    Notes: Twenty cases of onychomycosis were treated with systemic or topical antifungal agents and surgical avulsion of part of the nail plate. This technique is suitable for onychomycosis of limited extent and results in reasonable remission rates while reducing the time needed for systemic therapy. The patients treated presented with either a dermatophyte infection (12) or Candida paronychia (8) with secondary nail plate invasion. The classification of nail disease caused by fungi is discussed and a number of modifications are suggested.

    Notes: With the exception of acropustulosis, nail disease responds poorly to etretinate.When used to treat other dermatological conditions etretinate is frequently toxic to the normal nail and may affect any or all of its epidermal components.The desquamative dermatitis, induced by etretinate, may account for many of the drug's side-effects on the nail. These include chronic paronychia, onycholysis, onychomadesis, nail shedding, onychoschizia, fragility and the formation of excess granulation tissue. Some of these changes resemble those found in psoriatic paronychia and similarities are drawn between psoriasis and the retinoid dermatitis which presents with identical features in the discharge from beneath the proximal nail-fold.The desquamative reaction in patients taking etretinate always affects the lips and, in addition, may be widespread or mainly localized to the nail apparatus.

    Notes: Nail infections caused by Candida species are normally associated with chronic paronychia or chronic mucocutaneous candidiasis (CMCC). However, the role of Candida in the pathogenesis of other primary nail dystrophies has been questioned in view of their response to antifungal therapy alone. In the present study of 86 patients with primary nail dystrophies from which Candida was isolated, three patterns of nail involvement were found. Nineteen patients, of whom 17 had CMCC, had total dystrophic onychomycosis of at least two nails. The second group consisted of 27 patients with paronychia and lateral onycholysis. In a further 40 patients, who did not have paronychia, Candida was isolated from nails showing primary distal and lateral onycholysis. These changes were mainly seen in patients with peripheral vascular disease, particularly Raynaud's disease, or Cushing's syndrome. Nail biopsies from patients in the latter two groups confirmed the presence of yeasts and mycelium in the nail plate and 17 (46%) of those receiving antifungal therapy with ketoconazole or itraconazole showed complete clearance of the nail dystrophy. Good responses to therapy were seen more frequently in patients with peripheral vascular disease or Cushing's syndrome of whom 15 (72%) recovered; distal erosion of the nail plate, mycelium in the nail plate on biopsy or direct microscopy together with the isolation of C. albicans were all associated with good responses to antifungals. In addition to patients with CMCC or paronychia, therefore, Candida appears to be a significant pathogen in some patients with primary onycholysis of the finger nails, particularly where there is underlying peripheral vascular disease or Cushing's syndrome.

    Notes: We have studied 15 patients with photo-onycholysis induced by tetracycline derivatives, trioxsalen and fiuoroquinolones.1 Histological examination was performed in five cases. Three distinct clinical sub-types of onycholysis were seen, a classification justified by their different appearence.In Type I (nine cases) several fingers showed the separating part of the nail plate to be half-moon shaped, concave distally with varying pigmentation proximally. Type II (three cases) demonstrated on just one finger a well-defined, circular notch, widest distally and with a brownish hue proximally; the shape was as if the distal nail plate had acted as a convex lens focusing the light on the hyponychium and the distal nail bed. Type III (three cases) showed on several fingers such changes in the central part of the pink nail bed with no connection to the distal or lateral nail margins. This type shows the link between photo-onycholysis and subungual photo-haemorrhage. The latter represents an aspect or a sequel of photo-onycholysis and depends on the timing.UV-irradiation of normal fingernails with various wavelengths (313, 340, 360 and 400 nm) showed that 3–20% of the radiation could penetrate. The different patterns of photo-damage are due to the fact that the nail acts as a lens. Less protection through lack of melanin, and absence of sebum and a stratum granulosum favour penetration of UV-radiation and explain why the skin is not always affected. Discrete subungual hyperkeratosis resulting in keratin dust also explains some changes, such as nail dyschromia, so far attributed to haemorrhages or melanin deposition.

    Notes: We describe the clinicopathological findings in five cases of an acquired vascular tumour involving periungual or subungual tissue. We believe that these represent acral arteriovenous tumours (cirsoid aneurysms). There are no previous reports of subungual or periungual acral arteriovenous tumours.
