Zusammenfassung In 9 Jahren konnten wir unter 75 Fällen mit iatrogenin Läsionen der extrahepatischen Gallenwege bei 37 der uns zugewiesenen Kranken insgesamt 40mal die bilio-biliäre End-zu-End-Anastomose durchführen. Bei 3 Patienten war unsere Anastomose nach Entfernung der T-Drainage restenosiert. Bei richtiger Mobilisation können bis zu 4 cm lange Defekte des Hepatocholedochus überbrückt werden. Die Nachuntersuchungen zeigen in über 80 % ausgezeichnete Erfolge. Die geglückte End-zu-End-Anastomose ist jeder anderen Korrektur-Maßnahme entschieden überlegen. Bei langstreckigen Defekten im Leberhilusbereich wird eine eigene Operationstechnik angegeben.

Summary We have performed bilio-biliary end-to-end anastomosis 40 times in 37 of the 75 patients referred to us during 9 years for iatrogenic lesions of the extrahepatic biliary tract. In 3 patients our anastomosis was restenosed after removal of the T-drain. With proper mobilisation, defects of the hepatocholedochus up to 4 cm long can be bridged. Follow-up examinations showed excellent results in over 80%. Successful end-to-end anastomosis is decidedly superior to all other correction measures. An operative technique of our own for extensive defects in the region of the hepatic hilus is given.
