    Description / Table of Contents: Résumé Les auteurs présentent une revue de 50 genoux, chez 42 malades, opérés entre août 1973 et novembre 1976. Après de bons résultats initiaux, sont apparus divers problèmes. Les complications provenant de fautes dans le dessin de la prothèse, dans le choix des malades et du fait d'obstacle techniques sont analysées. Ce type de prothèse ne convient pas aux malades dont les genoux sont très instables et déformés. La crête médiane de polyéthylène à haute densité de la pièce tibiale n'est pas suffisamment forte pour résister aux contraintes en valgus ou en varus. La correction totale du valgus, du varus ou du flessum est essentielle pour éviter les complications tardives. La déformation associée en rotation externe des genoux ayant un important valgus doit être totalement corrigée si l'on veut éviter la survenue d'une luxation de la rotule. En conclusion, à l'heure actuelle, les prothèse avec et sans charnière ont toutes deux une place dans l'arthroplastie totale du genou.
    Notes: Summary A review of 50 knees in 42 patients operated upon between August, 1973 and November, 1976 is presented. Following initial good results, problems have been encountered with the passage of time. Complications due to design faults, selection, and technical pitfalls are analysed. Patients who have grossly unstable and deformed knees are not suitable for this type of prosthesis. The high density polyethylene central flange in the tibial component is not strong enough to withstand valgus or varus forces. Full correction of valgus, varus, or flexion deformities is essential if late complications are to be avoided. Associated external rotation deformities in grossly valgus knees have to be fully corrected to prevent late dislocation of the patella. Our conclusion is that at present there is a place for both the hinged and hingeless type of prosthesis in total knee replacement.

    Notes: Abstract The paper investigates quantitative differences in the signal characteristics of ventricular fibrillation (VF) and other cardiac arrhythmias. The analysis procedure comprises two steps: calculation of a short-term autocorrelation function (ACF) followed by a regression test on a plot of peak magnitudes of the ACF against lag values (the ACF/lag plot). We detect VF by testing the hypothesis that the ACF/lag plot of VF does not pass a linear regression test. Analysis of 31 separate episodes (of VF and other ventricular arrhythmias), each comprising three successive segments of 1·5s each produced the following results: (1) 100 per cent sensitivity (Se), 62 per cent specificity (Sp) and 74 per cent test efficiency (TE) after analysis of the first segment; (2) 100 per cent Se, 86 per cent Sp and 90 per cent TE after the second segment; and (3) 100 per cent Se, 100 per cent Sp and 100 per cent TE after the third segment. This method quantifies the notion that VF signals are nonperiodic with a random amplitude distribution, whereas ventricular tachycardia (VT) signals are usually periodic with more uniform amplitude distributions. Accurate discrimination and identification of VF can be very important in intensive-care settings, as well as in the design of automatic cardioverters and defibrillators.

    Notes: The quantitative effects of substrate concentration, yeast concentration, and nutrient supplementation on ethanol content, fermentation time, and ethanol productivity were investigated in a Box-Wilson central composite design experiment, consisting of five levels of each variable, High substrate concentration, up to 30° Brix, resulted in higher ethanol content (i.e., up to 15.7% w/v or 19.6% v/v) but longer fermentation time and hence lower ethanol productivity. Increasing yeast concentration, on the other hand, resulted in shorter fermentation time and higher productivity. The highest ethanol productivity of about 21 g EiOH/L h was obtained at low substrate concentration (i.e., 12° Brix), low alcohol content (i.e., 6% by weight), high yeast concentration (i.e., 4.4%), and high supplementation of yeast extract (i.e., 2.8). Productivity of this magnitude is substantially higher that that of the traditional batch fermentation of fed-batch fermentation. It is comparable to the results of continuous fermentation but lower than those of vacuum fermentation but lower than those of vacuum fermentation. Optimal conditions for maximal ethanol productivity can be established by a multiple regression analysis technique and by plotting the contours of constant response to conform to the constraints of individual operations.
