Notes: The wealing response of human skin to dimethyl sulphoxide (DMSO) has been quantified. Concentrations of 90%, 95%, and 100% were applied for 5 min to circular areas 8 mm in diameter. Wealing was scored on a five-point scale after 10 min.Marked individual variations were found. Wealing was strongly influenced by body region and was dose-dependent. The intensity of the reaction paralleled that to other irritants and was dependent mainly on thickness and integrity of the horny layer. The DMSO test is a simple, quick way to assess the barrier function of the stratum corneum.

Notes: The occurrence of low molecular weight (〈 500 dalton) eosinophil chemotactic factor (ECF) was studied in the blister fluid of patients with various dermatological diseases by an in vitro chemotaxis method. After chromatographic separation of the test substance on a small Sephadex G 25 column, ECF was demonstrated in blisters of five patients with bullous pemphigoid, in three patients with systemic drug reactions and in one patient with blisters after contact with primula. ECF was not found in blisters of patients with epidermolysis bullosa and herpes gestationis or in DNCB-induced blisters. Fluid obtained from suction blisters was negative in normal controls and on unaffected skin of patients with atopic eczema, but contained ECF in eczematous areas. The demonstration of ECF in tissue fluid suggests that this factor plays a role in the local accumulation of eosinophils at sites of certain inflammatory reactions.

Notes: A new human model for the evaluation of the atrophogenicity of topical corticosteroids is proposed. Formulated corticosteroids (creams and ointments) were applied occlusively to normal forearm skin of human volunteers for 3 weeks using Duhring chambers. Visible atrophy and telangiectasia were rated under a stereomicroscope on a histologically validated five-point scale. There was a close correlation between the parameters of atrophy and telangiectasia. Atrophogenicity varied considerably among the tested corticosteroids, ranging from none (1% hydrocortisone) via moderate (0.1% betamethasone valerate) to very severe (0.05% clobetasol propionate). A dose-response relationship with exposure time and concentration of the corticosteroid could be demonstrated. Furthermore, the vehicle influenced the resulting atrophy and telangiectasia. Fatty ointments seem to pose less risk than oil in water creams.

Description / Table of Contents: Summary Food allergies are to be considered strictly separate from other food incompatibilities, which are not caused immunologically. Mostly allergic symptoms are caused by cow's milk or chicken eggs. Allergic reactions are preceded by sensitizing events; certain characteristics of foodstuffs and conditions in the human body facilitate their development. Gastrointestinal symptoms very often are just accompanying signs. In differential diagnosis the so-called “pseudo-allergies” following food ingestion have to be separated. Most important diagnostic measures are clinical history, prick-/scratch test, RAST, gastrointestinal provocation and abstinence test. The therapeutic program consists of allergen abstinence, avoiding all allergy-arousing factors, oral desensitizing and pharmaceutical treatment.

Notes: Summary By means of a lengthy method of continuous cytologic and histologic study, an experimentally produced reactive process in lymphnodes was analyzed. This analysis was made both qualitatively and quantitatively, in an attempt to clarify the mechanisms of reaction and the underlying cellular alterations producing this change. The reaction begins in the medullary cords and gradually spreads to the cortex. New cells are formed. This cytopoesis, considered “accidental”, quantitatively exceeds the physiologic process, and depends upon stimulation of the reticulo-cellular system. The cells of this “accidental” cytopoesis are characteristically very variable in form. A constant transition of forms occurs. This dynamic cell, as it is seen in the reactive formation of new cells, can be observed only by continuous study. The small reticulum cell proves to be a particularly adaptable and plastic element, and is the real basis of the reaction. Large and small lymphocytes, as well as transitional forms and plasma cells are derived from it. For these cell types, no other “parent” cells can be postulated. The reactive formation of new cells reaches its maximum between the 20th and 26th day, and then shows a steady decrease in spite of further “stimulation” of the lymphnode. During the course of the reaction numerous quantitative interrelationsships among the individual groups of cells can be found. The conclusions gained from the cellular preparations are substantiated by means of histological controls and supplemented by a few important other points. The observation of the cytologic and histologic cellular movements, of the quantitative interrelationships among the individual groups of cells, and of the continuous change in form of the cells during the reaction are made the basis for the survey of the cytopoesis in lymphatic tissue. The large reticulum cell is considered to be the “parent” cell for the accidental formation of cells in reactive processes. The cellular supply is maintained probably by means of hemihomo-hemiheteroplastic reproduction. The small reticulum cell with the highest mitotic rate apparently reproduces itself in the homoplastic way. Those cells derived from it are responsible for the polymorphous picture of cells in the reactive processes of the lymphatic tissue. The theories of determinate developmental processes and the retrograde transformation of cells in lymphatic tissues are rejected. Both the bud-like formations on the nuclei of the small reticulum cells, and the appearance of dwarfed nuclei, in conjunction with the quantitative relation to the number of binucleated reticulum cells, suggest a mechanism of growth which shows similarity to the karyonomia or meroamitosis.
