Zusammenfassung Ein Kollektiv von 68 Patienten mit akuter Exacerbation einer chronisch obstruktiven Atemwegserkrankung wurden nach der Höhe des IGV, ausgedrückt in % eines für die Rt errechneten IGV-Erwartungswertes (IGV % E), in 3 Gruppen unterteilt und die Gruppen auf Unterschiede des übrigen Befundmusters, der klinischen Behandlungsprognose und der Zuordnungsmöglichkeit ätiologisch definierter Krankheitsbilder untersucht. Bei gleichem Ausmaß der Atemwegsobstruktion und vergrößertem IGV wurde ein hohes IGV % E (≥125%, Gruppe I) als Korrelat des Emphysemgrades bzw. emphysembedingten Elastizitätsverlustes, ein indifferentes IGV % E (91–124%, Gruppe II) als überwiegend funktionelle Lungenblähung und ein niedriges IGV % E (≤90% Gruppe III) als Ausdruck einer restriktiven Komponente bewertet. Die Größe von IGV % E war über den Behandlungszeitraum gruppenspezifisch konstant. Gruppe I zeigte vor Therapiebeginn den niedrigsten Broca-Index, die höchste Pulsfrequenz, den niedrigsten PaO2, den höchsten PaCO2, die kleinste VC, die kleinste relative Sekundenkapazität, die geringsten zirkadianen Spontannachschwankungen der Rt, das schlechteste Ansprechen im pharmakodynamischen Test und die höchste Beobachtungsfrequenz von EKG-Kriterien eines chronischen Cor pulmonale. Gruppe I hatte auch die ungünstigste Behandlungsprognose gemessen an der Höhe der Funktionswerte zum Zeitpunkt des Therapieoptimums, der Besserung über den Behandlungszeitraum und der stationären Behandlungsdauer. Bei Gruppe III waren wegen der heterogenen Zusammensetzung im Mittelwertvergleich mit Gruppe II charakteristische Unterschiede, außer bei der relativen Sekundenkapazität, verwischt. Die Brauchbarkeit des gewählten Einteilungsprinzipes, im Vergleich zu anderen Klassifizierungsversuchen chronisch obstruktiver Atemwegserkrankungen, wird diskutiert.

Abstract 68 patients with acute exacerbation for their chronic obstructive airway disease were divided in three groups by means of the intrathoracic gas volume (IGV). The measured IGV was expressed in percent of an expected value (IGV % E), which was estimated for each measured total airway resistance (Rt). In these three groups other lung functional patterns, clinical prognosis and relation to etiological factors were tested. While the mean Rt showed no significant differences between the groups, we correlated a high value of IGV % E (≥125%, group I) with a high degree of substantial emphysema respectively high lost of lung elastic recoil, an indifferent value of IGV % E (91–124%, group II) with the prevalence of a functional emphysema, and a low value of IGV % E (≤90%, group III) with an additive restrictive component. During the period of treatment the amount of IGV % E was constant for each group. Before treatment group I had the lowest Broca-index, the highest pulse rate, the lowest PaO2, the highest PaCO2, the smallest VC, the smallest FEV1 % VC, the smallest spontaneous circadiane variation of Rt, the lowest reaction after inhalation of a bronchodilatator and the highest prevalence of signs of a chronic cor pulmonale in the ECG. Group I had the worst prognosis in the sense of its lung functional values at the end of clinical treatment, degree of improvement during treatment, and duration of treatment. Besides the FEV1 % VC no significant differences between group II and III were observed, this because of the heterogenous composition of group III. The importance of the choised division criterion compared with other kinds of classification in chronic obstructive airway disease was discussed.
