    Notizen: Summary Twenty-four hour profiles of plasma C-peptide an index of endogenous insulin secretion, were performed in 15 Type 1 (insulin-dependent) diabetic children. Plasma C-peptide was detectable in six children, of whom four (‘C-peptide producers’) had peak values above normal fasting levels. In each of the six children with residual B cell function, there was a close correlation between plasma C-peptide and simultaneous blood glucose (r〉 0.50, p〈 0.05). Post-breakfast peak blood glucose was 10.2 ± 1.7 mmol/l (mean ±SEM) in the ‘C-peptide producers’ and 18.7 ± 1.7 mmol/l in the 11 children with low or no detectable C-peptide. Mean M-value, an index of deviation from an ideal blood glucose, was lower in the ‘C-peptide producers’ (p〈0.05). It is concluded that residual functioning B cells in diabetic children behave physiologically in that insulin secretion fluctuates in accordance with the prevailing blood glucose; and that the pattern of action of injected insulin is more critical in non-C-peptide producers who lack the post-prandial dampening effect provided by residual endogenous insulin secretion.

    Notizen: Summary Endothelial cell function in Type 1 (insulin-dependent) diabetic patients, both with and without retinopathy, was assessed by measuring the plasma fibrinolytic activity and fibronectin after 10 min venous stasis induced by a sphygmomanometer cuff. After venous stasis, diabetic subjects with proliferative retinopathy had fibrinolytic responses (median 0.13 increasing to 0.26 U/ml) in the low normal range, which were significantly less (p〈 0.005) than control subjects (0.17–0.68 U/ml) and diabetic patients with minimal retinopathy (0.16–0.68 U/ml; p〈0.01). Plasma fibronectin levels were similar in the different groups, but after venous stasis, rose significantly in the diabetic patients, both in those with proliferative retinopathy (mean 317–399 μg/ml; p〈0.002) and without retinopathy (312–371 μg/ml; p〈0.05) but not in normal subjects (304–333 μg/ml). These changes in fibrinolytic activity and fibronectin were independent of blood glucose, glycosylated haemoglobin, or indices of sensory or autonomic nerve function. These disturbances of endothelial function, together with known abnormalities of haemostatic variables and microvascular reflexes, might convert a usually temporary obstruction of capillary blood flow into a pathological capillary closure, and might contribute to the inexorable progression of advanced diabetic microangiopathy in spite of good diabetic control.

    Notizen: Abstract Annual coastal land loss in the sedimentary deltaic plain of southern Louisiana is 102 km2, which is correlated with man-made canal surface area. The relationships between land loss and canals are both direct and indirect and are modified by the deltaic substrate, distance to the coast, and availability of new sediments. Loss rates are highest in the youngest of the former deltas nearest the coast; they are lowest in the more consolidated sediments far from the coast. The average estimate for land loss at zero canal density in the six regression equations developed was 0.09%±0.13% annually, the present land loss rates approach 0 8% annually Although additional analyses are needed, we conclude that canals are causally related to a significant portion of the total coastal land loss rates The relation probably involves an interruption of local and regional hydrologic regimes. Reduction of the present acceleration in land loss rates is possible by managing present canals more effectively, by not permitting new ones, and by changing the design of new canals to allow more natural water flow

    Notizen: Summary The properties of two sodium-dependentd-glucose transporters previously identified in renal proximal tubule brush border membrane (BBM) vesicles are studied. The low-affinity system, found in BBM vesicles from the outer cortex (early proximal tubule), is shown to be associated with the high-affinity phlorizin binding site typically found in renal BBM preparations. The high-affinity system, found in BBM vesicles from the outer medulla (late proximal tubule), is almost two orders of magnitude less sensitive to inhibition by phlorizin and is apparently not associated with high-affinity phlorizin binding. The sodium/g;ucose stoichiometry of the outer medullary transporter is found to be 2∶1 by two independent methods. Previous measurements have established that the stoichiometry of the outer cortical system is 1∶1. It is suggested that this arrangement of transporters in series along the proximal tubule enables the kidney to reabsorb glucose from the urine in an energy-efficient fashion. The bulk of the glucose load is reabsorbed early in the proximal tubule at an energetic cost of one Na+ per glucose molecule. Then in the late proximal tubule a larger coupling ratio and hence a larger driving force is employed to reabsorb the last traces of glucose from the urine.

    Notizen: Summary The stoichiometric properties of the renal outer cortical brush-border membraned-glucose transporter are studied. Experiments which establish the glucose/sodium, glucose/phlorizin and phlorizin/sodium stoichiometries are reported. Three independent methods of determining the substrate/activator (glucose/sodium) stoichiometry for coupled transport systems are presented and discussed. One of these, the “Static Head Method,” is introduced here for the first time. This type of experiment appears to be more generally applicable than the usual procedure of directly measuring the coupled fluxes of substrate and activator to determine stoichiometric coupling ratios. The results presented in this paper demonstrate that the glucose/sodium/phlorizin stoichiometry of the renal outer cortical brush-border membraned-glucose transport system is 1∶1∶1.
