    Notes: Summary The principle of the pierced concave reflector is used since Hoffmann (1841) almost unchanged to the present day in ENT examination and operations. During a long course of development the reflector changed from hand to head. There is no fundamental difference between the frontal mirror and the modern head light. In this system of reflecting technique the physician serves unconciously as tripod. Long use gives disadvantages in terms of cervical spine syndrome and multiple discomfort of the vertebral spine. Since for physical reasons the development of head reflector lights can be considered as completed a new lighting system is presented to replace the frontal mirror. The supporting function of the physician is taken over by an especially designed tripod fulfilling the condition of floating adjustability of the light. A lense projector produces a light quality and intensity by far superior to the frontal mirror. The new lighting system having nothing in common with the tripod lights of the past century will facilitate the ENT specialist in his surgical and outpatient work and represents a true alternative to the frontal mirror.
