Zusammenfassung Die akute Oesophagusvaricenblutung wird heute durch Sklerosierung behandelt. Für das freie Intervall empfehlen wir, abhängig von der Leberhämodynamik, eine differenziertes Vorgehen: bei hohem Portalanteil an der Gesamtleberdurchblutung (über 20 %) führen wir eine partielle (periphere) Druckentlastung mit einem Warren-Shunt oder einer coronario-cavalen Anastomose durch. Bei geringer bzw. fehlender portaler Leberdurchblutung sind der porto-cavale Shunt bzw. die alleinige Sklerosierung indiziert. Mit diesem Konzept haben wir bessere Früh- und Spätergebnisse erreicht.

Summary Massive haemorrhage from oesophageal varices can be stopped by endoscopic sclerotherapy. For the free interval, we determine our procedure in accordance with liver haemodynamics: if the portal liver blood-flow is high (above 20 %) we perform a selective decompression technique with the Warren-shunt or a coronariocaval shunt. If the hepatic portal blood-flow is very low or absent we prefer complete portal decompression with a portocaval shunt or use only sclerotherapy. With this scheme we have good early and late results.
