Zusammenfassung Die aneurysmale und juvenile Knochencyste zeichnet sich durch ihre Rezidivfreudigkeit aus. Dies gilt sowohl im Fall der Spontanfraktur und anschließender konservativer Behandlung, als auch für die einfache Excochleation mit Spongiosaplastik. Aufgrund unserer Erfahrung muß die Excochleation durch mehrfaches Anbohren der Cystenwand und Abfräsen der Compactaleisten erweitert werden. - Im Falle des sogenannten Rezidivs oder bei Cystenvergrößerung muß die radikale En-bloc-Resektion mit Osteoplastik und Osteosynthese durchgeführt werden. Mit diesem standardisierten Vorgehen konnten vier Problemfälle ausgezeichnet gelöst werden. In einem Fall führte die verkürzende Resektion zum Ziel. Die Spätergebnisse nach 5 1/2 Jahren sind sehr gut.

Summary Aneurysmal and juvenile bone cysts are characterized by frequent recurrence, and pathological fractures caused by such bone cysts also recur frequently. Curretage of the cyst and spongiosaplasty is often unsuccessful. In our experience, excochleation must be followed by drilling the cyst wall and fraising the compact substance. Densely packed homologous spongiosa must then be implanted. In case of recurrence or if the cyst increases in size, radical en bloc resection is indicated and the defect must be bridged by osteoplastic and external fixation. We have successfully treated four difficult cases with this procedure. In one case, resection with limb shortening led to full recovery. Late results after 5.5 years are very good.
