    Notizen: Summary Following surgery for the removal of intrarenal calculi 30% of patients will develop severe wound complications. This study assesses the role of a short course of netilmicin given over the operative period, in 20 consecutive patients. Preoperative studies demonstrated that whilst 20% of patients had bacterial growth in the urine, culture of the removed calculus demonstrated that 50% of the stones exhibited a significant bacterial growth. Of the 20 patients, only one developed a wound complication. Following the preoperative dose, satisfactory netilmicin levels were obtained and no patient had levels 〉1 μg ml-1 by 7 h. Despite some degree of renal impairment, serum urea and creatinine values were unaffected.

    Notizen: Summary 65 men undergoing elective transurethral resection of the prostate under the care of one urologist entered a randomised controlled trial using Cefuroxime. Patients were excluded if they had received antibiotics in a 2 week period before surgery, if they had positive pre-operative urine cultures or if they had been catheterised. 58 patients completed the study. One patient in the antibiotic group and 2 in the control group developed urinary infection. The overall infection rate was 5.17%. The control and antibiotic groups were comparable as regards age and the duraction of resection. One patient in the antibiotic group developed an allergic reaction. It is felt that routine antibiotic prophylaxis in patients with sterile urine undergoing elective TUR is unnecessary.

    Notizen: Summary Experimental high velocity missile brain injury in the rhesus monkey produces widespread swelling of perivascular astrocytes within 30 min of injury. Possible mechanisms for this lesion include a direct effect of force, chemical mediation secondary to the extravasation of blood, alterations in the permeability of the blood brain barrier and ischaemia. The implications of this finding for the function of the blood brain barrier, for neurotransmission and for neuronal survival are discussed.
