Zusammenfassung Frühe Formen des kolorectalen Carcinoms entstehen auf dem Boden eines Adenoms. Über Erfahrungen mit 147 Adenomen des Rectosigmoids, von denen 20 schwere Zellatypien oder invasives Carcinom enthielten, wird berichtet. Nach lokaler Excision bei 11 Patienten wurde kein Carcinomrezidiv beobachtet. Das Operationsrisiko ist bei diesem Vorgehen deutlich geringer. Als alternatives Verfahren zur radikalen Operation bietet sich bei Frühformen des colorectalen Carcinoms die lokale Excision an.

Summary Experiences with 147 adenomas localized in the rectosigmoid were reviewed. 20 adenomas contained severely atypical cells or invasive carcinoma. 11 patients were treated by local excision. Only one recurrence of adenoma took place within 5 years after operation. From these results one can say that local excision of early colorectal carcinoma is an alternative method to radical operation and shows good results.
