    Notizen: There is an explosion of interest in the role of Campylobacter pylori as a cause of active chronic gastritis. This curved spiraled microorganism can readily be detected within the mucus gel covering the stomach mucosa, especially in patients suffering from peptic ulcer disease or non-ulcer dyspepsia. To what extent this intriguing microorganism is causally related to peptic ulcer disease remains to be elucidated, but all the evidence which is available so far supports a pathogenetically important role. There appears to be a striking discordance between in-vitro sensitivity and in-vivo efficacy of antibiotic therapy. At present, the combination of colloidal bismuth subcitrate and amoxycillin or tinidazole appears most effective in temporary elimination of these microorganisms.

    Notizen: Eighteen patients with duodenal, gastric or jejunal ulcers, resistant to at least 3 months treatment with histamine H2-receptor antagonists, singly or in combination with other anti-ulcer drugs, were treated with 40 mg omeprazole once daily for up to 8 weeks. All ulcers healed, the majority within two weeks. After ulcer healing patients were given maintenance therapy with high doses of cimetidine or ranitidine. Of 15 patients on maintenance therapy with H2-receptor antagonists, 12 (80%) developed a relapse after a period ranging from 3 to 52 weeks. Two patients were lost to follow-up. After re-healing on 40 mg omeprazole, two patients were given 20 mg omeprazole daily as maintenance therapy but relapses occurred again after 14 and 26 weeks respectively. After re-healing on 40 mg omeprazole, these two patients and one additional patient received maintenance therapy with 40 mg omeprazole daily. At present these three patients have been relapse-free for periods varying from 16 to 52 weeks. No side effects were registered during treatment with omeprazole. It is therefore concluded that omeprazole is highly effective in healing refractory peptic ulcers and that omeprazole maintenance therapy may be useful for prevention of relapse.

    Notizen: Abstract The merits of the Nd-YAG laser were evaluated in palliation of obstruction, or bleeding, or both, in 84 patients with colorectal malignancy. Luminal patency was restored in 20 of the 24 patients with obstructing colonic cancers. Haemostasis was achieved in 37 of the 40 patients with bleeding. Combined bleeding and obstruction responded favourably in 19 of the 20 patients treated. Only a few laser photocoagulation sessions, over a short period of time, were needed, and further laser treatment could be continued on an outpatient basis. Major complications such as perforation (6), stenosis necessitating a colostomy (3), and delayed post-treatment bleeding (2) were seen in 13% of patients, with a laser-related mortality in three patients. Minor complications, such as transient stenosis (22) and laser-induced bleeding (11), responded well to further laser application. Pain and warmth (20) resolved spontaneously. Treatment failures and complications were mainly due to inadequate accessibility or were related to tissue characteristics. Laser photocoagulation appears to constitute a valuable alternative treatment, not only to resection and colostomy, in the case of obstruction, but also for haemorrhage that cannot be controlled in other ways. Palliation, in its broad sense, is guaranteed because of the rapid results, reduced time in hospital, the avoidance of hospital admission, and the good tolerance of treatment.

    Notizen: Abstract It is disappointing that we are still ignorant of the cause or cure of Crohn's disease, despite a tremendous amount of research in recent years. Any hypothesis has to explain the hallmarks of the disease such as its widespread distribution, its predilection for the ileocaecal and perianal area and its granulomatous nature. Most frustrating in all aetiologic studies is the impossibility of distinguishing primary events from epiphenomena. Epiphenomena may be secondary to alterations in gut content, leaky mucosa with excessive absorption of antigenic material, secondary to microbial invasion or general ill health. No valid reasons have been found to explain the increasing incidence rate in the Western industrialized world [1], the high incidence amongst teenagers and the increased incidence in first-degree relatives.

