    Notes: Summary We studied insulin receptor binding and carbohydrate metabolism in 10 patients with severe hyperprolactinaemia and compared the findings with those obtained in 20 healthy control subjects. Insulin binding to monocytes and erythrocytes was significantly decreased in the patients with an excess of prolactin. Scatchard analysis of binding data indicated that a decrease in the number of receptors rather than in receptor affinity seems to be the prevailing cause of lowered binding in hyperprolactinaemic patients. Furthermore, patients with severe hyperprolactinaemia demonstrated significantly elevated blood glucose levels following oral or intravenous glucose load despite having significantly increased insulin levels after glucose administration. The infusion of insulin induced a delayed hypoglycaemic effect and a decreased inhibition of endogenous insulin secretion, as indicated by the suppression of C-peptide in the hyperprolactinaemic patients. The present data indicate that severe hyperprolactinaemia is associated with an insulin-resistant state, which seems to be caused, at least in part, by a down-regulation of insulin receptors.

    Notes: Summary Bay m 1099 is a newly developed inhibitor of intestinalα-glucosidase. Its ability to lower postprandial plasma glucose, serum insulin and C-peptide levels in Type II diabetics has been investigated. Fifteen obese Type II diabetic patients with inadequate metabolic control during sulphonylurea treatment received a standardized diet and were treated either with Bay m 1099, b.d. (100 mg before breakfast and dinner) or placebo for 3 days, according to a double-blind cross-over design. The postprandial blood glucose level was significantly lower during Bay m 1099 treatment compared to placebo after breakfast and dinner (AUC after breakfastp〈0.001). The reduced postprandial hyperglycaemia was associated with a decrease in meal stimulated serum insulin and C-peptide levels. Thus, Bay m 1099 may be a useful addition in the treatment of Type II diabetic patients.

Die Verimpfung des Herpes zoster-Bläscheninhalts auf die Kaninchencornea zeitigt im Gegensatz zum Herpes febrilis und Herpes corneae keine charakteristische makroskopische Reaktion. 2. Die mit Herpes zoster geimpfte Cornea ist gegen eine neuerliche Impfung mit Herpes febrilis nicht immun. 3. Die Allgemeininfektion des Kaninchens mit Herpes zoster ist weder bei cornealer noch intravenöser Verimpfung von Bläscheninhalt, Lumbalflüssigkeit und Serum des Patienten gelungen. 4. Die von Lipschütz als Zosterkörperchen beschriebenen Kernveränderungen konnten in den Zosterbläschen der Haut, nicht aber in der mit Zoster geimpften Kaninchencornea nachgewiesen werden. 5. Die Kernveränderungen der Hautefflorescenz werden entsprechend analogen Befunden bei der Herpes febrilis-Keratitis usw. als degenerative aufgefaßt. 6. Das mikroskopische Bild der mit Zoster geimpften Cornea ist durch das Auftreten von Ödem, Riesenzellen, ballonierender Degeneration, intra- und subepithelialem Infiltrat gekennzeichnet.

    Notes: Abstract The diagnostic value of different laboratory methods in detecting Chlamydia trachomatis infections in high risk groups was analysed. The efficiency of a direct specimen test was compared with serology (IgG and IgM ELISA) and culture in L929 cells, stained either with fluorescein conjugated monoclonal antibodies or with iodine. Patients (no. = 1041) with localized genital infections attending a STD clinic, sexual contacts and patients with ascending infections from urological and gynecological clinics were examined. Chlamydia trachomatis was detected in 225 patients: 210 (93.3% were reactive in the direct test (smears stained with monoclonal antibodies), whereas culture missed only 5 (sensitivity 97.8%) when stained by the same method. Cultures stained with iodine produced the lowest recovery rate (73.8%), but this rate increased to 80.9% when a second passage was performed. In addition the prevalence of Neisseria gonorrhoeae, Mycoplasma hominis, Ureaplasma urealyticum, Candida albicans and Trichomonas vaginalis was investigated. In patients with non-gonococcal urethritis (no. = 331) and cervicitis (no. = 353), Chlamydia trachomatis was isolated in 32.3% and 12.8% respectively. However, this pathogen could be isolated in only 3 (15.8%) out of 19 patients with epididymitis and 15 (14%) out of 107 patients with adnexitis, although 66.7% and 93.3% respectively had specific IgG antibodies. Specific IgM could by detected with a sandwich ELISA in patients with adnexitis (46.7%), epididymitis (33.3%), cervicitis (22.2%), non-gonococcal urethritis (14%) and in the sexual partners of patients with genital infections (35.7%). The direct specimen test with monoclonal antibodies is the method of choice for the diagnosis of a C. trachomatis infection in patients with urethritis and cervicitis. In ascending infections of the genital tract, however, even culture may give negative results, and serology may be of diagnostic assistance.
