    Notes: Summary A 55-year old woman with a history of herpes zoster in the dermatome supplied by the mandibular branch of the trigeminal nerve developed cutaneous red papules and umbilicated nodules within the same segment. The clinical and histological diagnosis was pseudolymphoma. The lesions showed a polymorphous infiltrate without germinal center formation. Immunologic phenotyping with monoclonal antibodies revealed the predominance of helper T cells and distinct compartmentalization of B and T cells. The lesions healed up within 7 weeks. The development of pseudolymphomas at the site of previous herpes zoster eruptions seems to be extremely rare.

    Notes: Abstract The wellknown biconcave shape of resting RBC reflects an intrinsic overall negative spontaneous membrane curvature. It depends to a high degree on the integrity of the spectrin-actin-ankyrin-Band-3 hetero-complex. Alterations of this complex have previously been shown to be associated with shape transitions, which have been abolished by treatment with enzymes reducing the surface charge of the RBC. In this report we show that treatment of erythrocytes with enzymes reducing the surface charge (e.g. neuraminidase, trypsin, chymotrypsin and pronase), consistently exerts a “stomatocytogenic” effect, i.e. it reduces mean mean curvature. Also the time dependency for the charge reduction and for the correlated decrease of mean mean curvature is shown. So-called stomatocytogenic agents (e.g. clorpromazine, tetracain and triton X100) and so-called echinocytogenic agents (e.g. dinitrophenol and Na-salicylate) are known to change membrane curvature in a dose dependent manner. It is further shown that by prior reduction of surface charge by various enzymes the dose response curves of all stomatocytogenic and echinocytogenic agents tested is shifted towards higher degrees of stomatocytosis or lesser degrees of echinocytosis. The data show, that in RBC pronounced curvature influences are produced by the surface charge located on the sialic acid residues in the glycocalix.

    Description / Table of Contents: Zusammenfassung Von 52 Rekonstruktionen der A. vertebralis (bis 31. 3. 1986) waren 6 im cervicalen Knochenkanal lokalisiert. In 73 % lag eine Stenose und in 27 % eine Kink-king vor. Zur weiteren Sicherung der Diagnostik und der Operationsindikation wurde der Einsatz von Dilutionssonden nach Applikation eines Technetiumbolus überprüft. Patienten mit Verschlußprozessen im cervicalen Abschnitt wurde einsubclavio-vertebraler Venenbypass komplikationslos angelegt. Die Nachuntersuchung von 48 Patienten ergab 3 Verschlüsse und 4 Stenoserezidive.In 3 Fällen wurde nach 3 Jahren ein neurologisches Defizit beobachtet. Die Rekonstruktion der hohen Verschlußprozesse waren komplikationslos.
    Notes: Summary Of 52 reconstructive operations of the vertebral artery (until 31 March 1986), 6 were situated in the cervical vertebral foramina. In 73 % of cases we found stenosis, while 27 were caused by kinking. We improved the diagnostic reliability and the assessment of the indications for surgery by injecting a bolus of technetium, and examining the quantitative differences between both sides and the hemodynamic changes. In cases of occlusion of the cervical part of the vertebral artery, a subclavian-vertebral bypass was implanted without causing complications. The longterm postoperative examination of 48 patients revealed recurrent stenosis in 4 and complete occlusion in 3 patients. After 3 years we observed neurological deficits in 3 patients. The reconstruction of high vertebral artery stenosis was without complications.

    Description / Table of Contents: Zusammenfassung Von 537 tumorösen Lebererkrankungen waren 187 benigne. Das mittlere Erkrankungsalter lag bei 39 Jahren. mit Dominanz des weiblichen Geschlechts (2:1). Patienten mit einer focalen nodulären Hyperplasie (FNH) oder Adenomen (Ad) nahmen in 80% Ovulationshemmer länger als 2 Jahre und über 70% länger als 5 Jahre ein. Durch Sequenzszintigrafie, Blutpooling, dynamisches CT (Kontrast) und Feinnadelbiopsie konnte in 87% die richtige Artdiagnose gestellt werden. Von 101 benignen Lebertumoren Ad. FNH Hämangiome (HÄ) wurden 41 reseziert und 64 weiter beobachtet. 9 von 10 nachgewiesenen Adenomen wurden durch Hemihepatektomie bzw. atypische Resektion entfernt (3mal Malignom). In 12 Fällen mit einer FNH (n = 38) bestand vor der Operation eine ungeklärte Dignität. Von 34 HÄ wurden 19 cavanöse H. reseziert. Die Letalität betrug 4,5 % (2 Pat.)
    Notes: Summary Of 537 patients with tumorous diseases of the liver, 187 had benign liver diseases. In this group (mean age 39 years) the female sex dominated by 2:1. Of the patients with focal nodular hyperplasia (FNH) or hepatocellular adenomas (HA) 80% had used oral contraceptives for more than 2 years, 70% for more than 5 years. Improved diagnostic methods (such as sequential scintigraphy, blood pooling, dynamic computer tomography, sonography and thin needle biopsy) enabled the correct classification of 87 of the tumors. Of 101 benign liver tumors (FNH, HA and hemangiomas), 41 were resected while 64 were observed for a longer time. Most (9 of 10) proven HAs were resected either by hemihepatectomy or by atypical resection. In 3 of these cases we diagnosed malignancy. We could not classify the tumor preoperatively in 12 of 38 cases of FNH.

