    Description / Table of Contents: Zusammenfassung Indikation: Jüngere, nicht shuntfähige Patienten mit rezidivierender Blutung trotz Sklerosierungstherapie. Technik: Stumpfe Dissektion der Speiseröhre ohne Thorakotomie wie von uns beim Carcinom beschrieben (Chirurg 1980, 51:296). Bisher operierte Patienten: 16, davon 4 Notälle bei unstillbarer Blutung (2) und Oesophaguslängsperforation (2). Letalität: 1 von 12 und 3 der 4 Notfälle. Ohne den natürlichen Verlauf der Lebercirrhose beeinflussen zu können, scheint mit geringerem Operationsrisiko die Rezidivblutung vermeidbar als mit Sperr- oder Shuntoperationen bisher möglich.
    Notes: Summary This procedure is indicated in young patients, for whom the shunt operation is not possible and who suffer from recurrent bleeding despite sclerotherapy. The technique is blunt dissection of the esophagus without thoracotomy as described earlier (Chirurg 51, 1980, 296). To date 16 patients have been operated on, 4 as emergencies in heavy bleeding (2) and longitudinal perforation of the esophagus (2); lethality was 1 of 12 and 3 of the emergencies. Although this method does not influence the natural course of cirrhosis, recurrent bleeding can be avoided with less risk than from transsection or shunt operations.
