (LDL) that binds poorly to LDL receptors. This disease appears to be caused by a mutation in the apo B gene. In the present study thirteen members of a family with moderate hypercholesterolemia (250–350 mg/dl) were investigated. Biochemical studies on cultured skin fibroblasts ruled out classical familial hypercholesterolemia (receptor deficiency). LDL from nine affected members displayed, in an “in vitro” cell binding assay, a reduced affinity (2.5 fold) for the receptor, and had normal electrophoretic mobility, size and chemical composition. Lp(a) levels in family members were comparable to those present in normolipidemics and lower than those observed in primary hypercholesterolemia. The disorder is transmitted over three generations as an autosomal codominant trait and all the affected members are heterozygotes and hypercholesterolemic.

    Notes: Summary Because methylmalonic acid (MMA) accumulates in the body fluids of patients suffering from inherited metabolic disorders involving its pathway, the quantitation of this metabolite is not only fundamental to diagnosis but is also important in the follow-up of affected patients under treatment. We describe the method set up and used in our laboratory for the measurement of MMA in body fluids and propose its application to plasma as the reference method for the biochemical diagnosis and follow-up of methylmalonic acidaemias.