    Notizen: Summary Endoscopically placed biliary endoprostheses were used to treat obstructive jaundice in 71 patients with ampullary carcinoma. Successful placement of an endoprosthesis was achieved in 68 patients (95.8%). Bilirubin declined in 67 patients (98.5%). There was no procedure-related mortality. Twenty-two patients (31%) received further surgical therapy, and 47 received an endoprosthesis as their only therapeutic intervention. In the latter group, bilirubin normalized in 44 of 46 patients surviving longer than 30 days (95.7%). Mean survival was 466 days (median 410, range 23-1515), which compares favorably with surgical palliation. Complications mainly involved clogging of the endoprosthesis, which was easily treated endoscopically and, more significantly, duodenal stenoses secondary to continued tumor growth in almost 25% of the patients. Although endoscopic drainage is a safe and effective method of relieving biliary obstruction in patients with ampullary carcinoma, we feel it should be reserved for poor surgical candidates and for those patients with a limited life expectancy due to metastatic disease.

    Notizen: Summary Endoscopically placed biliary endoprostheses were used to treat obstructive jaundice in 64 patients with advanced or recurrent gallbladder carcinoma. Successful placement of an endoprosthesis was achieved in 55 patients (86%). Bilirubin declined in 52 of 55 cases (94.5%) and normalized in 37 of 44 patients (84%) who survived more than 30 days. Procedure-related mortality was 3.1%. The thirty-day mortality of 14.5% was better, and the mean overall survival of 161 days was comparable to published surgical results. Due to the lower cost, improved patient tolerance, and reasonable survival, we consider endoscopic drainage to be the procedure of choice in patients with obstructive jaundice secondary to recurrent and unresectable gallbladder cancer.

    Notizen: Abstract Excessive Enterogastric reflux following partial gastrectomy is believed to be responsible for bilious regurgitation, vomiting, nausea, and epigastric pain. At endoscopy, striking erythema and inflammatory changes of the gastric mucosa may be seen. The nonsurgical treatment for this syndrome is unsatisfactory. Because of the potential pathogenetic role of regurgitating bile acids, lysolecithin, and pancreatic secretions, it seemed relevant to find out whether prostaglandin E2 (PGE2) in a dose of 0.5 mg qid could protect the gastric mucosa from further damage and thereby lead to symptomatic improvement. The results of this controlled doubled-blind crossover trial, comparing PGE2 and placebo, in the treatment of postgastrectomy reflux gastritis reveal no significant differences between PGE2 and placebo with regard to symptoms, endoscopic features, and histologic evidence of inflammatory changes. Thus, prostaglandin E2 in the dose used appears incapable of improving postagastrectomy reflux gastritis in patients with mild to moderate degrees of this entity.

    Notizen: Abstract The endoscopic insertion of an endoprosthesis is now a standard, procedure in the ultimate palliation of malignant obstructing upper gastrointestinal and biliary malignancy. The commercially available prostheses and introducing devices are adequate for the majority of upper intestinal, cancers. For some stricturing lesions, especially when associated with fistula formation, individual adaptation of a tygon prosthesis with extra widening rings is often necessary. Nd: Yog laser vaporisation of mainly exophytic cancerous tissue is mainly indicated for those circumstances which are less amenable to prosthesis insertion such as total luminal obstruction, noncircumferential tumorous involvement, polypoid cancers, excessively necrotic and chronically bleeding tumors, lesions extending within 2 cm of the upper esophageal sphincter, markedly angulated cancers of the cardia with almost horizontal tube positioning and cancerous overgrowth occluding, the funnel opening. Overall successful insertion occurs, in over 90% of patients. Main complications are perforation 5–8% and early or late dislocation. The procedure related mortality fluctuates around 2 to 4%. Overall, results with laser application are roughly comparable. The dysphagia free intervall after laser is only around 6 weeks for the majority, of the patients. Transpapillary insertion of a straight Amsterdam-type prosthesis rapidly became a standard procedure for palliation of malignant jaundice. For many patients with pancreatic cancer this endoscopic approach competes favorably with corresponding surgical palliative alternatives. Disappearance of jaundice is to be expected in the vast majority of the patients. The only major unsolved problem remains late clogging with biliary sludge which necessitates insertion of new prostheses. Most problematic to breach are bifurcation tumors. Cholangitis is a major complication if one does not succeed at the first attempt to drain both liver lobes.