    Description / Table of Contents: Zusammenfassung Von 1979 bis 1986 wurden an der Chirurgischen Klinik der Charité 190 Patienten wegen einer chronischen Pankreatitis operiert. Bei 52 Patienten mit einer cephalen Duodenopankreatektomie wurde in einer 6jährigen Verlaufsstudie die endokrine Pankreasrestfunktion verfolgt. Die eingesetzten Parameter (GTT, Serum-Insulin, C-Peptid, Sonographie, CT und Leberbiopsie) ergaben einen signifikanten Anstieg der diabetischen Stoffwechsellage, eine gesteigerte Verdauungsinsuffizienz und eine deutlich zunehmende Leberverfettung. Durch die Occlusion wird keine endokrine Schutzfunktion erkennbar.
    Notes: Summary Operations were performed on 190 patients for chronic pancreatitis at the Surgical Hospital of the Berlin School of Medicine (Charité) between 1979 and 1986. The residual endocrine function of the pancreas was followed up for 6 years on 52 patients who had undergone cephalic duodenopancreatectomy. The parameters used (GTT, serum insulin, C-peptide, sonography, computed tomography, and liver biopsy) revealed a significant increase in diabetic metabolism, deterioration of digestive ability, and unambiguous aggravation of fatty degeneration of the liver. Occlusion failed to provide any signs of endocrine protective function.

    Description / Table of Contents: Zusammenfassung Unter 2093 Lungenresektionen von 1968–1983 wegen eines Bronchial-Ca. fanden wir 354 kleinzellige Bronchialcarcinome (SCLC). Die Resektionsquote betrug 16,9 %. Die Pneumonektomierate beim SCLC lag bei 52,5 %. Die p.op. Letalität betrug 4,9%. Von den 265 Patienten, die bis 1977 wegen eines SCLC reseziert wurden, überlebten im Stadium Ia 43 % die 5-Jahresgrenze. In den fortgeschrittenen Stadien waren die 5-Jahresüberlebensquoten gegenüber dem Gesamtkollektiv aller Resezierten um die Hälfte reduziert. Die Resektionsbehandlung des SCLC im Stadium I wird befürwortet. In den fortgeschrittenen Stadien wird die radiologische und Chemotherapie empfohlen.
    Notes: Summary 2,093 patients suffering from lung cancer underwent pneumonectomy from 1968 to 1983. 354 of them where small cellular bronchial carcinomas (SCLC). Resections were done in 16.9 %. In cases with SCLC pneumonectomy were performed in 52.5 %. The postoperative mortality was 4.9 %. In staging by TNM-system 5 years survival rate in stadium I was 43 %. In stadium III the 5 year survival rate was decreased to 20 %. This justifies the resection in cases with SCLC in TNM-stadium I. In another stadium radiological therapy or chemotherapy is recommended.

Bei 19 Kindern erfolgte die Varicensklerosierung. Zu Rezidivblutungen kam es 7mal und 2mal trat eine Oesophagusstenose auf. Der distale splenorenale Shunt (Warren) wurde 16mal angelegt. Alle Kinder überlebten, postoperativ trat eine Pneumonie und eine leichte Pankreatitis auf. Viermal wurde bei der Nachuntersuchung ein Shuntverschluß gefunden. Blutungsrezidive traten nicht auf.
    Notes: Summary From 1980 to 1984 fortyeight children with portal hypertension were treated, 37 out of these had a prehepatic bloc. In 19 children a sclerotherapy was performed. Seven times a bleeding recurrency occurred and an esophageal stenosis was seen in 2 cases. The distal splenorenal shunt (Warren) was performed 16 times. All children survived, one pneumonia and one slight pancreatitis were observed postoperatively. Four times a shunt obstruction was found at follow-up examinations. Bleeding recurrencies did not appear.
