    Notes: Summary Water-soluble contrast media, which have been employed in the examination of the lumbar spinal canal for 30 years, can sometimes result in severe complications, such as arachnoiditis, epileptic seizures, and myoclonic spasm. These complications have been seen less frequently since the advent of metrizamide, a new non-ionic water-soluble contrast medium. However, a further neurological complication has now arisen-mental disorder, varying in kind and degree; for example, organic psychosis and perceptual disturbance. Some characteristic cases are described. A series of 75 patients, all of whom received varying amounts of metrizamide for the purpose of cervical, thoracic and lumbar myelography, has been studied. Seven patients were found to have a pronounced organic psychosis and three patients had visual illusions or hallucinations. It is suggested that there is some degree of correlation between the total amount of iodine received and patient age. By careful enquiry and examination, similar disturbances were also found some 4–8 h after myelography with other water-soluble contrast media.

    Notes: Summary Lumbar myelography was carried out with the contrast media Amipaque, Dimer X and Myelografin in 10 patients each. Five of the patients treated with each contrast medium were kept in a sitting position after the examination, the others lay flat. Blood levels and excretion were measured up to 24 h. The results are interpreted as follow: 1. After lumbar injection of the contrast media there is a short phase of distribution in the subarachnoid space (lag time) and they then are transferred into the blood with a half-life of 3.9±2.4 h. The transport from the CSF is almost completed approximately after 24h. The velocity of transport varies greatly between the individual patients. Watersoluble contrast media presumably flow passively with the CSF through the arachnoid villi into the venous blood. 2. The horizontal position of the patient reduces the lag time until the beginning of the actual transfer of the contrast medium. 3. The transfer of Dimer X begins somewhat later compared with Amipaque and Myelografin.

Dieses System besteht aus einem Kunststoffbehälter, der über einen Luftschlauch mit einer Vacuumpumpe verbunden wird. Es können mit diesem Einmalgerät vom Chirurgen 500ml Blut aufgesaugt und durch den Anaesthesisten wieder reinfundiert werden. Das System wurde bei venösen Thrombektomien, Bauchaortenaneurysmen und in der Shuntchirurgie bei portaler Hypertension eingesetzt. Bei 9 Operationen konnte dadurch auf eine Fremdblutzugabe verzichtet werden.
    Notes: Summary At the surgical unit of the University of Cologne a new system for autotransfusion (Solcotrans) was used 56 times in 14 patients. This system consists of a plastic case that is connected with a vacuum pump by an air tube. After filling the plastic case with 30 ml of sodium citrate, 500 ml of blood can be aspirated by the surgeon and reinfused by the anaesthetist. The system was used in venous thrombectomy, aneurysm of the aorta abdominalis and in shunt surgery by portal hypertension. Because of this system donor blood was not used in nine operations.

Dieses System besteht aus einem stabilen Kunststoffbehälter, der einen flexiblen Kunststoffbeutel enthält. Nach Aspiration von 70 ml Natrium-Citrat-Lösung können bis zu 500 ml Blut aufgesaugt und durch den Anaesthesisten unter Druck reinfundiert werden. Bei 25 Operationen konnte dadurch auf eine Fremdblutgabe verzichtet werden. Das System eignet sich zum Einsatz in der Gefäß-, Abdominal- und Unfallchirurgie. Blutqualitätskontrollen wurden durchgeführt.

This system consists of a rigid plastic case with a plastic bag inside. After filling the plastic bag with 70 ml of natrium citrate solution, 500 ml of blood can be aspirated by the surgeon and reinfused under pressure by the anesthetist. Through this stored blood was not necessary in 25 operations, the set is well suited for intraoperative autotransfusion in vascular and abdominal surgery and in cases of trauma. Blood quality examinations were carried out.
